Thank you so much to Care One at Wall for taking care of both my parents during this difficult time! It was not easy dealing with the whole COVID situation for my family but we were able to stop by and see them outside their windows. CareOne has been caring for my grandmother (Mimi) for a recent fall. The rehab team has been doing an amazing job helping her get back on her feet and back to her old self!
I came to Care One in the beginning of the year close to death. The clinical team and therapy team worked diligently with me to help me get better. They did such an outstanding job with me I am doing more for myself each day. I am so great full to have met them and have them by my side while I continue to get stronger.
There are many wonderful experiences that I had at Care One, I met friends, and great nurses like the staff morning noon and night the aides especially Nate and Yvette at night. I couldn t ask for better care. When I had a need they were there, I don t think I waited more than two minutes for anything 24 hrs a day. They had a great physical therapy department especially Greg and Maria. They helped me so much and the kitchen gave me everything I asked for they gave me. I want to thank every single one of them they made me well. If you every want a rehab this is the one to go to. Thank you Shirley Green
Unresponsive staff and administration. Will never do deal with another Care One facility again for anyone in my family. Very responsive when they want your business . My mother was in facility and the staff didn't return calls for weeks at at a time and the staff would be nasty and disrespectful when questioned. Horribly run and the Corporate Office was no better! Never recommend to anyone!!!
Care One at Wall is my place of choice when I need physical therapy. The staff are kind, friendly, caring, knowledgeable and always there to help when I have a question. The food was great and Katie the nurse on Marina was always making me laugh. I recommend care one to anyone in need of care.
Received collection notice for my father -Stay 8-20-15. Contacted Meridian who stated the collection notice is for Care one. Services were requested for my father from Care One to Meridian. (x-ray). Did Care One bill fathers Medicare, and then not pay Meridian's bill? When I contacted the Business Manager at Care One -Her response" I was not here when that happened so I can't help you." She then said, I can chase down actual bill from Meridian for Care One, as she does not have the phone number to call. "I can not take responsibility for this that happened before me." Left Message for next level at facility.
Love Love Love! Toured the place and thought too good to be true. Got my mom in, and my expectations were met and then some. I was super nervous leaving my mom with dementia but the activities people helped engage her and everytime I came to visit she had a smile on her face.
Beautiful facility, very bright and welcoming. Therapy gym is large and every patient has their own individual therapist. I would recommend this facility to family and friends based on the positive patient outcomes I have seen first hand.
Uncaring, unprofessional, rude. They do not care about their patients. If they know you're coming they will do their best to care for the patient. They let their patients sit in pain, cold and shaking. They DO NOT CARE! When questioned they blow you off like you have no idea what you're talking about. STAY FAR AWAY! LOOKS ARE DECEIVING..
My mother was there for 2 months of rehab and has dementia. Level of communication with key staff was abysmal. The phone rang "one way . I was mis-led as far as her ability to perform ADL's and, as a result, am overwhelmed now that she is home. A valuable hearing aid was lost and their collective response was to blame it on the dementia patient when, in fact, they should have taken responsibility for the lack of supervision that allowed that to happen. Compensation denied. Be careful when the Medicare money runs out!
The best thing they have going is the physical therapy! If you need a rehab with PT, go here. But, you must be aware and able to keep track of other things. The nursing staff during the week is great but the weekend crew do not pay close attention to detail. So, you have to watch everything, and I mean everything. The food is just awful. My family brought me dinner and lunch every day.
Overall, I did do very well there. I got good rest and SUPER physical therapy. In one week I was pain free, medication free, and walking beautifully.
My grandmother was at the facility for rehab after a hip replacement. The facility was recommended to us from a family friend. We were very pleased with the friendly and attentive staff. They showed that they really cared for the residents and us during such a stressful time in our lives. I would recommend them to everyone who is in need.
I came to Care One at Wall for rehab. My experience here has been excellent they are dedicated, caring, and knowable. I would definitely recommend Care One at Wall to family and friends.
After a very lengthy hospital stay, a trustworthy surgeon recommended Care One at Wall for my mother s severe wounds and rehabilitation. Because of the severity of her wounds, I was nervous and skeptical as her life depended on good quality care. Josephine Celentano came to meet my mother at the hospital, assess her needs and reassure us that her needs would be met and the transition would be smooth.
Months later, my mother is still receiving compassionate great care from the CNAs, RNs, doctors, NPs, therapists and all of the friendly staff. Her wounds are healing beautifully and her muscles that were wasting away have been rejuvenated to the point of her being able to take steps using a walker and being able to maneuver around in a wheelchair. (Her therapists even had her join them on their lunch breaks!)
Of course there are bumps in the road to recovery, but that is not to anyone s fault. That s life. (We have had some unfortunate situations with other family members at other local rehabilitation facilities.)My mother feels safe at care one and she has developed relationships with the staff. We couldn t be more pleased and impressed with her constant care.

The team at Care One at Wall are remarkable. The progress my mother has made while rehabbing at their facility has been better-than-expected. I highly recommend Care One at Wall.
My time spent at Care One at Wall is again excellent. I want to commend dietary, social workers, activities, nursing, aids , and administrative staff. The nurses who I want to say thank you the most to for being professional, kind, compassionate and loved are Rebecca, Warren, Love and Destiny. The aid I would like to recognize is Renee and Nate. All the Care One staff is a pleasure but I wanted to give these few a shout out.Care One at Wall is the only rehab I will go to and would recommend them to everyone. Thank you you Care One.
The clinical team and therapy team have done such an excellent job. They have gone above and beyond with my stay here. I have made great strides since coming here and feel stronger everyday. I know when they send me home I will be safe because of the strength they have given me.
This place is a joke - They told me my 83 year old mother was covered under medicare and then I revieved a bill for $3500+ and was told it's not covered. They said well we informed her of the cost....my mother has dementia so it's like telling a 5 year old. The response from billing was "well I'm not a nurse so how do I know?" DO NOT GO THERE. THEY ARE CROOKS!
While i appreciate the response online saying I would be contacted, I heard from no one.............. still waiting to hear from someone!!!!
We sent my Mother to Care One after a hospital stay for renal failure. This was during the pandemic so visits were limited to one or two days a week for 15 minutes. And the slots were always booked weeks in advance. The transport company they used, Gem, would leave her sitting at dialysis for hours before being picked up. My mother received inadequate, dangerous treatment at Care One and very nearly died. We were told she had contracted a VRE infection which was the cause of her disorientation and fatigue. Dissatisfied with her progress and condition, we had them send her to Jersey Shore Medical Center. She was diagnosed with having had several mini-strokes as well as having poor blood gases with high CO2. There was no infection. After a 10 day stay at Jersey Shore, we sent her to Hackensack Meridian at Wall. After 10 days there she was back home good as new. Thank God we took her out of Care One they would have killed her!
All of this is completely separate from the neglect and poor treatment my Mother received while at Care One. She would call us because staff would ignore her and not take her to the bathroom. We would have to call over and over before someone would answer to help her. They then put my Mom in diapers. Not because she couldn't walk. Because there wasn't enough staff to help her.
We thought my Mom would be at Care One for 2-3 weeks max to get her legs stronger. At our first meeting they told us they felt she needed to be there for over 6 weeks. Right then and there we knew this facility was focused on sucking as much money as possible from Medicare and not my Mom's health. With no visitors, the goal in the pandemic should be to get people home to family ASAP. Not at Care One. If you are in the Jersey Shore area and care and need subacute rehab, skip Care One and send your loved one right down New Bedford to Hackensack Meridian at Wall. You just my save their life.
My uncle was fortune enough to be cared for by the very caring staff at care one in wall. I can not thank everyone who participated enough in his long path during his sickness. In particular, the very attentive and knowledgeable nurse practitioner, Augusto Cespedes, who always made sure to stay on top of his medical necessities. He always made sure that our family was notified of any changes that were needed to better care for my uncle immediately. I would highly recommend this facility for a loved one that is looking for rehabilitation services.

My mom had a massive stroke on April 12, 2019. After a long stint in ICU, she was admitted to Care One at Wall on May 8, 2019 and she arrived in a mostly coma state and unresponsive. She leaves Care One at Wall on November 6, 2019 and she's leaving awake, alert, speaking, and walking with assistance! I cannot say enough good about the therapists, nurses, and aids at Care One at Wall! My mom was well taken care of and loved by Greg and Amanda (PT and OT) Rebecca, Felicia, Abdul, Jessica, Jocelyn, and Nate (nurse, nurse, nurse, aid, aid, aid) Robin and Tracey (speech therapist) Alysa (respiratory therapist) and Lauren (nurses desk)! Our family appreciates every person who made our mom's stay easier and more comfortable! Thank you from the bottom of our hearts!
PT & OT were awesome, nothing bad to say. However, the gym was continually way too cold - 50-some degrees. Some days it was so cold that they did therapy in a gathering room or in the patients' rooms.
One night the power went out and an aid strung an extension cord from the outlet powered by the emergency generator across the floor in my room on the way to the bathroom and covered it with a sheet. It didn't really matter, however, because 15 minutes later the generator failed as well.
The worst part of the facility was the food -- not just a complaint usually offered up on health care food. This was atrocious. There was no point in selecting things off the menu submitted days in advance; they came with random food including items from my NEVER EAT IT list. One night I got a tray that had previously been served to another patient. I saw one of the servers open crackers for a patient by biting it in her own teeth first. The head of the kitchen tried very hard to be accommodating but improvements never came.
3 times while I was there (about 5 weeks) there was a quarantine due to an GI infection running rampant through the facility.
Finally, my bathroom was incredibly unclean. Housekeeping ran a mop around the center of the floor, lifting nothing or going to the edges. Thinking maybe it was just permanently stained, one day I tried washing a spot with the washcloth I'd finished using in my own wash-up, and voila it came clean.
In summary, the therapy was great -- better to have it as an outpatient though.
Bad service they treat the older people bad and employees are very rude my grandpa was there and he would tell me so many stories don t let them fool you at all I would not recommend no nursing home is safe keep your love ones at home
The staff at Care One at Wall is excellent with extremely dedicated professionals. The facility is clean, well organized, and the environment is welcoming and cheerful. Thank you to all the staff who cared for my dad. It was a hard time for my me and my family, but I couldn't have asked for better care at all levels!
Care-one Wall review I have nothing but praise for Care-one at Wall. In April 2020, when I was diagnosed with Covid, I really didn t realize just how sick I actually was. After trying to fight it at home, I checked into Care-one for their care. I thought I d go in for a few days but I was there for 17 days. The first few days are a memory blur for me, I was just so exhausted and feverish. But for the last 2 weeks I have to say the staff were very caring, professional and cheerful. The rehab girls were fantastic, encouraging me while at the same time being aware of my limits. Nurses came in to check up on me in between the times they needed to come in to give meds, etc. They, along with the doctor assigned to me, tried a couple different antibiotics to help me and finally settled on a mix of two, one being the Hydroxychloroquine type of pill. I really attribute those meds and Care-one for saving my life. I can only imagine what the end result may have been if I had stayed home and tried to deal with the virus myself. So, 5 plus stars from me.
I am new with Care One and find the team here to be amazing. Everyone always comes in with a smile. They made me feel welcomed from day one and they show the residents with the same respect. I am so thankful to be apart of this amazing team.
Thank you for the wonderful way that I was treated here. Warren, Nathan, Caitlin, Anne, Manny and Destiny have made my stay enjoyable, educational and brought me back to a status of health. For me to say that I wanted to stay longer was an indication of the wonderful team at CareOne at Wall is.
Care One at Wall is my number one choice in healthcare. The staff make me feel like family and the food is delicious. Everyone I meet is kind and courteous. When I have a question they have the answer's. I would recommend Care One at Wall to all my friends and family.
Terrible human beings, treated my family like garbage, never in a good mood, i delivered to them and they made me wait outside in the cold for 20 minutes only to be greeted by two mean women and didnt even give me a tip AND didnt even meet the require amount for the food. My grandpa stayed there then transferred out because of how horrible it is. Definitely DO NOT send any of your loved ones here. Utterly DISGUSTING.
After searching and researching communities for my brother who is in his late 40 s with unique needs, we decided on CareOne in Wall. We are very grateful for his nursing staff, especially the unit nurse, Rebecca, who has gone above and beyond to understand his special circumstance and make revisions to his care plan. The activities and physical therapy departments have been great, doing everything they can during these difficult times with so many restrictions. My brother looks forward to the activities and doing trivia with John. A big Thank You to the entire staff for their patience and understanding.
Seems like a nice place. Except for the Xanax/adderalled out, clepto' receptionist Elizabeth. Oh and be careful who you let do your payroll, "never know an extra 0 or 2 might get thrown in there 'accidentally'." But yea just a grimey person, trust me I'd know, she just stole a bunch of stuff from me, even meds that I NEED every day
The nursing staff is amazing, so compassionate and accommodating not only to the patient but also with the family members. The nursing staff is very knowledgeable and are willing to go above and beyond to provide exceptional patient care. I would recommend this facility 100%
My stay here at Care One has been well beyond my expectations because of the dedication of the staff and I have to single out a few that contribute, but first I must tell you that I have had some of best meals ever, never expecting such in a rehab facility, I have been in the food related industry over 50 years in all phases of the business, and I have to tell Care One that if they don t already know they have a top notch high end restaurant chef. I mention a few menu items that to me were beyond word,foremost are his soups, an important part of a menu, they are some of the best I have ever had, lentil, cabbage,,each have numerous nutrients and the taste is phenomenal they have not come from a can , they are built from scratch giving you the nutrients they have ,I never bothered trying Salisbury steak, assuming it is another burger I ordered it and it was fantastic which was home I will try to copy his, i am waiting for the Russian Chicken to make the menu again because trying to figure out how it was wade is keeping me awake , I honestly can give high praise to every meal I have here, I Thank the Chef for easing my stay. I must mention a few other people who have made my stay here better. My aid Renee - note to Care One-Clone her.She absolutely is a very dedicated caregiver that starts your day on a high note, she will be sure that you have everything you asked for or need never questioning your request. I would hire her in a heartbeat, a pro- active employee is a employers hope and care One you have one.. I realize this is rather lengthy but I find it hard not to be a generic commentator and point to what I felt should be said
My father was recently admitted to Jersey Shore medical center and couldn't come straight home after the hospital. I was told he needed to get stronger at a sub acute rehab. After looking through options online I choice a few facilities in Wall NJ where my dad lives. He never stayed in a rehab so the process seemed a bit scary to me to make the choice for him. That same day Maryellen from CareOne in Wall called me to help guide me through the process. I had a million questions and she was able to answer everything I asked. I didn't receive a call from anyother rehab at all. The very next day I received another call from Ashley with CareOne at Wall just to see how my dad was doing and to make sure I was ok and to see if we needed anything. She noticed my dad was being discharged that very day and wanted to let me know. I was and still am blown away at how accommodating and on top of it they were. My dad admitted to CareOne that day, spend 2 weeks getting therapy which was amazing. He loved the food especially the soup they served daily. The nurses treated him like family. He especially remembers Ben who helped him every single day. I give this place 5 +++++ stars. I will never send my family anywhere else but CareOne.
Due to Covid restrictions I was not able to go inside to see the facility. My brother who is handicapped and can not speak, could not report to me by phone. Getting to speak to his nurse was often very difficult and even leaving a message for a call back was rarely returned. Theo Jackson was wonderful. Whenever I called he would go to my brother"s room, check on him, and get right back to me. Patricia Abramson

My mother had suffered three health events within one months time and was unable to get the proper physical and occupational care at her previous rehab center. I had both my mother and father in Care-One @ wall at the same time. They have been married for 44 years, so when Care One was able to accommodate them both in one room was a game changer for us. The nurses and aids treated them like family and made sure their stay was comfortable. The physical therapist were gentle but also motivated my parents to work hard to gain strength and confidence in order to succeed at home. I highly recommend this facility to any family looking for A+ care in a beautiful building.
I recently visited care one at wall for a community event. I was very impressed that the head of departments were there to greet the community. They were available to answer questions related to the facility. Unfortunately the possibility of a family member needing the services offered here may be necessary in the future, so having a staff that was approachable was impressive. I also was impressed with the nursing staff. During a lovely, but chaotic breakfast with Santa I witnessed the nursing staff making sure residents had their medication on time. The nutritional staff was very attentive to the community and the residents in attendance. The marketing staff did an excellent job. Thank you for for showing your professionalism.

i was looking for a long term facility for mom some one told me about CareOne in wall n.j i did not know they had long term care on secondfloor at this facility i walked in with out an appointment and was given a tour .It was very nice and clean the staff was really nice from admissions , nursing, rehab, aids, kitchen staff, cleaning.this was on a friday and had my mom in there by monday afternoon and things are really going great . so thank very much to all the staff at CareOne
PT was awesome, they really pushed me to get to where I wanted to be. Food was decent, and if it wasn't, they made me what I wanted. Kudos to the kitchen staff

He has gone above and beyond for my Grandmother during her stay and I cannot imagine how we would have gotten through this challenging time without him. My grandfather in law came here for rehab after an admission to the hospital and he had a great recovery. He loved his nursing staff, rehab staff, and the food he could not stop raving about, as the whole family enjoyed joining him for dinner and lunch. I am coming from a family members perspective, knowing the ins and outs, and knowing that they are cared for when you are not present is comforting. I trust that any of my family or friends will receive the best level of care here. Since then, I have had friends of family rehab here with great experiences as well.

Working in the healthcare industry is hard. Working in the healthcare industry during a global health pandemic is harder. I am grateful everyday that I have the honor and privilege to navigate this industry with the support and guidance of my co-workers and leadership team at Care One. For over two years I have worked for this amazing company. Each day I get to come in and work with patients and their families as they navigate the rehabilitation/post-acute world. Working in tandem with our community partners I get to ensure a seamless transition for each patient from one level of care to the next. I connect with the families as their advocate for the length of their loved ones' stay with us. Our team is a family and I am so proud to be a part of it. Some days we face challenges that no one could have predicted but we always overcome those challenges and rise above. Together. As a team. As a family. No one is perfect but we are one. CareOne has been a godsend. Mary Ellen and Ashley from administration are phenomenal at their job, compassionate and helpful, they go above and beyond to make the transition smooth and stress free. Eden is a social worker who individualizes the care plan to each patient and is extremely helpful and empathetic. I am so happy that this facility has made everything so easy for me and my mom while giving me peace of mind that my mother will be in great hands.
I treat patients at Care One at Wall and sometimes offer in service education to the staff. They have a very competent, compassionate and caring staff. Their administrator is dedicated to making sure their patients get the absolute best care. They go above and beyond medically and have strict Covid protocols that ensure safety. Great job Care One!
