CareOne at Holmdel Review

This is my first experience with a rehab center/nursing home didn't know what to expect based upon reports of nursing homes in general. The
staff is very accommodating & caring & feel very lucky to have my mom
in such a caring environment.

Not very happy about the lack of service. My relative is fully cognitive but physically weak due to surgery recovery. She has been in this facility for a temporary stay of over 2 months. During this time she has experience impersonal health care workers who delay answering call buttons to the point that she has to regularly soil herself then sit in the mess few sometimes several hours...especially at nigjt when family cannot help. When she asks for help it is not met with action. She was conviniately skipped over for a shower and has now waited for a shower for 7 days. It takes regular requests from family to motivate workers to do their jobs (give her required medicine, change her, help her to the toilet, etc). Keep in mind this is a woman that is fully able to have conversations and communicate her needs. It is disapointing that she has been putting up with this demoralizing treatment. I would not recommend this facility unless family accompanies the patient most of the day to "encourage" workers to do their jobs.

I am so happy with the team effort that you receive in your rehabilitation. Always spoken to with a CAN DO attitude. The personal care you get in a crowded gym proves they are committed to your healing no matter what your limits are. All the therapists are the greatest at getting you walking again. Dean, Maria, Monika, Ark, Cindy, Holly, Jessica. Beth are some of the most caring people in giving you your life back. Maria has an infectious spirit to lift you up and make you feel you can get well. Monika makes sure you understand what is going on with you and how you will heal.

The nurses here are above standards. Their dedication to each patient is comforting. Gladys, Cora, Sammy, Dana, Irwin, Lonnie, Kathy are on top of your care and have no problem calling the doctor at all hours of the night to ensure you get well.

The backbone of this place are the nurses assistants. Trina, Gitana, Alicia. Maureen, Destiny, Johnson, Debbie, Carmella, Ginny to name a few, take a humiliating situation and make it as simple and easy as possible like it's been your normal routine. Their compassion is the key in comforting you as you heal. They are Angels that come along with you in your rehabilitation.

And when it comes to house keeping, Janet rocks with keeping my room clean.

I did not know what to expect from a rehabilitation facility. I was treated nicely and respectfully. I also received therapy to help with my full knee replacement. This was also necessary and helpful. The therapists were nice and knowledgeable.

This is one of the best facilities for rehab! Their OT and PT staff and services produce such positive results for me. I'm still in residence, but think it will only be a short time more before I'm home and independent again. The care by the nurses and aids is wonderful and they are so comforting. The facility is clean and there is a lot to do for socialization. I recommend it highly.

Care One has been great in helping me stay informed and navigating care for my mother! Highly recommend.

My mother was a resident and I saw Careone as a family member. Then I broke my hip and saw Careone as a patient. I went into the hospital again recently and was very nervous when they sent me home. Luckily for me, I was very educated from my last stay at care one and I knew I could call them to help. Everyone welcomed me with open arms when they saw I needed some extra TLC. My days are filled with great food, activities, and most of all the therapy I need. The nurses were able to regulate my medications and the cna s took great care of me! I cant not thank everyone enough. I cant single out one person as the greatest because everyone here is educated and respectful.

To my grandfather and family while he was there. We were always notified of anything that was going on when my family was there.
During COVID the team and facility were wonderful as well.

Greatest activity director! Michelle Fugett really cares, genuinely!...she creates such an upbeat positive environment! Clean, upscale facility...caring staff!

My 97 year old mom has received great care here for many years, by a loyal and caring staff of professionals. She loves the activities, trips, and special events. Had a ball at the recent going to the dogs day, where many rescue dogs were there for the residents to pet. The new patio area is beautiful and really lets the sunshine in to the residents lives.

Where do I begin...5 Stars all around. Care One Of Holmdel by far exceeded my expectations. I was nervous when I had to pick a rehab facility for my mom. I am so thankful I chose Care One Of Holmdel. Mom was there for 6 weeks. It became a second home for all of us. The best nurses, aides, PT & OT, maintenance and housekeeping, chef and kitchen staff, activities staff and administration. I highly recommend this place. I can t thank the entire staff enough for their hard work. No one is sitting around doing nothing at this place. It s exceptionally clean, professional, attentive and friendly. I HIGHLY RECOMMEND CARE ONE OF HOLMDEL NJ.

Can't say enough good things about CareOne at Holmdel. Every person there from every department are caring & considerate of all the patients & residents. Their well being is paramount to all who work in CareOne at Holmdel. This is wonderful place for your loved one to be taken care of like they were home because it is their home & thats how the staff make them feel.

Care One of Holmdel is just perfect - their nursing staff, their aides & especially their Physical Therapy are wonderful! They are always alert & ready to help whenever you need them! If you are in need of Rehab Care One of Holmdel would be a perfect choice!!

My mom started her sub accute rehab at Care One in Holmdel. On her 4th night, she aspirated and started coughing vigorously. She requested help from the nursing staff several times during the night and was ignored; she requested 911 and was dismissed. In the morning, a chest x-ray confirmed that she had developed pneumonia. I do not recommend this facility to anyone that requires medical attention.

Presently a resident here. Staff is great, from CNA's to Administration. Nurses are attentive and caring and food service staff work with you regarding your nutritional needs. Housekeeping staff do a wonderful job maintaining a clean environment. I was impressed at how everyone knows your name. Overall very pleased.

Nurses are unresponsive. night time you're on your own. they're disrespectful to patients and family members. they don't provide help if it is an inconvenience to them.

As I prepare to leave Care One I want to thank all of the therapists who contributed to my recovery and rehabilitation. All were professional and a joy to meet and spend time with. I was a patient at CareOne, Holmdel for seven months except for going home for three weeks in between. Needless to say I met employees from every department! I had the opportunity to see the staff on an average workday and on the worst days during the covid outbreak. These people are to be commended for the care and service they provided to the patients during this time. Fortunately for us, they went above and beyond their duties. We felt like a family, sharing tears and laughter. I could probably thank each person by name but don't want to embarrass myself by misspelling someone's name. I guess you can tell that I found the staff and facility to be wonderful.

As someone who spent many days and many hours visiting my father in law at the Care One facility in Holmdel I can truly attest to the fact that a patient there receives excellent care and therapy. All the staff, including the nurses, aids, activity staff, cooks and kitchen help, and the administration do an outstanding job and provide the care and support to the.patients.I am very satisfied with the level of care my father in law received and with the respect and kindness he was shown. All the staff I came to know over the 3 months I spent there at Care One in Holmdel visiting my father in law were all professionals and wonderful people. I can recommend the facility to anyone in need of the services they provide. A top notch operation.Thank you allfor your wonderful help.

At this time of personal family stress and events that are out of our control Careone Has been supportive in every way. Our mom Has been treated with the upmost dignity and professionalism The complete medical staff including all nurses doctors and the general staff are always on hand to support and provide our family and mom with communication and timely updates. I would call this facility Care None but I can t take credit for that because I believe it was used before.
The one star is one star more than they deserve. My father was only here for a brief time before he was transferred to a different facility.
Upon transfer they forgot to send his clothes when they did they sent them dirty. But that s not the worst of it,
They lost the dentures of my father a 95 year old veteran a member of the Greatest Generation.
I have called there for three weeks not one of the social workers has returned my calls.
There has to be more caring places to take your loved one.

My husband just came home today from Care One and I would highly recommend this place. He was there for the past two months and when he arrived I actually thought he was never coming home. They worked with him every day from Natalie the speech therapist, to Monica for PT and Nicole for OT. They did not give up on him. The aides were wonderful. Miguel was one of my husbands favorites. All the nurses and staff were amazing. Dana, one of his nurses, well she became a good friend. The activities director/coordinator did a great job. Everyday was something different and you can choose what you wanted to do. I even got to know the chef for visiting every day. If anyone has someone that needs a good rehab facility this is the place to go. My husband is home today because of their care and compassion.

DO NOT SEND YOUR LOVED ONES HERE. Being an old employee of a care one facility, I had high expectations for my mother's care, what she received was far less than acceptable. Newly diagnosed with lung cancer with mets to her bones, she needed some PT and pain management. Her meds were rarely on time, staff gave unprofessional care WHEN they provided it. She suffered in pain while there, until we, the family called the doctor and asked for changes to be made, normally a nurse would make that call to doctor. And even after changes were made, she still did not get her meds timely. A confused male patient was allowed to enter her room multiple times, terrifying her while she tried to rest, she called for help and it went unanswered. Only when the family called the nurses station and questioned why the man was in her room, was he moved away from her. Then we demanded discharge against medical advice, as we felt she would be safer at home, they continued to drag their feet getting her supplies set up for home until the last day the insurance authorized her stay and she still went home without some things that were needed. They do not care about the patients here. COVID made things increasingly difficult because no one could advocate for her because no one could visit. Horrible experience, one star is even too high of a rating.

My Mother-in-law has been at this facility since becoming unable to care properly for herself 1.5 years ago. She and our entire family are very happy to have chosen Care One Holmdel. The staff is very caring and diligent and the activities director Michele F. is sure to keep everyone happy with the day to day events and activities that keep the entire population happy. It's a great feeling to know your 94 year old mom is safe and cared for - and that's what we get by having her at Care One Holmdel.

My mother was at CareOne Holmdel for weeks and we had a wonderful experience. The Therapy was exceptional along with the delightful staff.
The facility always smelled good and everyone was always smiling. I would recommend the facility to anyone interested. Tony S

The aids and nurses are lazy and your loved one will only matter if you can make regular visits and complain loudly.

I had excellent therapy and learned a lot. I gained more confidence in myself through having very great knowledgable therapists. The food was also very good.

Care One took excellent care of my father in the last days of his life. Knowing he was there with that amazing staff during this difficult time in the world really eased my mind when I couldn t physically visit with him until they allowed window visits. I highly recommend this facility to anyone needing a safe place for their loved ones!

Very clean and probably the nicest facility like this I've seen.

My mom went into the facility in March for rehab from having a stroke and surgery. I can t express enough how helpful they were to my moms needs. A team worked with her every day for physical and speech therapy and a nutritionist. Due to Covid visitors were locked out, only leaving family to be able to call and see her at the window. During this emotional time, the compassion each staff member expressed, was very heartwarming. Working with such a nice team made it a bit easier knowing our mom was in good hands.

I've been using the ACP Omni vision for only about 3 days now, and it has helped me with my balance and walking around. I started today using the walker and after using the Omni vision today - I've also started using a cane again! This is great! I am still really nervous, but it is helping me feeling more confidence in myself. We even were able to try the stairs, Twice!! with the cane, with assistance from my Physical Therapist. Thank you

My brother has been a patient there for 7 weeks. Having been diagnosed in August with end stage cancer. On some days he attempted to participate in therapy and other he did not have the strength. Many, many time he would tell me, the girls are wonderful , the guys at night are great . Daily he verbalized his appreciation for all they did for him and stated he never knew that there were people out there that cared so much. Words can not express the appreciation and respect his sons and myself, his sister have for everyone that made his last 7 weeks on this earth as good as they could be!!! He passed away tonight not only surrounded by his family but in a place that goes above and beyond, to care for other human beings. Not because they have to , but because you can clearly see, the want to !

This is the second time we chose CareOne for our family member. We didn t even research the second time because our family member had such an amazing time the first time around. She came out of SAR stronger and better! COVID times are rough for everybody. The people at CareOne worked hard to allow us to see our loved one while being safe. They helped us with window visits, outdoor visits and virtual FaceTime visits. We are grateful our family member is getting such good care during the pandemic. Thank you to all the staff, nurses, nurse s aids, PT, OT, SW, housekeeping and chefs!! A big shot out to Michele who treats my great aunt with so much love and care.

I would like to thank Liz Renner and the CareOne Holmdel staff for taking excellent care of our Mom. In these unprecedented times, they kept in constant contact with the family and got her back on her feet after a 6 week hospital stay. We were able to have Facetime calls and the Staff and Therapists filled us in on her progress daily. Thank you for all you do for your patients! You are all Heroes!

This is my fourth stay at this building and I've spent up to 100 days during one of those stays and I wouldn't chose any other rehab. The staff at this facility make me feel like I'm at home they are loving, caring, friendly, and compassionate including all staff, housekeepers, nurses, therapists, aides, and administration. I especially come here for the therapy because I know they will get me back on my feet! I would recommend to anyone who asks where to go for the best care.

Care one is doing a great job taking care of my mom after her recent hospital visit. They have afternoon and morning physical therapy sessions three meals a day in a nice dining facility with tablecloth s and waiters and waitresses. They also have an outdoor patio and a barbecue on Fridays where you can choose from hamburgers, hotdogs, chicken, drinks (virgin daiquiris) all served on a sunny patio; it s beautiful here!

They have social events every day. Mom played poker yesterday (chips not money) and they had baby goats that visited last week.

Maryanne the aide at night is the best! She takes great care of my father and makes sure he is happy. Maryanne is a hard worker and an asset to Care One. Nicole is a awesome OT and very kind too.The facility is nicely decorated inside and very clean. Parking is easy. I would recommend Care One at Holmdel.

My experience with Care One in Holmdel, NJ. Reads like a horror story. My Mother was there for 24 days my siblings and I would phone and get no answer constantly. They kept the call bell and the phone out of Mom's reach. She was left to lye in urine all night until just before the next shift. She was abused while being changed. Legs pulled up in the air while she sreamed in pain during changes. When they knew her entire body was in pain. Thrown from side to side like a rag doll also during changes. Disregarded when she asked for help. Her tray and utensils were taken from her while she was still eating. She requested help from a certain male aid once they finally answered the call bell. On the rare occasion it was in her reach. She was told she had to do it herself. The facility should change there name to Do Not Care One. Do not take your family members there. You will regret it.

My mother was in the recreational room and there was someone else/e feces at you feet. I noticed it as soon as I walked in. Horrible care ........extreme neglect

The staff at Care One in Holmdel are kind, caring, compassionate, professional and resourceful. I so very much appreciate the services they provided me with during my stay there.

First rate experience. Nursing staff was top notch. Whenever we asked for help they responded quickly. There were times we were going to ask for them to reach out to hospice and they had already made the call.

I can not say enough about the staff and the care here at care one holmdel. I came here after I had an emergency trach put in after I found out I had cancer reoccur in my throat, i also have a gtube now. The nurses have taught me how to do my own feedings just in case i have to go home with the gtube. The therapy department is great as well, I am getting some weight and muscle back since I lost so much with the cancer. If i could rate the staff and facility a TEN I would!

Care One Holmdel is directly responsible for my Grandfather passing from Covid-19. Beyond this they managed to fail to provide proper care including providing proper catheterization and medication. After a patient a door down from him was found to be positive with Covid they didn't take the proper precautions to keep him safe and then acted as if he didn't have Covid, trying to say it was simply pneumonia.

Nothing is perfect, but this places comes pretty close! the front office people do their jobs as well as anyone could possibly do. The departments such as, entertainment, activities, physical therapy, kitchen, house cleaning, maintenance, nursing, and the business office, so well done! The nurses, the aides - all the people that care for all of the residents here is beyond fantastic. Every request, whether medical or personal gets done. Nothing goes undone. The men and women that do their jobs here must be praised and thanked! This place deserves the highest rating that is possible!

CareOne at Holmdel is an exceptional rehabilitation facility! The individual patient care and genuine concern is felt across the entire staff. The administration is compelled to make your loved ones stay there comfortable and safe and keeps the family informed. Our mom has been in and out of CareOne at Holmdel for rehab, for various physical issues, over the past 5 years. Nursing, CNA, OT, PT, Social Services are fantastic and most have been at that facility for many years and have a keen knowledge of past patients which made our mom (and our) adjustment much easier. In addition, the Nutritionist and Chef will accommodate the patients needs and desires to be sure their diet coincides with the rebuilding process as well. The efforts from all at CareOne strengthened and rebuilt her physical and mental abilities to allow mom to go back home each time. Our experience at CareOne at Holmdel has always been extraordinary and we appreciate the dedication and full team efforts each time!

The physical therapy team is top notch. My husband has been here for 13 weeks and his OT therapist Maria and PT therapist Monika have been wonderful. It is so obvious that they are so invested in my husband's recovery. I can not thank them enough for the progress my husband has made.

Care one in Holmdel is a great rehab facility..everyone is so nice from the first day I spoke to Heather she went over everything in great detail about helping me when my mom was going to go in for rehab., she is so wonderful & compassionate about her job & helping people.One of the most important is keeping me in the loop and always staying in touch with me updating me on my moms progress, so I would always know how she was doing and especially keeping her safe during covid 19. The facility and the entire staff was so diligent during the most stressful time have a well planned program to keep everyone separated from people just coming into facility from rehab.havinging them quarantined to then moving her into a separate part of the building to h
Keep everyone safe and covid free..I was extremely impressed..this was not an easy task at all..but they worked long tiring hours to make it happen to keep our love ones safe..and they also had to step up and be an extended family because I could not come in to see mom I visited her at her window..Kaitlyn the social worker was always in touch with me thru out my moms entire stay updating and always calling me back and updating me or answering my questions no matter how busy she was..she always got back to me..I was extremely impressed with her professionalism & compassion for her residents..she is wonderful..katherine the head nurse was also always available to take my call or call me right back on any questions or update me on any medical issues with mom..she is the best..so sweet and very compassionate..I also would like to mention the 3 girls up at the front always answering my calls and leaving moms laundry so I could pick up or dropping stuff off for mom...as crazy busy as they were they were always so sweet, and happy to talk and help out..Angela, Brittany & Nicole(sorry if I got this wrong I think its Nicole) :)xoxo..also a shot out to Daniel & Johnston & the girls that took sure great care of my mom and made her stay there a safe & happy one..I'm diffently going to miss everyone there and I know mom is also going to miss everyone ...I think what really stands out the most is the way everyone worked so wonderful together to get an extremely difficult job keeping our love ones safe during an extremely difficult time..you are all very special and greatly appreciated xoxo

My grandfather was a patient here on and off for a few months while battling terminal stomach cancer. I was always afraid and had worries about having to put him in a nursing facility for care. Before Care One we had him at a different facility closer to home, and we had a horrible experience, so clearly I was worried about having to put him someplace else. I heard wonderful things about this facility and I must say my fears were put at rest immediately. He received such amazing care each and every time he was here. The nurses and CNA's were always beyond attentive to his every need. Always going above and beyond. He was never ignored and we as his family were never ignored either. I have never met such a kind, caring and loving staff. Everyone there is truly passionate about their jobs and they all became like family to us. Everyone there treated my grandfather, and us as they would their own. Sadly my grandfather was put on hospice care during his final stay at Care One. I can't even put into words how they made such a painful, heartbreaking time in our lives as easy as it possibly could be. I spent most of the day time there in his final days but would receive phone calls on how he was doing at night when I had no choice but to be home with my children. My grandfather passed away at Care One and you could see how truly sad they all were at his passing. I truly believe they loved him as much as I did. As hard as it was to have to have him in a facility at the end of his life, I would not imagine having him be anywhere else. I am beyond thankful for everything everyone did for him and for us and I would recommend this facility to anyone who needs the best care for their loved ones.

Thank you to CareOne at Holmdel for getting me home and safe. I had COVID-19 and thought the worst. I was scared to go into a subacute rehab in times like this. The staff was very encouraging, motivating and made me feel safe and at home. The physical, occupational and speech therapy did an excellent job and I could not thank them enough. I went from using assistive devices and having decreased strength and balance, to returning back to my normal routine without using a walker I m even bowling! The facility was very accommodating, considering these unprecedented times we are all facing and I was able to communicate with my family often via FaceTime sessions, window visits, etc. Thank you to the staff at CareOne for helping me regain my independence!

The staff at Care One are there for the patients. As I had a total knee replacement I was in pain and definitely did not want to exercise. But lo and behold with their friendliness and knowledge they got me to the point of no extreme pain. I would definitely recommend this facility and staff to anyone who needs this type of care. The staff listens and responds to all questions.

My husband has been there for 3 weeks in the rehab center. The first night we had a little misunderstanding, but it was solved immediately. After that, everything is going great. The nurses care is very professional, the aids too. The doctor visits frequently and they cover all the patients needs. The physical therapist work is outstanding. He receives it 3 times a day, and his improvement is unreal. The administrative staff is very competent and do even the impossible to make the patient and families feel welcome. The place is well maintained and the food is healthy. So far, our experience has been excellent and I feel they well earned and deserve the 5 stars. I highly recommend this place.

Loved the place...the staff was very receptive to my needs and went above and beyond the call of duty.

We brought my mom here after a hospital stay none of us really happy about it .after 2 weeks I have to say everyone we have encountered has been extremely nice and so helpful making a stressful and sad time so much easier A special shout out to the kitchen staff who are always happy and helpful no matter what the mood of the patients Thanks Careone Holmdel for making a hard time easier

My son & I had visited Care One in Holmdel, N.J. prior to my husband being discharged from the hospital. We knew we were in the right place as soon as Liz, the Director of Nursing, greeted us in a warm & friendly manner. We truly appreciated that she took the time to bring us on a tour of the facility & answer all of our questions thoroughly. My son & I were also impressed w/the cleanliness & the overall positive environment of Care One.
We were extremely pleased & satisfied w/the rehabilitation services my husband received for approximately two weeks. All of the staff from the Nurses, the Social Worker/Marie, the Speech Therapist/Mary, the Physical Therapists/Elizabeth & Vitale, & the Occupational Therapist/Adrian went "above & beyond" to assist my husband in preparing him to be as independent & self-sufficient as possible. They did an outstanding job in correcting a very serious issue my husband had been suffering with for two months.
We are sincerely grateful to everyone at Care One for working as a team along w/my husband's PCP, Dr. V. Perkari, who visited regularly to oversee the care my husband was receiving.
We would highly recommend Care One in Holmdel to anyone! Many thanks to all of you & God bless!

I gave 5 Stars for the great and consistent help the physical and occupation therapists gave me on every single therapy session I had with them. If it wasn't for them I would still be struggling with my health problems.
The food and consideration in all facets of my stay here were completely above what I had expected. Also the fantastic nursing staff were all tremendous and contributed to getting me better.
