BAD...DON'T STAY THEIR
I was moved there after back surgery. I went their to recover/rest. I was still weak when I arrived in the afternoon. A PT (physical therapy) person came to my room and demanded me to come to their shabby PT room. It was so dis-organized and not clean. The hospital area that my room was located was mostly empty. The next morning a different PT came to my room and once again demanded that I go to the PT room. I stated that I was in too much pain to participate in physical therapy at that time. She objected and stated that I might have to leave the hospital if I didn't do the physical therapy. I asked for the administrator to come to my room. THEY DIDN'T HAVE ONE! I agreed to go to the PT room that afternoon. It was so bad (they were using students) that I decided not to go back. The next morning when I refused the physical therapy all hell started. I was resting in my room when suddenly a guy appeared in my room stating all kinds of nonsense. He stated that their facility was first class. I let him rant for a few minutes. I then told him that I was there to recuperate from my surgery not physical therapy. I told him that I was leaving now at 9 AM. He stalled giving me what he called important paperwork until 1 PM. That gave them another day to bill because I stayed past 12 noon.
I have since found out that they were shut down at least once for poor performance.

The staff from the doctors to the nurses to the therapists were outstanding. Their care and concern literally gave my husband back his life. The insurance company wanted to send him to a nursing home from the hospital, but I appealed and he was sent here. During his 20 days here, he was stabilized medically thanks to the doctors and the therapists helped him clear the fog from his brain and get him back on his feet.

The absolute most unprofessional, careless place I...

The absolute most unprofessional, careless place I have ever encountered in my entire life. Absolute incompetence in every possible way. I cannot even begin to describe the extreme level of incompetence and total indifference by their unprofessional and pathetic staff. They should be forcibly closed. The following "care" was provided to an 80 year old referred for a 3-10 day stay from Medical Center of Plano. Medication, walker, oxygen, and breathing treatments not provided as prescribed, and when the indifferent staff was alerted of the problems multiple times, the "nurse" simply shrugged her shoulders and informed us that we were "on our own". That literally happened. Was then given oxygen that would not reach the bathroom, when the 80 year old patient took off the oxygen to use the bathroom without a walker she nearly collapsed. HealthSouth then attempted to bully the patient into signing insurance and payment forms and would not address any concerns of the patient. Finally, family arrived and called a private ambulance to transfer the 80 year old patient back to Medical Center of Plano given the unbelievable and completely unacceptable treatment and incompetent, indifferent staff. When the private ambulance arrived, a third party private contractor who happened to be at HealthSouth at the time confirmed the entire situation and my account of events that took place. Every available avenue will be pursued to hold this organization accountable for it's treatment of this patient. Avoid at all costs.

The staff was wonderful! The doctors and nurses were great. They listened intently to my issues, analyzed my problems and setup my rehabilitation requirements. The cafeteria provided delicious hot meals. The pharmacist came by on the day I arrived and the day prior to my departure. The rehabilitation therapists were knowledgeable, engaging and truly cared about my welfare. This location provides onsite dialysis. Every room I viewed was clean and sanitized. My two week stay was glorious!!!

My mother was admitted for rehab from home due to ...

My mother was admitted for rehab from home due to falls. At time of admission we went over her correct medication list multiple times and finally I signed her final list.
while I was visiting her, nurse asked her for pain Med and she said give me Tylenol please ,next thing the nurse came in room and told her I have hydrocodone for you 10/325.
She went to rehab from home due to falls and dizziness and at home she has never been on opioids or on benzos.I was very upset that how come a 70 yr old opioid naive patient is on 10 mg of hydrocodone when she doesn t take it at all at home and doesn t have too much pain and is not acute post op from a major surgery.I requested to see her Med list and found out that she was on norco 10mg every 4hrs prn pain and norco 20 mg every 6 hrs prn pain. She was also on temazepam 30 mg prn at night. She doesnt take Temazepam at home as well.Giving opioids and benzos together to a 70 yr old opioid naive patient when there is a FDA black box warning and CDC warning against concurrent use of opioids and benzos is criminal negligence.Just such high dose of opioids to a 70 ur old opioid naive could be life threatening.Upon further investigation I found out that at night she was given hydrocodone 10 mg and in morning she was given 20mg of hydrocodone and that s why she was dizzy all day.
This is very dangerous practice and this rehab should be investigated before it causes serious damage to patients and cause some lives.I talked with the physician and nurse manager Alex Kiume as I wanted to file a formal complaint.
Alex told me to send him an email and someone will reach out to me.
No one has reached out to me nor I was given paper work to file formal complaint.
Seems like Alex asked me to send him the email so he can then delete it.

The things served to patients which are called "food" should be enough to embarrass any institution. I realize that a rehab center isn't a restaurant, but what you serve to patients should cause you to reconsider why you entered the health care profession to begin with. What's wrong with you people?

It is unethical to enforce their own home health agency Encompass even when patients are transferred to them from another agency. Even when they want go back to the agency of their choice. They should change their practice to be fair to other providers in the community.

Many of the people working here are great caring p...

Many of the people working here are great caring people who are doing a fantastic job. But I can't give it more than 3 stars for a couple reason. One problem is unless you are the patient it is extremely difficult to talk to a Doctor. The Doctor makes his rounds before visiting hours start, so even if you have time to sit in the room all day long you are never going to see him. But when I checked my Mother in Dr Colon gave me his card with his voice mail phone number if I had any questions. That sounded great! Except when I called that number about a week later I found the number had been disconnected. Nobody I talked to at the hospital seemed to care that the number had been disconnected . I even left a voice message for the Director of Quality on the phone number issue and he never even responded. What kind of quality control is that? So to talk to the Dr I had to call the main hospital number . They asked me what I wanted before they would transfer the call, after they determined they would allow me to try to talk to a Doctor they transferred me to nurse. Again I had to repeat my story and convince a nurse that should be allowed to ask the Doctor a question. Then all the nurse could do is to give the Doctor a message with my question. The Doctor never called me back, a P.A. named Lilly called me back and gave me incorrect information. The Doctor could have given me the correct info, but I guess he was to busy to spend 5 minutes on the phone with low level person like me. The second issue I have with the place is that as they approach the end of the time period that Medicare likes to pay pay for they decide patient is suddenly cured and can go home. On the first day they wrote a goal in the wall for my Mother to get out of bed on her own and walk, just like she could before her surgery. She certainly did not reach that goal, but they did not appeal to Medicare that a 86 year old woman might need a little more rehab time than a typical patient. They just discharged her and hoped that "Home Health" could finish the job with visits a couple of times a week. Even though they had not accomplished it working with her everyday. To summarize If I could have talked to a doctor when I wanted to and if they had kept my Mother another week until she reached her "goal" I would have given them a 6 star rating.

The worst experience for my mother was there. They lost her dentures, came back 10 folds worse than when she get on, I will if there cameras to be able to see what they did for her.
Bad place for your loved ones

My father was placed here after an ER visit to reg...

My father was placed here after an ER visit to regain his strength. While in the ER his CT showed no signs of stroke. However, while in their care he had symptoms of a stroke which went undetected by the doctor, who saw him daily for 12 days according to the staff. These same symptoms went undetected by all nurses, rehab and all staff here. I was on vacation when he was admitted. But, when I returned I noticed these symptoms myself at this point he had been in rehabilitation for almost a week. His head always down, eyes closed and the inability to hold his head up. Inability to walk without maximum assistance or some days not at all and staying in bed. At this point the case manager told me they were releasing on Monday. This was on Friday. I was frightened and expressed my concern that he was so far from his baseline. She moved his release by 1 day to Tuesday and I hustled to get 24 hour care set up at home. Worried all that time about what was going on. Tuesday, I noted to the nurse he had slurred speech, or aid whichever it was, and that fell on deaf ears. They proceeded with his discharge and I felt helpless. Essentially he went from rehabilitation to the ER within a few hours of his release from Encompass and was diagnosed with a cerebral bilateral stroke that was old as the bleeding and swelling had stopped. This stroke that was not detected on the CT 12 days before now shows on the CT and confirmed with an MRI. 12 days without any medical personnel recognizing he had a stroke? Please avoid this rehabilitation hospital if at all possible. If they can't see what a non medical or lay person can see and it is scary!

I just got discharged yesterday and this place is awesome!!! I had not been able to walk for 9 months. On my first day PT got me on my feet again. Everyone there would smile and help with your concerns. I loved that they had props for you to practice. For example they had restaurant booths and tables so if you go out to eat with friends you would know how to adapt. I had muscle wasting and they would stretch my muscles to help me with my walking. The day before I got discharged I had walked the length of a football field. Everybody there was great from PT, OT, nurses Dr and staff. If anyone needs rehab I would suggest going there.

A little upset that when we called the day before they made it seem that my grandmother would get a private room, that it wouldn't be an issue. The next day guess what? No private room available. We would have picked a different place if we knew that she wouldn't get a private room from day one. I'm surprised they even have non private rooms in this day and age. Just my 2 cents.

My husband was a patient there twice. The staff was compassionate, caring, and capable. He left both stays much better than when he came in. Little problem with the correct diet on the first visit, but that was corrected.
From the front desk to the nurses, therapists, techs, case workers, all were helpful and caring. I was very confident while my husband was there that he was well cared for and safe.

my husband from unable to walk nor feed himself, he is now able to walk with walker, able to use his right hand take care of himself and talk more clearly. I want to thank the PT Tosha, OT Krystal, and SPT Erica job well done.

I recently had a second back surgery and after receiving exceptional care the first time Requested to be sent back directly from Baylor Uptown to Encompass Plano.
The service/ care for over 12 days was just as I remembered. From the Nurses, Physical Therapist, Front Desi, Food Service, Nurses Aide, and Maintance staff were not only Professional but Caring when meeting my overall needs towards my recovery.
I would not hesitate to recommend Encompass to both family and friends if ever a need arose.
I am now home and continuing with home care through Encompass.
Felt it was necessary to share my Second experience and if I return it would just be as a visitor,,
Thank to All

My husband was there for about 10 days to 2 weeks after a fall that caused a head injury. Unfortunately after he came home later he passed away from a stroke. but that had nothing to do with his stay there. I am a retired RN and cant say enough good things about the place. Everyone was competent, extremely caring and the therapists were wonderful. they went above and beyond in his care. I even got one free meal a day as his wife so that I could eat with him. He was busy everyday with therapy so that he could recover his motor functions. They had the best social worker that I have ever run across. She was truly amazing. She always responded as quickly as she could and her work load was huge. She went way beyond what I was used to. I am just so sorry that these few people had such a bad experience. My husband was there last summer--2016.
