Love the split rooms (semi- private ) because your...

Love the split rooms (semi- private ) because your beds are not next to each other , your guests have the freedom to interact with you and not feel like there imposing on the other room mate, privacy curtains for extra, patient bathroom and the nurse and aids help and treat my mother special and that makes her kid's very happy.( It may be a 5 star by the time she leaves).

My 91 year old mother was admitted on a saturday for severe sciatic pain in her right hip. She was there for over 8 hours without pain & other meds that she has been on for 16 years. The facility lost her clothes and tried to give her someone else's. She rang the bell for help to go to the bathroom and some days waited hours as no one came. She had the same socks on for 5 days. No clean wash clothes or towels were given for days. She would ask for help with something and people wou and say they would be back annever came. Communication was very poor from nursing staff, doctors, and social workers. There was so much miscommunication and lack of medication schedules followed we took her out and left to another place. As a medical professional myself of many years I would not recommend this facility.

I was very impressed with this facility. My father had a two week stay after suffering a stroke and they had him in far better condition after his medical issue. Both my wife and I were extremely pleased with the results and all of the caring staff.

Evergreen was terrible for my elderly mom, who stayed there for three weeks after a partial hip replacement. She left Evergreen in worse shape than when she entered, that's for sure -- at least mentally and emotionally, and maybe physically too. It was also bad for me and my sister as we tried to help our mom from the outside during the pandemic. As another reviewer said, PT/OT seemed good and competent according to my mom (and I agree, based on the therapist's phone calls to me). But nursing care seemed abysmal. My mom said the aides and nurses mostly ignored her (often silently holding up an index finger to indicate "wait!"), and she said she often had to wait hours for water and other care she requested. Besides her suffering, my major complaint is terrible communication. It was hard to reach and get calls back from my mother's nurse manager and social worker. I kept getting assurances that I'd get calls back that never came. Very unprofessional and unacceptable. And they mismanaged her discharge. I was told two or three times about a trusted wheelchair-transportation company that they'd book to get my mom home. But the social worker didn't bother booking the ride until several days after she gave me my mom's discharge date, and by then the company's schedule was full. Why repeatedly talk up the place and then wait so long to book it? Ridiculous. I wouldn't send a family member here, and in fact we bent over backwards after my mother's subsequent health setback (right after her discharge from Evergreen) to NOT send her to rehab. This place was probably the worst experience of my mother's long life. They couldn't even keep her safely in her bed and admitted she "fell out!" Have they not heard of bed rails?! To add insult to injury, they lost her expensive hearing aids and finally "found" them weeks after her discharge -- probably because we threatened legal action for compensation. As you can tell, I could go on and on. This place is AWFUL. I don't have experience with other rehab places, so maybe Evergreen is typical. I sure hope not. Don't send a beloved family member to Evergreen if you can help it. Just don't.

In less than 2 days after dropping off months of medical documentation and an over 2 days worth of work to establish the decline of my mothers state, THEY THREW AWAY ALL THE HIPPA PROTECTED DOCUMENTATION, RESEARCH, AND DIAGNOSES

Did not feed her. 3 straight visits I saw no meal or a 4-6 hour old uneaten meal when it was past time for the following meal which obviously NEVER ARRIVED

Nothing but excuses. And the audacity to send a bill for 5k when my mom was covered by insurance

My husband is the patient at this facility now. He came from Providence Hospital after he was diagnosed with heart failure. I am Physician and I am so pleasantly surprised on how well this facility runs. All Doctors, Nurses, Nursing Assistants and Therapy staff are so knowledgeable and caring. We are treated with such love and respect. I do not remember everyone's names but I will write another letter to Administrator to make sure they all know how much we appreciate everyone. My husband is feeling better and we plan to take him home this Saturday. We would definitely recommend this facility to all our friends and family. And their food is great too!

I have sat here and read all the reviews and it s very strange that my experience was very different ( In a good way) my mother was transferred there after a fall for their rehab services and much to my delight I found GREAT occupational therapist and physical therapist who are very patient and willing to work with my mother. The nursing staff was awesome as well as the CNA s they did everything they were asked to do and my mother was for the most part was very satisfied with the level of care she received from Evergreen and I would highly recommend Evergreen to anyone who needs nursing home assistance or rehab assistance. The director of nursing seem to have a bit of an attitude when we had a meeting in the conference room but once I got her together we were fine. All in all it is a nice place, I can think of other places that in no way can measure up to the care my mother received from evergreen. It s my hope that this review Negates all previous ones, not to minimize their experience but simply to say that I found it to be a worthy facility.

I will never recommend Evergreen Health Center to anyone. My mom has been in several rehab centers, this is the worst. Lack of communication from nurses and staff in therapy room. The employees hide from you. The nurse wouldnt give me daily reports.
I will give a shut out to Dorothy and Filo for a job well done!

Things have changed here. Staff ignore beeps and stand around and talk to each other about topics unrelated to patient care. My relatives bathroom doesn't get cleaned either so it smells like urine.

My mother entered Evergreen Health and Living Cent...

My mother entered Evergreen Health and Living Center on April 30, 2019 for rehab and was there for over three weeks. My observations are mixed. On the good side, it's a beautiful facility, and most important, her physical therapy and occupational therapy were very good. I live far away and was able to come in to town to see two OT sessions. The therapists gave her exercises that pushed her and the facility mimicked a home situation: bathroom, kitchen, stairs. They also arranged a special session with me there, where I showed them Mom s bathroom setup. If my calls to staff weren t answered right away, I did receive a call back (especially from Heather, the head of the therapy department), with detailed answers to my questions and a detailed description of Mom s progress. I also received helpful information on Mom s medical condition from the medical records department. Mom said all the staff were very kind, and the ones I met (e.g., nurses, attendants who brought meals) were extremely pleasant and attentive. One problem: even though on Mom s whiteboard in her room it said no coffee, only hot water with meals , the staff kept bringing her coffee. A few problems did arise that related mostly to administration. I called Evergreen on May 1 to see about Mom s rehab. The person at the nurse s station was new, and said that Mom wasn t on a list for rehab, and that they didn t know anything about her. That disturbed me, and I left a message for the therapy department; they called me back later and told me that with the new staff mistakes were being made, Mom was being evaluated, and that an update would be called to my sister (primary contact) in several days. On my mother s pre-determined discharge date, one of her caregivers came to pick her up, and no one at Evergreen was prepared for her discharge according to both the caregiver and my mother. No one had her paperwork ready, no one helped her pack, and both women had to wait at least a half hour for an Evergreen employee to pull the paperwork together. This is unacceptable. They should have had the paperwork ready, her belongings packed, and should have had Mom waiting at the door in a wheelchair, ready for her caregiver to pick her up. Her caregiver had to do all this for her. So I d give three grades : a 5 star for her rehab, a 3 star for the on-floor staff, and a 1 star for the administration.

My father is there now supposedly for rehab. He has dementia/Alzheimer's. Staff says he is not compliant ... Gets out of bed without assistance, well when you have to go to the bathroom and no one comes for twenty minutes what are you suppose to do? He waited and had accidents, now they have s diaper on him. He had regressed so much since his admission last week. Evergreen had been a huge disappointment.

My dad has been here for 4 days. Some of the staff is great others not so much. Nashwa in physical therapy is the sweetest! Very polite very caring and very good with my dad. Sara the nurse who sat with him great person kept my dad company and made him very comfortable. The place is clean and well kept. Terry the manager is excellent!!! Hands down the best manger they have here! She makes you feel very comfortable , very knowledgeable and had guided us every step of the way. Super friendly and goes above and beyond her job!!!!

I am on the fence right now about Evergreen..my mo...

I am on the fence right now about Evergreen..my mom is presently there for an after surgery stay ,this is her 4th time here in a 8 year period.This time Im not sure whats going on but her room smells like urine and feces which we thought was a chair but its the carpet and just yesterday there was a big bug in her bathroom which scared her and tbey said house keeping would handle they never did. At this point Im going to request a room change and hopefully that will work. She has a couple more weeks to be there and hopefully it goes better than this last one. Its definitely cleaner than most but when you have to make requests for simple things like getting your roon vacuumed its a little rough.

Beware of Evergreen Living Center! Do not admit your loved one if they are a total assist patient. My Grandmother had a bedsore on her backside. It was healing well when she arrived to Evergreen from the hospital. The neglect she experienced was unprecedented! The wound festered for 2 months as we begged the staff to turn her, shower her in a gerrychair and brush her teeth. When she needed to be hoisted for therapy, the staff complained. This is the worst of all nursing homes and shouldn't be allowed to be licensed! Horrible staff, horrible place!

St. John hospital knowingly discharged my mother too early to this facility. She had laid in bed here for three days with pneumonia. I knew something was wrong and I was ignored on it when I brought it to the nurses attention and all I got was she's weak and tired and she just needs physical therapy. My mom could not sit up or move or hardly open her eyes and burning up with a fever. She could not feed herself. I begged the nurse to make sure she trys to eat and when I go back food and drink sits on the tray. Warm. Anyways she developed chest pain and was transported to Beaumont hospital where she stayed the remaining time till her passing. (Thankful to Beaumont drs and nurses who went above and beyond trying to save my mom) where they found numerous things wrong with her that St John hospital and Evergreen overlooked and ignored. I recommend going elsewhere and it's sad our parents can be treated this way. I can't be there every single second. That's why the family is supposed to trust the care their parent receives but nowadays you can't trust many but there are wonderful nurses and drs that go the extra mile to care for the patient. I salute you

My Mom has been at Evergreen for about two weeks and I have been very impressed with the staff, services and care. The aides are personable, friendly and helpful, the nurses are on top of things and there has been excellent follow up from all staff with any special requests I've made both in terms of additional information needed or my Mom's care. I think the team at Evergreen does an outstanding job and I am comfortable and confident that my Mom is being well taken care of.

Great place for rehab. My grandmother was there. We liked it and she got stronger and was able to come home to live alone. She is 89 now, cooks, and managing on her own with our assistance.
My family and I would recommend this facility for a great rehab!

Staff is horrible. Staff members are so mean and rude. Therapists seem pretty nice amd compassionate, the aids on the other hand have absolutely no compassion. Patients lay in pissed beds for days and when asked to change them you get nothing but attitude from them and they dont change the bedding. So disgusted how people can be so mean. If you can not have compassion cargiving for patients then you need to find a new job. My heart really goes out to the patients for getting treated so poorly. Breaks my heart.

terrible place; lazy staff who are very dismissive of patients. my aunt is there now and is being ignored. Beaumont should be ashamed of this facility! Dr Hyde came to visit and we were impressed with her but haven't seen her in over 10 days. my aunt can't wait to get out of there!

If you care for your loved one's this is not the place. My mom called us constantly to contact nursing station to get her pain meds. The nursing manager Terry only returned my call once. My mom got an infection and now she won't return my calls. My mom NEVER got a shower during her time there. To not return a family members call is UNPROFESSIONAL. If you lack compassion for patients and families maybe find a new line of work. Only interested in filling beds. I will be contacting the State and other's. Patients deserve better. I pray them or their loved one's don't experience the treatment they give to patients but Karma.....

My father was there receiving end of life care. He was on hospice at Evergreen. I am a little late to write this but I want to make sure that everyone knows how great this facility is. My father and my family always saw compassion and caring from all staff. It was much easier for us to know that he is in good hands and feels comfortable. Thank you very much Evergreen!!! Our family will always appreciate what you did for our father and us.

There is a distinct split in care at Evergreen. I am here presently and have 4 more weeks to go. I give an excellent rating to PT and OT . Someone comes in in the morning says what time PT and OT will be here and they come as scheduled. The nursing staff is cold, uncaring and short with the patient. They are forever saying how understaffed they are. I ve been here a week and have yet to be fully bathed and hair washed. I need skilled nursing care because I m getting IV antibiotics. Weekend medications get totally thrown out of schedule.Not sure what the next 4 weeks will look like.

This place is horrible! My mother passed away from Covid-19. They were bringing in patients but would not allow us to visit. NO COMMUNICATION at all. If the Lab. that ran her Covid test hand't called me I would never even have known she was tested. Horrible!!!

This is advertised as a 5 Star rehab and at best is a 1Star. There are no skilled nurses and the place is run by assistants. My mother had Alzheimer Disease so she was at there mercy for everything. We had to pay someone all day to be with ber so she was able to eat, drink and urinate. No one was ever there for her. She was dehydrated frlm not being offered anything go drink.
When you call to check on the condition of your loved one they don't know they have to ask the assistant because they haven't even been in the room except to pass meds.
There was no rehab going on for my mother because she needed too much time. The staff is very rude including the case manager. Just felt like I needed to let others know about this place so you do not get fooled like we did when it says 5 Star you expect good care. You will not get it here!

My family and I can not thank enough all staff at this facility for all the wonderful help and caring they displayed. My wife who is a retired Physician said that she did not expect to see such a professional and knowledgeable staff. I am still here in the facility and want everyone to know that this is a great place to receive needed therapy and nursing care. Doctors will see you as many times you need.. Facility is very clean and well maintained,. Food is better then in the hospital!

I was receiving rehab at this facility several times. Every time I was able to get stronger and came back to my daily routine. Now I live alone, drive, and enjoy my life. I want thank everyone at Evergreen who helped me to feel better. They have greatly run facility and their therapy and nursing staff are EXCELLENT!! I nightly recommend this place. Dr. Tamler and Dr. Hyde took care of me and they were great!

My mother-in-law was a patient at this facility and I am giving it 5-stars because everything about her stay was top notch and first class. Family members really need to do their homework when choosing a nursing facility. My 5-star review is just a personal opinion, but Evergreen is actually RATED 5-Stars by Medicare. Before selecting Evergreen we toured a lot of places in the surrounding vicinity and none of them were as nice as Evergreen. The best advice I can offer is to make an educated choice for your loved one and check the star ratings on Medicare's website.

I had a back surgery and had to choose the right rehab place. I came to Evergreen for therapy. This was the best choice. I did not want to leave.Therapy, Doctors (especially Dr. Tamler), nurses and nursing assistants are so caring, friendly and knowledgable. I felt like home there. I would recommend this rehab place to everyone! Food is great too!

My experience was as a nurse applying for a position at this Southfield facility, I can't believe Beaumont keeps their affiliation with this facility. I have never encountered such lack of professionalism within management. After waiting over fourty five minutes for my scheduled interview the different scenarios I witnessed and unsatisfactory lack of full filling patients needs was very disconcerting. Before I could interview from the waiting area/lobby I'd already decided I could never work at this unorganized, poorly managed, uncaring staff with bad attitudes...then when my interviewer came and was so alarminly rude , disgruntled and disrespectful towards me I'd had it...I kept my professionalism surrounded by chaos. Beaumont needs to revamp this facility or pull their association it's giving them a poor look to outsiders.

This place is disgusting! Employees (nursing & medical assistants) are horribly rude and lazy. My Aunt laid in bed for three weeks to the point she was hardly able to sit in a chair for 40 minutes. When asked for her medication list we were rudly told " we funna switch shifts in a minute can you wait". Her roommate told the nurse she had diarrhea, never got Imodium and sat for two hours in a soiled diaper. I wouldn't bring my worse enemy here

Wonderful place for rehab after the hospital. I'm glad to be going home - cause home is home. But they did a wonderful job on my knee. Thank you to all of the nurses and the food was almost as good as mama makes!

We have been saddened and angered by the disrespect and abuse our 90 year old mother received while at Evergreen Health and Living Center, although the PT was good. Do Not Use This Facility!!! The Nursing staff in her unit made her feel unwanted and that she was putting them out if she needed help!

The nurses barely see patients at all, and do not involve themselves in patient care. The LPNS & CENAs do the actual patient care. Wait times for call light response is routinely greater than 20 minutes and longer. I have had to call the house manager 5 times thusfar in my 3 week stay because pain meds were delayed greater than one hour.

My auntie is currently staying at Evergreen and my wife and I have been thoroughly impressed so far. Every morning her nurse and therapist make sure she's comfortable and well attended to. She's looking forward to going home in a few days, but I can't say enough about how well she is being cared for here.

The lpn's and Nurses treat the CNA's so bad ...no incentives, always short of staff, too much work and not enough money... 9-15 patients on day shift depending on the staff, 2-3showers per day, pass water, pass and collect dinner trays clean pantry, soiled utility room or shower room, empty trash , check and change every two hours, do vital signs and weights, get people up before dinner and lye them down after dinner and get them ready for bed. I've been physically assaulted by residents quite a few times and nothing has been done...they are all about the money.They prefer to pay their crappy Nurses and horrible and lpn's to do CNA work. They DON Mila says CNA's come a dime a dozen and they talk about their CNA's very bad. They had 13 citations from the state in 2016 and they prefer to pay their lpn's almost $30.00 a hour to do CNA work and only pay us 12.15 to do our job... And they were firing people who voted for the union they sent letters to the CNA's houses telling us to vote NO for the union....They are unfair to their CNA's and we do the most and hardest work...
