Walked in for information, the workers were very helpful very determined to fix the situation with an older relative. Nurses and doctors very attend full. Nurse Ian walked me through the whole facility showing a great environment we are looking forward towards the future.
The facility is clean and the communication is great, I enjoy working here and caring for the residents.

Omg I am so happy I got to facetime with my mom today and she looked so good. I appreciate the staff caring for my mother and I am hoping for her to return home as soon as possible.
They treat me good. The administrators, nurses, cnas, RTs, rehab team, medication aid, and every people who serve this place. Especially Ian and everybody. I used to be on a trach, g-tube, covid-19, after 3 months I don't have a trach, g-tube, or covid anymore. I'm fully recovered now. I highly recommend this place for anyone that needs skilled care especially ventilator.
Please don't send your loved one to hell - Meridian Scare. This place is a joke, they put the residents in their rooms and then don't do anything about caring for them, they restrained my mom without consent. Dementia patients were free to roam in and out of other residents rooms scaring my mom. The facility has old equipment that is gerryrigged. When I visited she had blood soaked through her gown that was so dried it stuck to her stomach when I pulled it away. The nurse said my mom spilled her breakfast, BULLSHIT my mom wasn't even capable of feeding herself and it was BLOOD, she had a sore where it came from and they wouldn't answer my inquiries as to how it happened. My mom cried the whole time there that they were all mean to her. My mom even complained that someone was hitting her. She is not there anymore and will not go back, thankfully she is in a better facility now that is awesome and actually cares for residents. She's thriving now and I believe would have died in Meridian Scare if she had been there any longer. ZERO STARS!!!! Again, DO NOT put your loved ones there!
As a speech pathologist who has worked throughout San Antonio, Meridian Care provides excellent rehab services for physical therapy, occupational therapy, and speech therapy. The rehab team works effectively together to help residents achieve goals!
Whatever you do do not take your love ones to be treated in this facility my wife was treated there and it was a horrible experience while she was being taken care of I got a call one day telling me they had to take her to the emergency room because she was non responsive I went to the ER and I couldn't believe my eyes. her treat collar was so dirty it looked like it wasn't changed in days and it was stuck to the back of her head in her hair she had bedsores and her lips were peeling like she was in a desert for weeks and she had a very bad Infection I had to take her to a different facility where thank God she is doing better
I was in Merdian for 11/2 years. Those people were a God send for me and my family.I came to San Antonio flat on my back,in an ambulance. Not able stand on my legs or move from a bed to a chair without help. They had to use the lift on me because I weighted in the 350 pound ball park. I was on a vent and was told I would never be off the vent by the Doctors in Oklahoma. Thats How I came into Merdian. Today, I live independently in a senior appartment complex in Schertz. So don't let anyone on this web site tell you not to put your loved ones in their hands. Merdian Staff and fellow patients, are the greastest people in the world. I am off the vent and off oxygen and I am leading the kind of life I want to.God Bless
My father was at Meridian for 5 1/2 months before he passed away recently. Both my brother and I were very pleased with the care given to our father during the last few months of his life. Our father was a 'trach' patient requiring 'on demand' trach care from the respiratory therapists on staff. Prior to being at Meridian our father was at two other care facilities in San Antonio that were not able to provide basic 'trach' care. In addition to proper respiratory care the staff at Meridian provided support and comfort to not only our father but to both my brother and myself. Over the 5 1/2 months at Meridian I spent an average of 26 hours a week with my father and the staff. I would like to thank the many staff members who we came to know personally as good people: Abu, Shelley, Issac, Samson, Natalie, Ausencia, Carrie, Fe, Chris, Sue, Barbara, Tricia, Asia, Cynthia, Marianne, Richard, Betty, Alan, Steve, David, Wendy, Sharon, Cody, Frankie, Bobbi, Rosa, Nadine, Frank, Chrystal, Claudia and Ms. Barbara Wilson. I apologize as I know I have forgotten some names. One CNA in particular who would be with our father during the weekday mornings (her English was not the strongest but her heart was very strong; I believe she was from Cuba).

I been with Meridian Care for 11 years as a Med aid great company to be part of, Staff are amazing.
My wife was at Meridian approximately 7 months, before she passed away from ALS. She loved the staff at Meridian and even though they were slightly understaffed during the weekends, they made every effort they could to attend to my wife's needs. Twice, after my wife had to go the hospital due to ALS complications, she was very happy to return to Meridian after being in the hospital ICU, because it felt like "going home" she said. The nurses were excellent and the RT's were also excellent. My wife was on a ventilator full time and Meridian did an excellent job of managing that part of her care also. Her medications were on time. The CNA's displayed care for her as if she was family. I really appreciated that and so did my wife. Sure, there are always a couple employees that can sometimes be stressed, but that happens everywhere! Both of her doctors were also very good and the department directors and therapists were also very attentive to my requests and my wife's requests. Thank you Meridian for taking care of her so well, during that hard time. I'll always feel gratitude for the way you took care of my wife. God Bless you all.

our family is extremely happy with the care of our love one whom has been cared for at Meridian for 9 years. She is well taken care of and loved. We have developed a personal relationship with her caretakers and know that having that one to one communication about our sister is a must. I know that at times there has/is a shortage of staff and that the administrator must make this his priority to keep staff well trained and paid so that our love ones are given the continued proper care. I'm just mad at myself for not taking the time to investigate this facility and staff throughly prior to my husband's stay! We can't get out of here fast enough.
Meridian Care Crestway has the best professionals and has given me a better perspective in helping my family with therapy and their medical needs. I was given a tour of their facility and was immediately taken care of and I could not have asked for a better place for my family. Their devotion to helping their patients and assisting families in need is important to me and I highly recommend Meridian Care Crestway to anyone in San Antonio!

I hate this place!! If it wasn't for their ignorance leaving my father in his bed after he fell for over 15 hours (6 am till 9 pm) "UNRESPONSIVE" he would of never suffered his last few month of his life that you guys helped end. He was able to walk, speak, and move, but after a few short days of being in this place all that came to an end. IF YOU CAN'T DO THE JOB THEN GET ANOTHER ONE CAUSE PPL TRUST THEIR LOVED ONES AND PAY FOR YOUR SERVICES!!!!!!!!!!!
I ve been at meridian for about 2 years and it s by far the best facility I ve been to ,even the one my grandmother and grandfather were at in Louisiana pales in comparison to the care and overall feeling at home here. I just wanna thank all staff members and upper management and administration for the experience I ve had and continue to have !!!!!

THIS PLACE IS TERRIBLE!!!!!!!!!!!!!!!!!!! If you love you family members AT ALL. DO NOT send them to this place. But by all means if you don't care about their care and how they are taken care this is the place for you. They are EXTREMELY under staffed especially on the weekends. Getting them to do ANYTHING is like pulling teeth. When a patient is supposed to be turned every two hours and goes all night without even seeing one of the CNAs that are supposed to do this is unacceptable. As well as not getting the medication on time and in the case of my family member he would have spasms when they did not get it. They promised many things that they NEVER delivered. If I could give this ZERO stars i would because it does not even deserve the one.

I have been working at this facility for 4 years as a Respiratory Therapist and have loved every minute of it. The level of care here provided to these residents is second to none. This place literally is my home away from home!

I enjoyed living here. I've met many people the staff are very helpful when you need help. We have many activities like bingo gardening arts and crafts variety games and entertainment. I'm pleased being here.
I called Meridian Care today, May 7, 2018, to inquire about visitation hours. I was trying to pay a visit to a friend. After the phone ringing several times, a lady answers " hello ". - Very unprofessional! I ask visitation hours and she replies, " 8 to 8, I think ". I've not even been there yet and already hav a bad opinion.

This place is posting fraudulent reviews to boost their stars. It was 3.0... It s all from people working there that are making the positive reviews. Do not believe it. This company is not updated and VERY short staff as well as unorganized/rude.
I called to check on my mother, waited 20 minutes. I hung up and called back, waited another 15 minutes. Neither time did anyone pick up to ask what i was on hold for or sorry for the wait , NOTHING. There was also an issue with her breathing in mid September. I called and spoke to the nurse about it. She said she would get with the RT that took care of her and let me know. Got no info from her as of yet. Mom had a care plan meeting first week of October. Spoke with the social worker about it. She said she would refer it to another person, Faye. I still have yet to hear anything. There have been a couple of times where i have stopped by to drop off a goody bag for mom. Both times there were people in the waiting room without masks. My mother has been here for quite some time now. A majority of the time things are fine. I seems the longer that visitors are not allowed to see their family members the more difficult it is to actually get information on how their doing. There is a lady that calls me once a month from a sister facility about my mothers care. I guess I will have to refer to her. It is also been discussed about my mother to plan for dialysis. The social worker there is telling me that she is told mom is not a candidate, but yet when my mother has to go the BAMC they say they don't see why she couldn't have dialysis in her current state. I'm just confused and frustrated. I feel as if I am being brushed off to the side.
This is a great advance Rehab & Nursing Facility that has shown to have done so much great work with so many. Over the years the facility has developed with the time and upgraded and advanced to help so many people recover. This is a great place to work and to receive rehabilitation, they are not just a facility they are family.
Since I came here I was unable to talk, move around, and they gave me therapy. Now I am able to speak, write, and move around. Respiratory therapist takes good care of my breathing and treatment really well. Rehab therapy has been helping me a lot that now I'm able to do so many activities. I'm doing well.
I worked there for two years. Everything Emma said is completely true. It is a horrible place to put your live ones.
It s sad to come visit my family member and not even have a chair to sit in for my visit. It s definitely not a place that encourages visits. As I walk down the hall way none of the rooms I saw even have a chair for visitors. Facility needs a good cleaning and a ton of updates. Super sad
This place is amazing . I so happy this facility is in San Antonio.
The only reason i would give this place one star is that it has indoor plumbing and the roof doesn't leak. I have a family member there and we have to go EVERYDAY to take care of him,check to see he has oxygen , make sure his socks are not rolled down around his ankles,cutting circulation off to his legs and ankles.One day who ever changed his bed forgot to give him his pillow back.

Can only visit till 3pm and no weekends. Your family member is in a prison. Why such strict visitation policies ? Don't they realize that ppl have to work to pay the outlandish cost of this prison at nearly 12 grand a month ? Who gets off work b4 3pm anyways ? They purposely don't want anyone to visit so they have unrealistic visitation times. Why no weekends ? Absolutely pointless and cruel to the patients !
It does more harm than good not letting your loved ones see their family. They blocked us from visiting at all for first 15 days even though our loved one just came from being in the ICU at a nearby Hospitol for over a.month so zero risk of covid.
Our loved one thought we abounded them as the staff never called us, would never return calls and repeadly left us on hold for over an hour everytime we called for an update. State prisons have more visitation rights than this !
Of course we eventually found out why they dont want visitors.. You will see the lack of care and attention to detail. Our loved one instead of getting better went downhill fast and got several bad infections that almost killed them... Luckily we realized what was happening fast enough and had them moved which ended up saving their life..
Stay Away.. You have been warned....
Patient care is awesome. Staff works together as a well oiled machine. Everyone always very friendly in all areas of care from house keeping and dietary to the nursing and respiratory staff. Would definitely feel comfortable having my family member taken care of by these caring individuals.

Hello my name is Oscar and have been a part of the Rehab team @Meridian Care going on three years.
The therapy team at Meridian Care are amazing and do the best they can to help rehabilitate patients in order to return home or improve functional mobility with in the facility.
DO NOT SEND YOUR FAMILY THERE AT ALL. I REPEAT DO NOT SEND YOUR FAMILY THERE AT ALL. My mother contacted scabies there. They NEVER treated her for the scabies. On Christmas December 25, 2017 I took my mom to University hospital (she's currently there now) because of the horrible swelling, scars on her ENTIRE BODY and the wounds on the backs of her legs. When they took the bandage off of her legs her flesh came off. University Hospital Social Workers are filling a complaint with the state because this the worst case they have ever seen. My mom is only 49 and bound to a wheelchair and she's a stroke patient. Fe (fay) is a TERRIBLE charge nurse. I didn't receive a call from them when they dropped my mom or when she fell out the bed multiple times. My mom would call me to tell me. I would try calling Fe or texting her but she would ignore my calls. DON'T SEND YOUR FAMILY THERE!!!

I have only been with this company a few months, but I will say it is very family like everyone here has been wonderful, all the staff from nursing down to the kitchen staff has been very welcoming and always helpful and willing to help. The residents and families are great to work with. We recently had a snow storm and the DON and ADON came out helping staff get to work. Everyone here is very passionate AND HELPFUL, this is a family/ team they have here at meridian. I love my job and love working for such a willing company.
I have been working at Meridian Care for more than a year now and I am impressed by how this facility handled the pandemic situation! We have NO out breaks, Very strict protocols, and strict Guidelines to follow. All of which was to ensure our residents and staff are SAFE and constantly have their guard up by having weekly updates. It was and still is a difficult situation but we are continually striving hard to better this place. We may not be perfect but we are adapting to every circumstances that comes our way by remaining vigilant.
I love the staff because of their compassion, knowledge, and very helpful. The respiratory therapist are a life saver. They take good care of me. The nurses are awesome. They are keeping me well and healthy. I feel comfortable throughout my stay. Thank you Meridian.

My Dad was placed here out of necessity for about a year. I would say that compared to other nursing homes, it has been the worst in regards to patient care. My dad almost died out of negligence from the nursing staff for taking him off of blood thinners without consent from the doctor or medical power of attorney. In addition, a family friend also passed away while under their negligent care. CNA'S would also ignore my dad's call light after entering his room and turning it off, without resolving the issue. Most of the staff just seems to disregard the residents' feelings and it seems to be coming from upper management. I would strongly urge anyone not to put a loved one in their care. Very bad care.

Great company to work for, staff is friendly and provides compassionate care for each patient. This company has a great management team that comes together and works hard for their patients and staff.
For the most part, the staff is great. There are some CNA s that need to be retrained or change professions. I have a degree in social work and I have friends that work with this population and I hear horror stories about other facilities. As long as you visit with your loved ones on a regular basis, you ll be able to see anything negative about their stay and report it to the Director of Nursing.
I have been working for Meridian Care for three years now and it has been a great facility since the beginning. The staff is very caring for all the awesome residents!
