1.. Our Dad is in Lifecare of Columbia. He was admitted around May 17th and fell out of bed 3 times on May 18th. We filed a complaint with DHEC and was emailed back. We was informed that it would be about 90 days to complete the investigation. I feel this facility need to be reprimanded/watched to stop people from being hurt/harmed while under their care. My original complaint was regarding 3 falls in 1 night. I am very concerned about the patient that fell and stayed on the floor all of that time while I was visiting my Dad.

2. Last week the wound care nurse, Angie called me and I voiced my concern about Herbert White's wound vac constantly beeping and that the way it was placed, He was always laying on the tubing. I explained that when he was in Palmetto Health, they "redirected the tubing so that patient would not be constantly lying on the tube. She blew me off and did not address it. Two days ago Angie informed us that he developed a new pressure ulcer on his achilles tendon of right foot. See photo. She called the doctor and an appointment was made for yesterday. It was confirmed that he developed a new ulcer from the tubing. This did not appear overnight. If the dressing on his heel was being changed 3 times/week, this ulcer should have been caught long before she reported it. At the appointment, Specific instructions were given to "redirect" ...or "bridge" the tubing so he will not be laying on the tubing. See attached photo. This delays healing process and caused another 3-6 months of treatment to a new wound that did not exist 4 weeks ago, when I alerted the wound care nurse of the positioning of the tubing.

PATIENT FELL ACROSS THE HALL AND YELLED FOR 40 MINUTES on June 18, 2019. STAFF DID NOT RESPOND UNTIL I SAW HIM ON THE FLOOR AND YELLED FOR THEM TO HELP HIM. PHOTO ATTACHED.

While I was visiting with my Father, I heard a loud and constant scream from one of the rooms across the hall from my Dad. This went on for about 40 minutes. When I was leaving from visiting my Dad, I glanced across the Hall in room 4 __8.or a room nearby it. I saw legs on the floor. The old man was on the floor yelling for help for over 30 min. I immediately yelled down the hall. "You all have a patient on the floor down here".

The staff slowly walked toward the room...never Thanking me but made negative comments as if I was wrong for letting them know about the patient on the floor. ..or I was wrong for capturing this neglect. This poor man was yelling for help for far too long.

On or Around June 6, 2019 around 7:30 pm, we found our Dad soaked in urine. We immediately asked for linen to change him. Since he is alert and oriented to everything, he stated: "The Therapist changed me around 7 this morning, but no one else came in to do it. They took my urinal away". We gathered linen and began getting him cleaned up.

The CNA came in and said..."If it was 12 hours, since he was changed, it would be brown". We replied...."it is brown!"

I am very concerned about my father's and other patient's safety at this facility. We tried to have him transferred but the case managers seem reluctant to sending referrals and following up.
