I wanted to take a moment to thank the folks at Spring Creek in FoCo for taking such excellent care of my Grandmother. Unfortunately she passed away last December but she was very well cared for in her final year. A special thanks to Mr. Todd who gave her the special attention that our family needed as we were not able to be there as often as we would have liked. I hope to not have to deal with end of life issues again anytime soon but good to know we found a place that treated our Grams as they would their own.

This was an overall horrible experience. I was sent from hospital for alleged rehabilitation. Staff is overworked and untrained and unavailable. Management fails to address issues. I was fed processed food despite dietary restrictions. Vital signs were not taken daily and medications were inaccurate and not given correctly which significantly impeded my progress. Wound care was given by improperly trained staff and I changed my own dressings much of the time. Pt and Ot staff were rude and refused to accommodate a schedule. After two weeks I demanded to be transferred or released to home care. They discharged me to home and refused to provide transportation. Staff encourages smoking by residents. After several weeks at home, my condition with proper home health care has dramatically improved. I would not recommend placement of any patient at this facility for any reason. Their poor reputation is well known in the local medical community, including their own staff to whom I complained daily. I was desperate to get out of hospital after over four weeks and unfortunately should have done better homework regarding their bad reputation. Terrible and traumatic experience during a very difficult time for me. Fortunately, I can and will demand adequate care. Facility should be investigated by State of Colorado.

I am so appreciative to this facility. My mom was in a tough spot and her condition made her no longer a fit at Lemay (Fort Collins) and sister facilities. From our first interaction with Todd Truax (CEO) and Ty Graef (Social Service Director), we felt comfortable and open in our discussions. They felt that my mom would be a fit, although needing some additional setup. As my 82 year mom describes it, we went from a factory (Lemay) to the feel of Mom & Pop shop blended with professionalism. This has been a big improvement for my mom. Are they perfect? No. Are they providing very good care? Absolutely. Cathy Wright, my mom's social worker and assigned contact, has provided exceptional follow up. The caring and empathy blended with a perseverance has meant a great deal to us. It is so great to see the CEO walking the halls and working with residents. It starts at the top. For many families, this will be an excellent fit.

The staff at the Spring Creek were absolute angels...

The staff at the Spring Creek were absolute angels. It takes a special person to display the caring, love and patience that this team showed while we were there. The care my mother in law receive was amazing! I can't thank the nurses enough for everything that they did for our family!!!

As a person who is in an out of many SNF, my observations and interactions at this facility have been wonderful. I have seen many residents come in and discharge back to home as well has long term residents who are happy there. There are lots of activities and it always smells and looks clean. Love Waffle Wednesdays!!

Terrible place. Take forever to respond to call bu...

Terrible place. Take forever to respond to call button. Do not give scheduled showers. My dad was in the rehab section and they did not do any rehab in 6 days. My dad ended up with major blood clots and ended up in hospital ICU. I was there every day trying in get answers and was ignored most of the time.

This is the 3rd time in the past 2 years that one ...

This is the 3rd time in the past 2 years that one of my parents has been admitted to Spring Creek Healthcare Center, and is receiving Sava Care rehab. We chose this facility as the rehab for our Mom this episode following a fall at her home. As a physical therapist and a daughter, I have been pleased with the quality of care and the outcomes at the time of discharge of each episode. Each time they have rehabilitated our parents so they have been able to safely return home. We are included in the planning process for our parents and there has never been any surprise at the time of discharge.
Spring Creek is an older building and it has been tastefully updated. All rehab rooms are private. Patients can choose from a menu for each meal vs. be given only one or two choices. They can make a salad of any wrap items on the menu and for a SNF I think the food has been reasonably good. I would give this facility a 5 star if it was a newer building. The level of care is what is the most important for our loved one.

My husband was in Springcreek for over 7 years after a brain injury. Many staff members care for and about the residents but administration does not. Lots of issues at this facility, high turnover, old building, with minimal upkeep except cosmetic. So many problems including an antiquated phone system which contributes to lack of communication with family. There is no receptionist...ever, so the phone rings and rings. Pretty scary when you want to check on your loved one. Administration is never available and does not return calls. Bottom-line: Stay away. Don't subject yourself or your family member to this place.

Unbelievable treatment of employees and patients Regional office in basement corrupt, the whole organization has been under investigation for years, how does this still go on??? Clinical people know Sava is the worst, don't work here, don't send your family here, revolving door of clinical help, if you are not corrupt and won't concede to bullying, narcotic abuse, etc., you will be attacked, I was told so and got out immediately, don't risk your licenses or your family here, the WORST

If it weren't for the awesome PT here I wouldn't recommend it. My mom needs psychical therapy after a stroke and their PT is great but the majority of the staff is very neglectful. I call with concerns and it takes 2 weeks to get a response! They also told me she could only have 2 showers a week! That is bull crap!!!! I also do not like the fact that they serve processed food to these people. There are no healthy, low salt, sugar free or dietary friendly options for these people!

I live down stairs from Spring Creek Health-care ( nursing home) on the far East side. In the parking lot. I LOVE Garden Square . The living here. is Amazing! Along. with all of the people who live here. & a special care from the people who work here!! They are Outstanding !!

As a caregiver from an outside agency this place is horrible. Lazy, uneducated staff. Completely neglect the residents and the cross contamination I watched as they go room to room with the same gloves, even handling medication cups in one hand and a dirty brief in another. Being here on an overnight, they skip the 2 hour changes and repositioning. So sad and disgusting. Many of the staff are extremely rude and often hard to find.

fair warning. the administrator that is about to be running this nursing home just destroyed the nursing home I worked at for almost two years. she literally sent every loyal employee off while she was employed and after that she decided to abandoned ship and this is her next destination. Pay careful attention when this lady is in charge. From family members, co workers, and self experence - this is not one to leave in charge of your family.

The facility provides a caring community for residents to live in. The environment and staff promote the cultivation of meaningful relationships for all who live and work here. Like any community, the members have the opportunity to learn and grow together. This is evident by the improvements made over the last few years.

My husband Mark had bilateral knee replacements in...

My husband Mark had bilateral knee replacements in January 2014. It was because he had both knees done at the same time, that I could not give him the care he needed at home. It was then that we decided to admit him to Spring Creek nursing rehab for three weeks so he could get the care that he needed. The staff there was stupendous! All the nursing staff were wonderful, caring people that took great care and motivated my husband to keep moving to heal faster and to get better. Whenever he needed them, they were there fairly quickly in a 95% occupied nursing home! The physical therapy staff were as wonderful as the nursing staff. They took very good care of Mark, showing him the ropes and not letting him get hurt, all the while motivating him as well to heal faster, to get up walking and to get better so he could come home sooner. Even through the pain of a bilateral knee replacement surgery, it was a pleasant experience for Mark due to the caring staff at this facility. The price was right too! We appreciated it immensely.

Highly recommend this facility! I am a social worker with InnovAge PACE and I feel very confident referring my clients to Spring Creek. The staff are knowledgeable, efficient, compassionate and welcoming. It s been a pleasure working with their clinical and administrative team.

DO NOT WORK HERE! I went to go apply for a CNA pos...

DO NOT WORK HERE! I went to go apply for a CNA position here and no one was at the front desk. A family member of a resident went and got a CNA then she went and retrieved her boss. After standing there for 10 minutes when the manager finally come to talk to me, she rudely asked me if I had the correct application. When I said I didn't because their ad said to just come in with a resume, she rudely handed me the huge application package and said, "fill it out at home and then come back some other time". Then she just walked away. No thank you, no handshake, didn't ask my name or what I was applying for. NOTHING. If they are this unprofessional when hiring, I can't imagine what they are like to work for. BEWARE!

The staff was wonderful from the PT Director, Soci...

The staff was wonderful from the PT Director, Social Worker, Customer Service, PT and OT, Nurses, CNA 's, cooks and cleaning staff. Very warm, friendly, helpful, caring and cheerful.
My father (93 yrs) was in Rehab and improved his strength and endurance to come home. He enjoyed the food, and his time in OT and PT therapy. (His PT and OT were excellent.)
We had a great experience there.

PS. I want to share too, I continued to be my fathers best advocate even though he was in Rehab for 3 weeks. I visited him most days morning before my work and after work even for just 15- 30 min. to keep up to date with his progress.
I spoke with nurses, to establish I would do his laundry, what medications he was taking daily, CNA's for how my father was doing with his self care (going to the restroom by himself) showering days.
I spoke with PT and OT to see how progress with there with self care, dressing himself, and walking. I spoke with activity director on ideas to stimulate his mind and games we could play. She showed me a closet with activities we could use and put a card table in his room so he could write notes to friends. I spoke to Social worker when my father received a bill that medicare was to cover and found when I called the AFMC they just did not have his insurance information attached to his record. So it was taken care of.
There are a lot of people working in a health care facility to assist our Loved ones, but family ( in my opinion) are the ultimate care givers to help coordinate and communicate with every one to help a family member receive the best care. I do think the reason I enjoyed the experience with my father in Rehab is I felt I became part of the team
to help him recover and I had such supportive and wonderful help from
the staff there.
My encouragement is to stay involved with your loved ones care, be willing to learn. If something is not going well take responsibility to learn who there can help solve the problem. Remember progress not perfection.
All of us are human and imperfect, but if we work together we can have a much better outcome.
