Our mother has spent the last two months at Meadowbrook Care Center. She has been blessed with having one of the most exceptional physical therapist we have ever had treat our mother. Kerry O Hara is a truly dedicated professional who treated our mother with the utmost respect and patience. Kerry inspired our mother to never give up through her words of encouragement and wonderful sense of humor. Kerry always returned our calls in a timely manner. She took the time to personally write out a comprehensive discharge exercise plan and showed us how to safely transfer our mother. Kerry is passionate about helping her patients reach their therapeutic goals and is very compassionate! Ms. O Hara goes above and beyond and makes this world a better place! Meadowbrook Care Center is fortunate to have such a special and extremely qualified physical therapist on their staff. We are forever grateful for all that Kerry has done for our mother.

Scott and Sherrie have been fantastic and helpful in transitioning my Mom into your facility. The staff is unbelievable with the finer details for the care for her. In such an unfortunate situation it has been so much easier for me and my family. I want to thank everyone involved.

I am very alarmed that I can call the nurses station at any given time through out the day and I can rarely ever get to speak to anyone in Unit 1A. This makes no sense to me you have several nurses on hand 24 hours and it is ridiculous that the phone just continuously rings. I understand if they are doing rounds with patients, but not one person is there to answer the phone. I called this morning and was transferred over 8 times and the phone just rang between 6:00 am - 7:15 am and finally security walked over. They were all standing around talking listening to the phone ring. That is UNACCEPTABLE!!!!

I wanted to share a review about my brother-in-law s first few days at Meadowbrook care Center in Freeport. We had experienced another facility when he last took sick a few months ago. Not knowing anything about nursing facilities I figured that this was the norm. It was absolutely horrible so this time when I decided to pick a location I decided to pick a location close to me and that was closer to his house and easier for us to visit. All I have to say is if anybody is looking for a place for care for their loved ones, this is the place for you and for them.

All the departments I have spoken to go out of their way to make your experience as a caretaker and a patients experience a great one. The physical therapist was unbelievable, the dietitian was a pleasure to deal with, and then There is Daniel, the nurse an absolute delight!

When a loved one gets sick and you only want the best for them it is such a trying experience,
one that leaves you with worries and concerns. I can only say that every day when I go to see him and I leave, I leave feeling like breath of fresh air has come over me when I get on my car. I am at total peace.

In a scale of 1-10 this place is a 20. Your staff is incredible. My special appreciation to all of you for making your patients feel like they are important.

No one answers the bells that they have
You have a tall black female supervisor who was very rude to me
Nurses tell you that they are going on their break and can t help you
They lose your laundry when it goes out
The laundry takes forever to come back
Physical therapy was great
The food was great
The two girls Kiana and Leah on the concierge team are great
They don t force the mask mandate with the visitors which is upsetting. People s lives are worth something. I m going to call the state on them regarding this first thing tomorrow.
I have witnessed the nurses not saying anything to these vile people about masks

I would like to express my sincere appreciation for Sherry Ann, who has been outstanding in helping me as I begin the process of applying for Medicaid for my mother, a resident at Meadowbrook.. Sherry is extremely knowledgeable, and is very clear in her explanations. She is making a laborious task quite manageable. I am very grateful for her and I look forward to working with her in the months ahead. Thank you so much, Sherry.

I also would like to thank both Dr. Jaglall and Barbara (3A Unit Manager), who have been so caring, respectful, and responsive to me over this past year. As busy as they always are, they make time for my questions and concerns, and have never made me feel as if I am bothering them. Thank you so much, Dr. J and Barbara.

Delivering quality healthcare is difficult for a number of reasons. These challenges are exacerbated in the care for our senior population. Economics, family expectations, staffing and more make this an increasingly difficult task. However, Meadowbrook Care Center has demonstrated an ability to combine a nurturing environment with the delivery of quality care.
I would like to specifically thank Joyce in admissions and Jeanne and Scott in Physical Therapy who have been wonderful and helpful. Natalia and the care delivery Team have helped to make this challenging situation more manageable. The key is the culture created by the ownership Team. It starts with the first experience at the front desk and continues with others throughout the building.

Meadowbrook Care Center provided me with the care that I needed from the day that I arrived until the day that I left. The nurses know what they are doing and the the whole staff is amazing. Kianna the concierge is always on top of your questions and concerns and she goes above and beyond to get anything you need. Odanys is very sweet and knows what he is doing. He made my communication with staff very smooth. Social workers are very good and food is also good. I would recommend to family and friends!

Finding a facility for a family member can be a stressful decision, but Meadowbrook was the best choice we made for my Great Grandma. The staff is very attentive and always treated her kindly. The Transitional Care Nurse, Joanne, went above and beyond to constantly keep us updated, and answered all of our questions. Even if they weren t nursing related, she would always inquire and get back to us. My great grandma loved the therapy and therapists that worked with her and made her feel loved every day. The dietary staff was also very accommodating as my great grandma only eats certain foods. Lastly, the housekeeping staff was amazing. During times like this, it is very important to keep everyone s space sanitary, and they always made sure everything was clean and organized. I would 100% recommend this facility to anyone!!
