Thank Goodness my Mom was only in there for a short amount of time. This place is a nightmare. They do not care for their patients. My Mom went into diabetic shock in their care. I probably do not need to say anymore than this. The rooms are filthy, They lost her clothes in the laundry, and they were tagged with the proper name. She was there for over 2 weeks, and they did not give her a shower. You need to micro manage the staff to ensure your loved one is partially taken care of. The provider's prescribed the incorrect meds. The MA, nurses, etc. are awful. The discharge process did not follow up to ensure the durable medical equipment was delivered to ensure a safe departure. Do not place your loved ones in this place. It is a disgrace.

This place horrible to work, they don t pay you even cover 40+. You don t get lunch breaks and they expect you to stay an extra hour, but they only pay you for7 hours. The superiors horribl. This place is good at sucking money from pt and staff alike. They like when admission comes and put them even with infected pt. They even send ppl to hospital for no reason to get money from rehab. I hate the place even more when they don t pay me for 2 extra hours each day and still treat me a slave. A lot of discrimination among staff, and low quality care because staff stressed no money feeling used n no payment.
Sprain Brook Manor provided my loved one with exceptional care. There is a wonderful, family enviornment and the staff truly went above and beyond to make sure our needs were met. Thank you Sprain Brook for providing a great experience and for being such a caring and comforting facility.
Sprain Brook Manor rehabilitation offers one of the most pleasant Rehab experiences after surgery. The therapy team is amazing and professional. The facility is nice and spacious with great food and always new menu options. The transfer from the hospital is seamless and they provide door to door transportation for family members. Overall an A+ experience
My good friend has lives on the 4th floor of SBM for over 2 years. Because I live so far away I am not able to visit. Luckily he does have visitors who check on him regularly, and they report that he has adjusted, through the support of the staff of this house, to embrace this stage of his life at 99 years come this February. Keep up the good work.
My mom was sent to sprain brook manor rehab to do physical therapy in December, and next thing you know we find out she has Covid. The facility did not even call to inform us that she was positive for Covid and was experiencing symptoms. In fact, we found out incidentally by an employee s slip of the tongue when we called to speak with my mom. At that point they said she was completely asymptomatic. We had to fight for hours to get any information about her covid diagnosis, and to be allowed to speak with her. We were hung up on multiple times over the course of hours when asking to speak to her. When we finally spoke with my mom she said she was feeling extremely sick, not asymptomatic as the staff claimed she was. We arranged on our own for an ambulance to take her to a hospital. When she arrived at the hospital later that day she was diagnosed with covid pneumonia and was severely ill. We still do not know how long she was experiencing covid symptoms at this facility and when they diagnosed her. The facility s actions of not contacting us about her Covid diagnosis, saying she was asymptomatic, and not allowing us to speak with her was shocking and unbelievable. Update: I ve come to find that multiple families have experienced the same thing. A Covid outbreak happened in this facility and families were either not informed or were told that their relatives were asymptomatic when in fact they were severely ill. Absolutely appalling.
DO NOT USE THIS PLACE!!! Sprain Brook Manor is horrible!!! I would give this place no stars if I could. Besides being a depressing looking smelly dump, the nurses are cold, heartless, uncaring, have bad attitudes, and are rarely around. They are negligent and inattentive and will just leave your family member in a room alone all day. Due to their negligence and lack of care my parent was hospitalized multiples times for dehydration and pneumonia because they ignored all of the obvious symptoms. Empty oxygen tanks and clogged feeding tubes were completely ignored despite the beeping and despite calling a nurse multiple times. And you can forget about finding a doctor on site. I made a big mistake putting my parent in Sprain Brook Manor. RUN, DON T WALK, AWAY FROM THIS PLACE!!!
This facility use to be the best but is now the worst, noone knows what they are doing and noone takes responsibity for things that are 100% their responsibility. My parent was a patient last year, she was a fall risk and fell fracturing her wrist, and when she contracted bacterial pneumonia they gave her steriods instead of antibiotics. She passed away due to their negligence and now they are sending bills totaling $13,000 for balance due. Not kidding.
I had such a wonderful experience at Sprain. Starting from the moment i walked in with my grandmother Thelma, the receptionist Gloria was so pleasant and kind. We were brought up to our room and the nurse supervisor answered all our questions. Therapy started right away and we were on the road to recovery with the help of Edward PT and Kelly OT. The therapy worked miracles with Grandma and i am so grateful. The food was delicious and dietitian Melissa was so kind to bring up all grandmas favorite foods. The nurse Mara greeted me daily with a big smile and kept the family in the loop at all times.
Lastly, to Akiva, the Administrator who literally does it all. Whether mopping floors, giving out food trays, singing karaoke with the residents and helping paint doors Akiva was always there for us and always went out of his way at all times days and late nights to make us feel comfortable and happy. Sprain Brook is a great place to be. Everyone is very friendly. The recreation is outstanding. The care is very good. Housekeeping is excellent. The activities department keeps us busy. Thank God for that. Everyone is so welcoming, this is a great place to be
As the administrator of the facility I am very proud of the incredible work we achieve as a team here at SBM. We are are rated 5 stars by CMS for our quality measures and quality care outcomes and that is a direct result of the tireless efforts and interdisciplinary approach of our staff.

By far the worst nursing home in Westchester County!!! We agreed to place our grandfather there so he would be closer to family and within a week my family and I realized what a colossal mistake we had made!!!! Now, keep in mind, he was there for rehabilitation and family visited DAILY!!! Multiple times a day in shifts. Day one upon admittance, roaches crawling on the floor of his room (which by the way, he noticed!!!). By day 2, he was wearing clothes that belonged to a resident across the hall. Not the same room (which I could understand) but ACROSS THE HALL!!! His hearing aides were lost which by the way, comes out of pocket since insurance covered the initial pair. The cost $3500!!! So we had asked for a schedule as to when he would be receiving services so a family member would be present to properly insert his hearing aides (which would ensure that he would be able to hear directives from the therapists) and we would remove them once his therapy sessions were completed for day. This would also ensure no further loss. We made it VERY clear that we were more than willing to do this since Sprain Brook Manor claimed ZERO responsibility!! Let s jump to a week in...now keep in mind he was there for rehab which also meant speech & physical therapy....it was determined that services were to be discontinued!!! Funny because his medical insurance entitled him to THREE MONTHS of therapy!!! It s week ONE!!! So I decided to get the head people to meet w myself and my family. The powers that be decided to discontinue his services for physical therapy & speech bc he wasn t progressing !!! A week in and they decideD to D/C services he was entitled to receive?!?!? AND, here s the best, his Physical therapist WAS NOT aware!!! Now, maybe I m crazy but I m pretty sure I m not -it IS the provider who determines the level of progress, not those sitting in an office all day!!! So I did my homework as well the speech therapist was meeting w my grandfather approximately 45 min after being given a dose of meds -meds which had side effects of making one drowsy and lethargic. Now if you or I were drugged w/ a med that had those side effects, we wouldn t be progressing either!!! So I decided it was time to spring the old timer out of there!!!! I put all necessities into position for his arrival home w family -therapists weekly, hospital bed, Walker, commode, case worker visits, etc. Now, I notified them as to the date and time I would be picking him up. PLEASE KEEP THAT IN MIND!!! Upon my arrival, I found him in the community room/feeding area slumped In a wheelchair!! His pajama pants were at his ankles and he was soiled through TWO diapers!!! YES -2 DIAPERS!!! AND they were expecting my arrival?!?!? The process to get all equipment needed at home meant that he stayed there slightly under 3 weeks. In that time, he lost 22lbs!!! Now, you may read this and think it s just a one case scenario and I agree bc I too have read/heard that one horrific story. Now, I am that family member who witnessed the worst care 1st hand and my family and I were there DAILY!!! So I often wonder, had we not gone there daily, how would he have been treated??? Because the level of care was an ABSOLUTE DISGRACE!!! ABSOLUTE DISGRACE!!!!
My Grandmother passed away at this facility a few weeks ago. All I've seen on my visits and heard from family members is how horrible they treated my grandmother.. I am disgusted knowing how terrible this place is... Save your money and keep family members far away from this trash heap... And to the management/staff I Sincerely hope you are never treated the way you treat others... Also note the 5 Star reviews are actually from current and former employees..M.
This place is the worst. Shutters falling off the building, unkempt landscaping and staff unfriendly. We've visited my cousin there 3 times in the past 6 weeks. Due to covid restrictions, the first time we stood outside the service entrance with air conditioners dripping on our head and yelling to my cousin, second time was a little better but the last time today 7-30 was unbelievable! We met her in the front entrance with no chairs for us, we brout our own, the automatic door kept opening and closing and worse yet an aide walked in from outside without a mask and stood less then 2 feet from my cousin before entering the lobby with patients -- all without a mask. They did not sanitize anything and when I complained, the main guy lied and blamed everything on COVID. Terrible, I'm reporting them to Westchester Board Of Health.
I volunteered at Sprain Brook manor a few years ago and had the best experience. The care and attention given to the residents is great and leaves nothing to be desired. Everyone deserves to feel a sense of community and that is what Sprain does!

The worst place you could ever put a Family Member. The Facility and there Staff are horrendous. If you care for the person that needs Physical Rehabilitation or a Nursing Home DON'T NOT USE THIS PLACE.
