Ever since Las Colinas got taken over by Genesis t...

Ever since Las Colinas got taken over by Genesis the care has gone down hill. Director of Nurses (DON) and the Asst Director of Nurses (ADON) never do what they say they are going to do. Will tell you they are going to get back with you and never do. I just had an incident a couple of weeks ago that I addressed with the ADON concerning my mothers oral care. My mother is total care so she is not able to do anything for herself. So it is their responsibility to take care of her oral care. It clearly had not been done, she had so much build up. The ADON told me she would take care of it. I go back about 4-5 hours later and nothing had been done. That should have been done asap. I can go on and on about the inadequate care they give there. I am having my mother moved to another facility because it's very clear they just don't care and am so tired of hearing "Yeah your right" or "I'm so sorry" or "It wasn't me". They just make excuses. I would stay away from this place if you care about your loved one.

I had to rate to post but DO NOT send your loved one here ! I was told by hospital don t look at the reviews show up unannounced and ask questions. Yes it looks nice but once my mom was transferred there I couldn t get anyone to return my calls. After 5pm nurse staff was terrible we waited over 30min for someone to answer call button and only to wait longer for assistance. The only thing they were quick about was to take your food and horrible food at that. My mom was there for physical therapy to help her walk after being hospitalized for a month I helped her ( changing ) they took long for assistance. Her meds were given to her later and later she was diabetic so we started a notebook to write everything down I wrote her vitals that morning I don t want to go into detail but my mom was rushed to hospital that afternoon and passed the next day. So yes listen to the reviews I wish I did

Therapy was great. The nursing staff took great care of my wife. They helped her with all of her needs after suffering a stroke a month ago. She will disharge home today and we are so appreciative of everything they did for her.

I had the pleasure of being a guest at Las Colinas...

I had the pleasure of being a guest at Las Colinas for 5 nights during the recent winter storm. I had a hot shower, 3 meals daily with Hot coffee every morning, clean clothes maintained by the laundry staff and returned all on the same day, a bed to sleep, and access to Direct TV. The staff worked hard with a smile to care for the residents, making sure they were all fed on time, (many of the staff spent several overnights to ensure the residents were cared for). Staff helped each other, picking them up from home and back so they were able to make their shift. Fortunately Las Colinas never lost power, water, or any other conveniences many of the staff and families did without for several days. I haven't met a staff member there that has been rude, uncaring, or accommodating to the residents that call Las Colinas home. I would highly recommend this place. The rooms are spacious, and clean and comparable to many 5 star hotels.

My dad had a stroke and needed some rehab, we came to look at the facility we liked what we saw. Modern, elegant, smelled cleaned. Jacob was our admissions guy, very nice and very professional. He told me all the good things and used hishis soft voice convincing me. BIG MISTAKE! Los Colinas is horrible! The food is always cold, the food is disgusting, the CNA are ignoring the nurses call button - up to 30 minutes sometimes. I visit my dad and I hear the nurses button go off in hallway and it never gets taken care of in a timely matter. I even complained about the noise, but they don't care. While in this facility my dad got a fracture on his toe and it's unexplained. My dad has been at the facility for two weeks and Los Calinas did not send authorization to his insurance because they want him out! No warning I found out by them calling say I need to sign discharge papers for my dad, no reason given they blaming the insurance, it has nothing to do with insurance. I called insurance and they said Los Calinas never sent authorization for another week. Now my dad has no where to go and it will be out if pocket if we don't get him out. Remind you; they told me on Thursday his discharge date in Sunday. Unorganized facility, they need to be shut down! Do your research and read the reviews, they are are truthful. Worst skilled nursing facility! Don't send your loved one here. Also, they never answer the phones, good luck trying to reach someone inside the facility. Nurses don't answer the phone the case manager takes forever to return your calls. It's a complete night mare, I'm pending to move him to different facility where we can get the proper care.

Please dont trust this place for your loved one.. Get a wi fi camera for your loved ones room its your right. They will have to post a sign on the door warning STAFF of the camera. I had a hidden camera in moms room and staff was constantly turning things over trying to find it. Dont be fooled by reviews by people paid to post them from other countries giving them high reviews.Rehab and nursing assistants are great but the rest is a nightmare. Luckily my loved one is coming home soon.. I feel so sorry for those that have to stay. May God watch over them.

Do NOT send anyone you love to Las Colinas Westover Hills. Go to Warm Springs at Westover. If you are so unfortunate to come to Las Colinas after 5pm on the first day, be aware staffing can't help you with anything. You could go without medication for a day while they get medications in their computer. Stay away, dont be fooled by smooth talking representatives who extol the virtues of Las Colinas, because as of now I haven't seen any.




The day before I had filled out a list of foods I liked and disliked, I hoped the food would be better but the very next morning the breakfast plate was full of my dislikes on the list. Thanks kitchen staff. Did you even bother to read the list you sent to me? That continues to happen, meal after meal.
Even though you may have medicine as PRN which means you can have it when
you need it, the nurse working the night shift makes you wait 20 mins before finally giving it to you, thanks. Understand the meaning of PRN? Some nursing staff.

This place is terrible! Los Colinas has gone down the tubes since it was sold to Genesis Health Care Systems last year. My mom had to wait for four hours one night for a pain pill. She finally got it after a family member threatened to "raise hell." She sat in a wet diaper for over two hours and after my husband finally found a CNA, (there was no response from pushing the call button), the CNA refused to change her, stating that that wasn't part of her job. Mom got pneumonia when she was there, was not seen by by a doctor, was not given any oxygen, medication, no breathing treatments, despite my many requests. We took her out of Los Colinas and she went to Christas Santa Rosa Hospital, where she stayed for several days being treated for the pneumonia. There was one good thing about her stay,, Beatrice, the Physical Therapist was excellent and helped my mom.

I am currently receiving therapy services at this facility. I am set to discharge tomorrow and I wanted to mention a few things about my experience here. That facility is kept in a remarkable state of cleanliness and always in an orderly fashion. The staff that presented the various types of treatment exhibited a very high quality grade of experience and professionalism and also showed a high degree of compassion. I was impressed with the amount of communication in regards to my treatment and progress. I also required wound care and they did a phenomenal job. I would highly recommend this facility to anyone who may need the service.

I really love this facility! This is my second stay at this center for therapy services and I absolutely love the staff. They treat me with respect and make you feel at home. I appreciate everything this facility has done for me. Next month I am having an additional surgery and there is no other place I would consider going.

A friend of mine has a parent there. I saw a photo of the dinner. It was disgusting. A piece of bread. Chopped up hot dog with beans. That was it. My dog eats better than this older adult does at this facility. BEWARE!!! My friend went and brought back food for her parent. So sad and awful!!

My father was a patient for rehabilitation starting off with the director of admissions Jacob Fuentes, was awesome, informative and professional are just a couple words I can find to describe him. The staff starting with the bead nurse April was amazing, Tish and Mark also Susan were patient and very accommodating with care and medications administered each day. Physical therapy staff Jennifer and Caleb took care and strengthen my dad each day. Occupational therapy and speech therapist June always free to greeted us and worked with smile, all of the C.N.A's on staff are phenomenal. My dad is leaving today 11-13-2019 and without the support and encouragement and extraordinary care we couldn't have done it. So thank you Las Colinas of Westover for being my extended family and supporting us during this time.

I could write a book on this place, I'm going to be honest. When it was New and my grandfather was there it was Great!!! Then.... Staff started to change alot and the facility got sold to Genesis which was the hugest mistake ever!!! My grandfather was there for 2/3 yrs and they pushed him out because of insurance and the "New" management... not to mention they lost his things... and WORST OF ALL, they pushed him out by labeling him with horrible behavior issues that he doesn't have!!! It's been hell trying to move him closer to us (his family) and thanks to this Crap Hole Colinas, we barely get to spend time with him anymore cause he's far away.... Please don't put your loved ones there!!! You'll regret it in the end!

I was placed in hold for another 9 mins only to be told if I could call back in 10-15 mins as they were doing reports....REALLY.....I Work at a hospital.....I HAVE HAD NURSES TELL ME THAT THIS PLACE WAS TRASH SINCE BEING TAKEN OVER OVER BY GENESIS. I'm a 73yr old man who had shoulder replacement surgery recently and I was brought to Las Colinas of Westover for physical therapy. I have been here for two weeks and during my stay the staff here have been great! If I could give more stars, I would. The therapy and nursing staff has been very helpful and attentive if my needs. I am scheduled to discharge home and I wanted to thank everyone for every thing they did for me. If I am ever to be asked to recommend a facility for therapy, I would recommend this one.

They are always cooped up in there office they don't do anything fun for their residents is smells in the hallways. The place is just a miserable facility i feel sorry for any family members left there.

I wanted to express my gratitude to Las Colinas of Westover for their concern in meeting all of my our needs while my husband recovered from knee replacement surgery. I also want to say that the therapy and nursing care that he received was appropriate for what he required and we were very pleased. Any need was addressed in a very reasonable time frame and the administration was very helpful and communicative. The food was not what we were used to but we understood that they were following strict dietary orders to promote good health in regards to his diagnosis, aside from his surgery. My husband is discharging today and we are very excited to have him back home.

The reality is this. Not one nursing home is equip...

The reality is this. Not one nursing home is equipped to take care of that many patients with the staffing they have. Even the facilities that have low ratings have staff who genuinely care for the residents. If handling a sick elderly patient was so easy, you'd think the better option would be the family to do what is necessary to take of them on their own but many cannot be bothered with the constraints of doing so. I can't begin to describe how heart breaking it is for staff to see that these patients have family who could take them home with help of a provider and won't be bothered. Won't come and take the patient out for a nice outside errand, nothing. Families go at first, they make sure to give their over bearing demands to the staff as to give the impression they are a " hands on" family, but eventually those visits fade and happen less and less often.

Veterinarians will tell you that the best place for your domestic animal to die is at home in familiar surroundings with the people they know. Even if it appears that they aren't aware, they are. So why leave your family member in a facility to die and then complain about compassion? Where's the love for your family. Geez.

If I could give TEN stars I would! Las Colinas, We are beyond grateful for the quality of care you're providing my Dad and the peace of mind you have provided us. I set out to find the best care available for my Dad who needs rehab therapy and 24/7 care. Las Colinas has been the answer to all my prayers. First and foremost, the staff is exemplary. Too many to name, but, Jacob in admissions was very helpful and understanding of our needs, Everyone makes you feel welcomed and accepted. They are professional, kind, respectful, friendly and caring.Las Colinas has teams of people working together beautifully to provide the best care possible for their patients. Cleanliness matters, and they again have exceeded my expectations. Las Colinas is immaculately clean and is designed like a resort with warm colors, fireplaces, wide hallways, windows galore and large rooms with sturdy woodwork and wheelchair friendly bathrooms. We were able to take part and participate in their daily activities & church services. Last but not least, they serve three good home cooked healthy meals or your choice off a menu when you feel like eating - because everyone is an individual and they respect that. I have received warm hospitality, support and peace of mind knowing we can count on them day and night. Las Colinas, thank you for welcoming us!

Been here since sat afternoon. Has beeen the worst 1 1/2 day of my life ! The staff here is lost. The nurses don t seem to understands what you are saying. Shelby and Ben were the ones who stop to help me. One nurse came into my closed room throws on the lights at 12 am!! I had back surgery. That doesn t feel good or is good to be twisting like that. My bed doesn t work at the feet..... not good. They have nothing that was suppose to be order for me. I had to wait a whole day and a half for pain meds. Just FYI..... back surgery hurts like nails going down a chalk board. This facility is nice but I think staff needs to be re looked at especially one nurse at night. No linen. A really horrible experience. I don t think any facility I ever worked in has ever been like this. And I have worked all three shifts!!! Might have to AMA myself. I can t do better at my home

That is the worst place ever we had are dad at that ugly place he feel twice the last one he told us that a resident pushed him but the staff said he feel near the door of the room but after he got back from the hospital I guess the housekeeping did not clean well there was blood by his bed I told my sister how did blood get this far well after couple of months we took him out and put him in another facility and I know when we brought him to the new place he was going to be fine and he was happy at his new home my dad is no longer with us but I am glad he got out of that ugly place

The staff is not equipped for end of life Hospice care. The staff needs a course on empathy and Covid sanitation. No one takes accountability as the staff truly doesn t know who s in charge. Something terrible happened to my mother here over the weekend and it s Monday 4pm and still no one has called me with any kind of an explanation or even an apology.

This is the most dysfunctional group I have ever experience. Lots of samples:
day 1: Sat in his room for 1:45 min till family member shows up because the rehab center did not bother to come in and asset my dad.
day 2: No meds given.
day 3: Family member raises hell because he had been there and no bath and no meds. Family member finds out person given meds signs form stating they had given my dad meds. Later when confrounted by family member and my dad admit she sign the forms without given meds because she was in a hurry to leave.
day 4 and day 5 room has not been cleaned, bathroom not cleaned. Family member witness housekeeping asking if she could leave early without cleaning any rooms. No sheets on bed, sheets left on the chair.
day 6 no breakfast brought to dad. Dad had to call office to find out why he did not get breakfast.
day 7 no coffee min breakfast. Lunch and dinner always late. Again complaint and demanded Dad get a bath.
Feel sorry for some folks no TVs are working during holidays. Where dinning table is, the TV has not been working.
The folks that are suppose to be helping, gather in groups and talk and make a lot of noise in the halls. Sometimes you can not even here your conversations with your love ones in there rooms even with the door closed.
Please do not bring you love one here. Place is nice but the work ethics are terrible. Today is 28Nov2016. I doubt this will even post.

This nursing home is short staff on nurses and aides all the time. Lots of admin though. They are also short on supplies like diaper, wipes, water jugs, bath towels, trash bags the list goes on. They rarely have fresh fruits or vegetables just starches. They do not have the staff to feed the patients that need assistance. They do not brush their teeth, wash their face, wipe their nose or offer fluids. It is a constant challenge to receive adequate care for my loved one. Most families have the same complaint. Are hands are tied. The state does not really do anything, admin just gives you lip service right along with the staff. If you admin your loved one plan on being there daily, several times a day if your loved one does not have a voice. You will need to be their advocate. Be prepared. If is a constant battle. The bottom line is this corporation DOES NOT CARE ABOUT THE PATIENTS OR THEIR STAFF THEY ONLY CARE ABOUT MONEY, THE BOTTOM LINE.

About a year ago I transfered mom here to recover post stroke and heart attack. The doctors are good and so are the RNs. But that's where it ends. My mother had became sick, she told a CNA over and over again, she threw up all over herself. I couldn't get a hold of mom that day until late afternoon. When I did she was crying and saying she had been sitting covered in her own vomit for hours and no one helped her. I immediately had her transferred to an ER. The management kept apologizing to me for not helping her or notifying me. NEVER AGAIN. STAY FAR AWAY OR MOVE TO ARIZONA

Great place to stay! From the CNAs, to the Nurses, to the Administrators. Everyone here has been amazing at maintaining a family-like atmosphere and always having smiling faces for the residents and resident s family members!

My grandma died here, from negligence.. they did not give her her meds, which they knew she shouldve taken, and did not give her her insulin.. which ultimately she was found unresponsive in her room, for who knows how long... do not put your loved ones in this facility

RUN.....AWAY.....FAST....Don't be fooled by the nice facility and the friendly front desk person. Once you pass the front desk everything is downhill from there. The nurse call buttons go unanswered for extended periods of time. All the food seems to be ground beef based..then they wonder why nobody wants to eat. The personnel parks in the handicapped spots so handicapped visitors end up parking in back of the building 25 yards from the door because you can only enter the front door despite at least 4 or 5 other entrances. They seem to be obsessed with everyone using Medicare to pay...strange.

I am very happy with the care my brother is receiving here. He has been in several San Antonio nursing homes, and this one is the best. The staff is very busy but is also very responsive. My brother gets visited 1 to 5 times a week by family, and I have never seen or heard from my family any complaints about care provided. I have praised specific staff members and nurses about the quality of care provided, also how interactive the staff are with the patients. I guess I could find fault if I looked, but to see the staff giving the patients what they really need, caring and positive interaction. Really where it counts, GREAT JOB!

Terrible place. Instead of saying bye and wishing her well or that they're gonna miss having her there, one of the staff comes out and starts arguing with my grandmother about a wheelchair. Im glad we took her outta there. I wasn't surprised that there was others leaving there. Most of the staff is rude and they didn't even bathe my grandmother today.

Unable to post full review - likely size limitation. Posting a smaller version. Safe to say you do not want your loved one(s) here. Do not be fooled by the scenery. Horrible mgt. which in turn is responsible for staffing shortages. Most nurses are kind - a few are absolutely awful. Rehab work your loved one was placed here for does not take place. Food is usually cold and room visits by nursing staff are way to infrequent. We moved my mother-in-law to Sorrento on Fred. Rd. - a totally different place. Las Colinas mgt. never contacted us after we moved her out 10 days early. They showed no concern whatsoever. Operated by Genesis HealthCare. They should remove all mgt. here and start over. Sorrento on Fred. Rd. 1000x better than Las Colinas.
07/31/15 - update - spoke with the admin. for this facility on phone over a month ago about my post. She said that a review would be conducted and a follow-up call would take place. No such thing ever happened. She did mention Genesis took over in Feb. of 2015. I had no experience with this facility, but suspect the Genesis acquisition has taken things down hill.

Words cannot express how grateful I am for the car...

Words cannot express how grateful I am for the care and treatment my father received during his stay at Las Colinas.

During his hospitalization I was told my dad would need rehab services after he discharged. My brother and I made it a point to tour multiple facilities in our area before making a decision. I was very impressed with how knowledgeable the admission person was about his building, the breakdown of my father's insurance coverage and the tentative plan of care, which was a stark contrast when compared to other representatives we encountered.

From the moment he arrived my father received exceptional care. Nursing staff was very attentaive and his therapy team second to none. He has expressed to me that he would like to come back and visit with everyone in a few weeks when he gets settled. For someone who was so against going to another healthcare facility from the hospital, it was amazing to see the transformation from a reluctant participant to an over appreciative success story.

I knew I would leave a review but was taken aback by the reviews. After some research, I have come to find that the majority stem from a photo of food that went viral years ago and an overwhelming amount of negative reviews are from people that have never even experienced their service. (By the way, my father had no complaints about the food whatsoever.) For instance, on Yelp they are 3.5 stars. But if you click the sub-link at the bottom, 11 of the 16 unlisted reviews are 5 stars. Of those were included, they would average close to 5 star.

Thank you Jacob, Matt, Jason, Apryl, Abby, Chris, Caleb, Jennifer, Patricia and Carmen and the entire team for everything. We are forever grateful

The kitchen staff have very little work ethic. The entire scheduled kitchen staff decided NOT to show up on labor day. Thank goodness for the wonderful nurses and assistants. The nurses took the reins and cooked for us, it was actually better than what was usually served. The kitchen usually never fixed an alternative menu choice although they published a choice. I even met with the chef and discussed my diet. I am a bariatric - surgery person therfore my diet will always be different from the average person. They mostly never took this into consideration.
My nurses and nurse assistants were wonderful but you can only do so much when you are always understaffed. I had a wound that had to be changed every other day. It was a very painful experience. The wound care nurse there is awesome, I can't remember her name now but if it wasn't for her, I would be dead by now. She was able to get a Dr to look and except my case.
The dr that saw me once a WK there had no experience, she was nice but most of the time she forgot what she said she was going to have done before she walked out my room. She did prescribe me iron shots because I was so anemic. Don't let a Dr do this to you. I now have "iron stains on the skin of my upper arms. I can't wear sleeveless shirts anymore. It looks like I have terrible bruises on my upper arms. It looks like my husband beat me. I always wear a jacket or a shirt with sleeves that can cover it.
It was a terrible place to be but thank God for the angels he has placed thereto watch over the poor souls who have to be there. I thank him for the ones I had.
My mom (73 yrs old) brought me food I could eat practically daily so I'd have something to eat.
There is a problem with theft. I had to be taken to the hospital twice. Each time items of mine were taken. The first time it was a healing blanket and a throw. The second time it was my make-up. It's sad that someone I knew there was not trustworthy. That to me is so important. People should be able to trust the people who are taking care of someone they love.
I was told before this Corp bought this place it was so much better. I didn't return after the second time in the hospital, I went to another facility. It was old but everything was so much better.

I am currently a guest at Las Colinas of Westover. Today, I am completing my therapy program and discharging home. I wanted to take a moment to express my sincerest gratitude to the entire staff for the excellent care provided to me.

I was at another facility a year and a half ago and it pales in comparison to this facility. There are no words that I could say that would justify the level of appreciation I have towards the staff for everything they did for me.

I would highly recommend this facility to anyone needing rehab services. I am excited to go home and I know I would not have been able to do it without everyone here. They truly care and it shows.

Just discharged my mother Friday 08/30. This morning she asked to go back.. Truth is that I read the reviews prior to admitting my mother for rehab, I had a good feeling about this place regardless of several bad reviews; so we decided to admit her.. I Thank God for giving me positive vibes about Las Colinas. It has been the best experience we have had.. The place is gorgeous, the staff is so friendly and you're loved one is not just a patient/resident, they're treated as family.. to tell you honestly that my mother wants to go back tells you everything.. when I'm ready to let my mother be a resident I will look no further than Las Colinas.. Thank you God for putting Las Colinas on our path..

I fell and broke my patella. Spent 2 weeks at hospital then came to las Colinas, for rehab. I had to quarantine for 14 days. But anytime I needed assistance I got it in a timely manner. My physical therapist were good about coming to my room and getting me to do my exercises and strength therapy. I m a diabetic as well and have hypertension and my nurses were on it about checking my sugar levels and taking my blood pressure to make sure if I needed my my insulin or my blood pressure meds. I was very impressed with my nurse Vanessa she s a 21 year old with 2 kids . She was super sweet and friendly and genuinely concerned for my well-being. As was my night nurse Mark he was great with me as well. Once I was out of quarantine and could visit the therapy room, my therapist Caleb and Jennifer were awesome and they helped me get to meet all my goals. The aides were there to assist me whenever I pushed my call button too. Super friendly young staff mainly college students. But very respectful and willing to meet your needs . I would most definitely recommend this facility to anyone who is looking for long term care and for those looking for rehabilitation. They have new administration and lots of new nurses and aides.so please don t go by the old reviews. They cleaned house and hired all new administration personnel.

My loved one has been there for a little over three months. They have new staff. My loved one has been requesting to see her doctor and has yet to see her. They still don' t have a social worker. We have not even had a plan of care meeting. They are quick to collect the fees but, the customer service and quality of care is poor. They even have an ombudsman that visits on a weekly basis and that does not seem to be working for the residents. This person works for the state. If you have other options i do not recommend this place for your loved ones. The new director is never to be found either.

Everything that has been posted is absolutely accurate. After 1 week of my loved one being admitted some of the belongings were "lost", CNA'S do take forever to answer the call light, there is a nurse by the name of Luis who works on the weekends, he's not professional at all, he talked over me when I was trying to address a concern, after he argued with me he checked my loved ones sugar and stuck my loved ones finger in a very ugly manner. He really seemed frustrated and annoyed. They do seem short staff. It is true that they forget to dispense medications to the residents because they did it to my loved one on the 3rd DAY after admission. ALL of the staff seems to lack communication with each other. My loved one can not swallow and the medication aid was going to administer the medication orally luckily a family member was there to stop it. After that they NEVER came back to give the medication that was due. My loved ones toilet was clogged the smell was unbearable one of the cleaning ladies told me that she had reported but no one came to unclog it. It's very concerning what these people are doing. The facility is beautiful but most of the staff is lousy, unprofessional and they lack compassion.

So far the only one that seems to show compassion and professionalism is a lady that wears black scrubs I forgot her name she was very nice and even called me to give me an update.

I rated this facility a 5 simply for the fact that it was so clean and the friendly staff showed so much kindness, and passionate attention to my husband and his needs. My husband's specialty doctors had pretty much threw in the towel implying no more could be done for him so i decided to let the facility's doctors take over. With fresh eyes the staff got my husband looking and feeling so much better. I had not seen him look this good in years.
If you want the best, look no further as this facility is by far the best place to be.

My mom was hurt by workers here she just left the hospital with fractures on her hip ,femer and spine, she was also over medicated ,she was also been starved DON'T leave any family member here if your love one is in this facility mostly now that you can't visit good luck we moved my mom

I recently admitted to this facility for therapy and completion of antibiotics. After one week, I am already set to discharge home. They reserved a private room on the rehab unit for me and it was great. The food was delicious and the employees were awesome. Would recommend anyone who needs therapy to consider this facility. I am discharging today and I am very grateful to the entire staff for everything they did for me.
