This place is sad and overall very depressing.

My grandmother is very observant of how she and other patients are treated. MY GRANDMOTHER WAS HERE FOR A SMALL INJURY, SHE DOES NOT SUFFER FROM ANY MENTAL ILLNESS which means SHE IS FULLY AWARE OF ALL SHE SEES, FEELS AND HEARS. With that being said, there are certain employees, nurses or assistants that treat the elderly differently.

My grandma shared how certain individuals from this facility treated her very rude and talked to her in a very disrespectful tone as if she's not aware of how she's being treated.

Like in everything, there are those who genuinely love what they do and show care and we are thankful for them! BUT the percentage of those people who love what they do is typically very low.

Those that are cold and mean to the elderly, should rethink their career and also remember that one day they will also be old.

And this facility, the management should take action.

I understand that it might be hard to find genuine nurses/assistants that love what they do, BUT there should be a system in place where the nurses/assistants are monitored or observed to ensure that patients are treated with respect and dignity.

Hello to family members who care for their parents, if your family member is aware mentally and they need attention when they push a button then don't come to this place, if they're used to eating basic good food, don't come to this place some of the staff members are very nice but this place is very disorganized and don't check in on a Friday because you will not get someone from nutrition until Monday afternoon, one room smells of urine and another room had ants, we had to search for toilet paper twice and I had to drive home to get a pain medicine for my mom on my way back they called and said they were sorry but they forgot to transfer it from the urine room to the ant room. I came in one day and she did not have her diaper on and she is incontinent and she has a whole package of diapers with her in her closet but for some reason she did not have them on . I don't like to write reviews like this but this is very important because it's a place that takes care of our loved ones so I felt compelled and so we checked our mother out and I did so because I was worried about her safety these are my sincere honest thoughts as a Christian man please take these reviews, all reviews seriously , there Street address is 911 Bryant Street I found that interesting, God heal you and bless you, blessings from a loving caretaker and son

Horrible 1 star snake pit. CNA unskilled. 1 hour wait times to go too bathroom. 1 shower in 15 days. CNA sleeping on the job. RN, a couple of trained. My favorite, the CNA asked me if she was going to get paid because of the Gov. Shutdown.

My mother just stayed here for a couple of months and Palo Alto Sub Acute did an ok job managing her care and she liked the food. However, I DO NOT recommend sending your loved one to this place. I will first acknowledge that we are living in the time of COVID-19 and that our essential workers are angels doing the best they can. Thank you to everyone on staff for coming to work each day to care for our loved ones. That said, as a family member the center failed miserably in the lack of warmth and communication with the family. Every communication was a struggle, every phone call was a run around, and not once did I speak to a doctor about my mother's care even after repeated requests. My bedridden mom felt shamed asking for help during her stay. The discharge was sloppy and my mom did not come home with clear and helpful support and instructions for the family, nor did we receieve the home equipment you promised to order upon discharge. Perhaps Palo Alto Sub Accute should do more satisfaction surveying with their clients so that feedback like this doesn't have to be publically posted. Some quality improvement measures need to be put in place.

DO NOT SEND A LOVED ONE THERE. THIS PLACE IS A DEATH TRAP. FACILITY IS VERY, VERY RUN DOWN, TERRIBLE "MEDICAL" STAFF.
MY HUSBAND WAS TRANSFERRED THERE W/ THE SUGGESTION OF STANFORD HOSPITAL, SO THOUGHT IT WOULD BE A GOOD SKILLED NURSING FACILITY. I WAS WITH HIM FOR 4 1/2 HOURS AND NOT ONE PERSON CAME IN TO EVALUATE HIS CONDITION, TAKE A PULSE, BLOOD PRESSURE...
I TOOK HIM OUT OF THERE 4 1/2 LATER TO SAVE HIS LIFE. NOT ONLY DID STANFORD ALLOW HIM TO BE TRANSFERRED THERE, BUT REPORTED THE FACILITY TO BE CLEAN, AND SKILLED. NEITHER OF WHICH THIS PLACE IS.
IN MY OPINION, PALO ALTO SUB-ACUTE IS A SOCIAL SECURITY MONEY GRAB, WHICH YOU READ HORROR STORIES ABOUT SIMILAR PLACES. I AM MAKING IT MY GOAL TO SHUT DOWN THIS TERRIBLE PLACE.

This place is a dump. DO NOT go there or send loved ones. This picture is very deceiving. Horrible meals, FILTY, lack of staff, untrained staff, no therapy what so ever. They don't even have a wheel chair or a walker to assist patients . You don't want to be dead in this place, let alone needing therapy. -0 star. NO CARE

I personally am not a patient there but I have visited and patients are at their mercy of the short staffing and lack of Patient Care. Their are only a few staff members that have empathy and compassion for patients and theses staff members deserve recognition. Nurses are asked to work a Double shifts, when after 10 hours a nurse's patient care is greatly impacted. The food is atrocious. Everyone gets the same food and I use that term lightly. When patients lift the cover it is most times unrecognizable as food. Vegetables are over cooked and mushy There are no specific menus for those who need special diets. The portions are very small. How is a patient to get strong and recuperate without the proper nutrition? This is supposed to be a Sub-Acute Facility but I do not believe this facility know what Sub-Acute means. People go without daily baths and most times the patients are only wiped down with a warm wet towel. How do you control infections and cleanliness that these poor souls need? There is no one on one PT. They give you exercises to do and then go get another patient. No one watches to see if the patient is doing the exercises correctly. There seems to be little one on one care throughout the whole facility. I would not wish this place on my worst enemy. If I could give them less than 1 Star, I would.

Short on staff to attend to patients. Restricted diet on release orders not even taken into account. Everybody gets same food. Patients are not turned or moved so as not to develop bed sores.
