1.. Our Dad is in Lifecare of Columbia. He was admitted around May 17th and fell out of bed 3 times on May 18th. We filed a complaint with DHEC and was emailed back. We was informed that it would be about 90 days to complete the investigation. I feel this facility need to be reprimanded/watched to stop people from being hurt/harmed while under their care. My original complaint was regarding 3 falls in 1 night. I am very concerned about the patient that fell and stayed on the floor all of that time while I was visiting my Dad.

2. Last week the wound care nurse, Angie called me and I voiced my concern about Herbert White's wound vac constantly beeping and that the way it was placed, He was always laying on the tubing. I explained that when he was in Palmetto Health, they "redirected the tubing so that patient would not be constantly lying on the tube. She blew me off and did not address it. Two days ago Angie informed us that he developed a new pressure ulcer on his achilles tendon of right foot. See photo. She called the doctor and an appointment was made for yesterday. It was confirmed that he developed a new ulcer from the tubing. This did not appear overnight. If the dressing on his heel was being changed 3 times/week, this ulcer should have been caught long before she reported it. At the appointment, Specific instructions were given to "redirect" ...or "bridge" the tubing so he will not be laying on the tubing. See attached photo. This delays healing process and caused another 3-6 months of treatment to a new wound that did not exist 4 weeks ago, when I alerted the wound care nurse of the positioning of the tubing.

PATIENT FELL ACROSS THE HALL AND YELLED FOR 40 MINUTES on June 18, 2019. STAFF DID NOT RESPOND UNTIL I SAW HIM ON THE FLOOR AND YELLED FOR THEM TO HELP HIM. PHOTO ATTACHED.

While I was visiting with my Father, I heard a loud and constant scream from one of the rooms across the hall from my Dad. This went on for about 40 minutes. When I was leaving from visiting my Dad, I glanced across the Hall in room 4 __8.or a room nearby it. I saw legs on the floor. The old man was on the floor yelling for help for over 30 min. I immediately yelled down the hall. "You all have a patient on the floor down here".

The staff slowly walked toward the room...never Thanking me but made negative comments as if I was wrong for letting them know about the patient on the floor. ..or I was wrong for capturing this neglect. This poor man was yelling for help for far too long.

On or Around June 6, 2019 around 7:30 pm, we found our Dad soaked in urine. We immediately asked for linen to change him. Since he is alert and oriented to everything, he stated: "The Therapist changed me around 7 this morning, but no one else came in to do it. They took my urinal away". We gathered linen and began getting him cleaned up.

The CNA came in and said..."If it was 12 hours, since he was changed, it would be brown". We replied...."it is brown!"

I am very concerned about my father's and other patient's safety at this facility. We tried to have him transferred but the case managers seem reluctant to sending referrals and following up.

HORRIBLE.....HORRIBLE.....HORRIBLE service!!! I wouldn't recommend this place to anyone....the staff shows so much neglect towards patient....they suppose to check on patients every 2 hours....that's a lie & God forbid if they have to use the restroom or need their medicine especially for pain....it take them for ever to respond to the patient....I've actually witness it myself....I had to go to management & that still didn't help! They don't know how to talk to patients..if you ask me they need to get rid of everyone there and hire a whole new crew or just closed the place down.....despite the reviews we've heard about this place my family & I decided to give them a try because we believe people & places do change over time but not this place!! PLEASE DO NOT SEND YOUR FAMILY MEMEBERS HERE!!!! I am very dissatisfied with the service here......! A terrible place to put your love ones!!!!!! ,communicatio is horrible.....haven't seen or spoken with a doctor since my dad has been here & every time you ask the nurse a question about something they don't know....I'm like what the hell do y'all know.........

If I could give 0 stars I would! DO NOT SEND YOUR LOVED ONES TO THIS FACILITY! From the Director to the Director of Nursing, Nurses & CNA's horrible communication and they're good at covering their tales!

This is an outstanding facility. My father was a temporary resident for rehabilitation. I am so thankful for the staff. I live out of town and they informed me of all changes and improvements he made. They have video calls which is a plus. They were patient and provided outstanding care. The facility was impeccable. I highly recommend this facility. You won t be disappointed if you are looking for care.

This facility is a joke and should be investigated...

This facility is a joke and should be investigated for neglect of patients! They are dishonest of the neglect and will do anything to cover it up! Not giving patients their important medications like blood pressure pills and insulin is extremely dangerous! And then to lie about it! Not excusable! Don't put your loved one in this home unless you can closely supervise that they are being taken care of the way they should be.

I was in Life Care Center of Columbia in October 2...

I was in Life Care Center of Columbia in October 2018 for rehab for a total knee replacement. Except for the pain (which was to be expected), I cannot tell you how appreciative I am with the care I received. I had WONDERFUL CNAs who went out of their way to assist me in any way; Star, Ms. B, Brittany, Nichelle......couldn't ask for better staff. My rehab staff were absolutely, without question, THE best: Debra, Barbara, and others who worked with me. When I was released from the center, my follow-up out-patient rehab staff praised the staff who got me started at Life Care. I've worked in a long-term care facility in the past and know how difficult it is to please everyone with food choices; but I can say the food in Life Care was delicious!! My nurses were outstanding, too, and worked diligently to assist with pain management. From the initial visit, Tina and Stephanie were informative and accommodating. Everyone I came in contact with was so nice, and treated me with respect and dignity. When it is time for my second knee replacement, I WILL opt to go back to Life Care for my rehab. Thank you ALL for a job well done!!!!!

A lovely facility that doesn t have the institutio...

A lovely facility that doesn t have the institutional feel that so many places seem to have. It s never good to have to be away from home, but things like the ice cream parlor and plenty of spacious visiting areas offer bright spots to visit or just relax. The staff is compassionate, knowledgeable, and genuinely try to help even in situations where there s not much they can do except offer reassurance. I also appreciate that Life Care is a faith based organization and I feel that this shows through the way they treat residents, visitors, and staff. Definitely worth taking a tour, especially if you re looking for easy access to interstates for ease of travel!
