I can't understand the rave reviews. This facility is under new management and very run down from the time my uncle was here last, he only stayed one night and had to get out of there. He had actually been looking forward to returning there. Very thin mattress, grimy floors, poor food...We were so disappointed and sad.

I was there last year for rehab n the staff was excellent it was clean n safe everyone is friendly wound care was awesome my husband n I really liked this place n we like how we were treated n we like how friendly the staff was they even brought in an extra hospital bed n stuck it in my room next to my bed and they let her stay the whole weekend and they helped shower her the staff would take her in the lobby n let her do arts n crafts n stuff I would highly recommend this place to anyone and everyone that needs to be in a nursing home long term care or to just be there for rehab short term care LOVE THIS PLACE

Over the past decade I have been in multiple nursing homes & I would rank Avalon West as one of the best facilities I have ever been to. Not only do they create a great environment for optimal recovery but they provide a personalized level of care as if each patient was their own loved one. Thank you Avalon West for going above & beyond to make a difference in all the lives you touch.

Man oh man I feel awful even giving one star. I have been here to visit my abuelita every week. Each week it smells of urine and there is these poor folks a lot who i hear asking for help and no help is given. My Abuelita is in the memory care unit so the door is locked on inside and out. The nurses at the desk don t help you in half the time and good luck getting out my sister and I tried to get out and we knocked on the doors and made eye contact with three different nurses. Nothing. While we were trapped we watched as two nurses performed a urine lab sample ON THE RECEPTION DESK. THAT IS UNSANITARY and would make the sample contaminated because you know EVERYONE TOUCHES THE RECEPTION DESK. That should be done in a lab! I wrote down all the details and plan on calling the health department tomorrow. In the room my abuelita is staying I made a mess and asked for a broom the nurse said its ok . uh no it is not let me clean it up. My abuelita got placed on hospice today. I have seen the hospice nurse once. She got realized from the hospital at 5 pm and sent back to Avalon. It was 8 pm and she had not received her prescriptions because of some reason like they hadn t been delivered or I dunno. She is on morphine which means she is in a lot of PAIN. She was yelling (when every she was conscious) no one came to check on her. Why is she in pain? She acquired two bed sores on her leg that were discovered by US. We bathed her Sunday and a patch fell off her leg, we discovered a white puss filled leg. She went to the ER Tuesday and she is now on hospice her body can t fight the infection and she is too old for antibiotics. (97) after ten there is one nurse for the whole memory care unit. who isn t a hospice nurse i might add. So many negative things it breaks my heart.

I was there for the last 5 weeks and I think it's a good place, somewhat ran down in ways but nice. Within days I had met all the staff in Management and they addressed every concern. I was very reluctant to go but happy I did. Great Therapy staff and they rotate to cover and help on it. Food is smaller portions and a few have poor communication skills. Never missed meds or appointments. Asking the staff they like it there and most have been there a long time. That tells me they are treated well. Yes at times there is a smell but not for very long. I would recommend this place over many others I have visited people at.

During our walk through Avalon it is clean and the food looked good. We brought Mom to Avalon to rehab after a broken hip. We soon found that Mom was not being cared for properly, and, because of this, my brother and I found it necessary to spend a lot of time there in order to insure that Mom was fed and being cared for. I found Mom sitting in an uncomfortable wheelchair for over five hours (with a broken hip!) with her bent over and almost falling out of the chair. I told the nurses that this was unacceptable and that I never wanted this to happen again. I was promised that it would not happen again; three days later the same thing happened. Twice staff "forgot" to bring her meals to her! We were there and asked FOUR times for her tray and when it finally arrived at 7:20 (normal time is 5:15) we were told it had been sitting on a table in the dinning room -- it was ice cold! Mom could not remember when she ate so she would not have used the call button to ask for her meal. Mom was moved to a room closer to the nurses desk in order that they could "keep an eye on her." What a joke. The nurses rarely checked on her! The physical therapy was very good; the occupational therapy was not nessary and was obviously done in order to bill the insurance. The CNA's were pleasant; most of the nurses not so much. We complained to the head nurse AND the Administrator. The Administrator tried to make things better but if the staff is not willing to follow through with good care, whatever the Administrator says doesn't mean much. Mom had only been at Avalon for three days and all of her clothes disappeared NEVER to appear again. I was asked time and time again to bring more clothes but every time I did Avalon lost them! I never did get them back. Unbelievable! I brought Mom home to my house and even though Avalon personnel told me that it would be too hard for me to care for Mom, it was much easier for me to have her in my home than for me to be at Avalon 8 hours a day and coming home stressed because at my home I just did whatever needed to be done and I didn't have to continually ask for things to be done like I did at Avalon. I DO NOT RECOMMEND AVALON unless you can and are willing to spend 8 hours a day there.

This place is absolutely terrible and if you take away anything from this review let it be: PLEASE do not take your loved one to this place. PLEASE. They do not care for the patients properly and are extremely neglectful. Our grandma was in Avalon and she had Alzheimer's. The staff seemed lazy, uncaring and would only do the bare minimum. At first, I thought it was cause they were busy but I would go out in the hall and they all would be sitting around the front desk chatting or snacking. One of the cleaning crew even told my mom that theft is regular there since the staff know people there are dying so they take their stuff. And unfortunately, my grandma's wedding ring was stolen. We have photo evidence she was wearing it while she was at Avalon but at the time we left (because she passed away) it was nowhere to be found and when we asked where it was, they had the audacity to blame it on our family and said one of our family members stole it. This place is atrocious. Don't take your loved ones here.

Do not send a loved one to this facility unless you want to be treated like garbage and needing to request any care given. Wish I could give 0 stars. I am a healthcare provider and this place will not receive any of my patients.

Absolutely love my experience with Avalon West. Very friendly staff who are willing to help out and address problems!! The facility is clean and you can tell staff really care about the residents.

TLDR:

TLDR:
Pro: Jeff (the nurse) is awesome
Cons: food is awful, poor help from staff, they never billed insurance for part of his stay, tried to make us pay for his stay and said insurance denied it.

Pro:
The only pro is a couple of the nurses, such as Jeff. He was fantastic and my dad absolutely adored and appreciated him. He was incredibly kind and helpful. He got to know my dad as a person and would help him very quickly whenever he worked.

Cons:
The worst there is the administration. They didn't get the prior authorizations required for prescriptions, refilled many of his prescriptions during his stay too soon, and I had to call and email multiple times to update the insurance information they had because they kept trying to bill an old insurance. Due to them not getting the prior authorizations and refilling too soon, it cost my dad thousands (for prescriptions alone) as he then had to pay out of pocket for them. They also sent any lab work to an out of network lab which would've cost him more, but I fought with them until Avalon West would pay the bill for those labs.

Tim Needles kept telling me my dad had to privately pay for about half of his stay because insurance wouldn't cover it. Each time I reached out to his insurance, they said they were waiting on more information from Avalon West. After I sent many emails to Tim and made multiple phone calls to Tim, he finally connected me to their biller (ShayLynne). She was nice and said she would help me appeal the decision. She ended up leaving and a few months later, I received a letter saying his bill was being sent to collections. I reached out to Tim again and he finally replied, connected me with Ashley Sand, and said it would if he didn't pay the second half of his stay (over $20K). I told them about the conversation with ShayLynne and was told they couldn't appeal his actual stay. When I asked Ashley why it couldn't be appealed, I didn't hear back. I reached out to his insurance again and found out that they were only billed for his therapy for the second half, not his actual stay. Again, they never even attempted to bill his insurance. I emailed Ashley and Tim and let them know about the conversation with his insurance, which I have in writing, and they never replied. I've tried calling and each time I call Tim and Ashley have happened to just left for the day, regardless of the time I call.

Finally, the food is disgusting. There's quite a long delay (20 minutes or more) from the time the button is pushed to when someone enters and if they don't want to or can't help (they'll get someone else), it's even longer. It smells like urine in the halls.

The staffing here at Avalon West has been nothing but incredible! The doctors that have come in to see my father have also been very great and are always very nice to my father. The therapy people here are amazing, my dad loves them all, and always work great with him. They all have taken very good care of my father. He has been here four times in the past. Avalon West will always be my number one chose over all facilities.

My mother has been here two weeks. The staff are very helpful, friendly and kind to her. I have been by her side so I see it all. No complaints go unaddressed. Things change quickly and needs are met if you have a concern. I would recommend this clean, beautiful facility.

I am a physician at Avalon West. I have worked here as a consultant for over 20 years. I have seen many changes including numerous administrators and directors of nursing. For the last 10 years, I have seen continual improvement. The administrator and DON are available to speak with residents and families at almost any time. I have worked with them at other buildings and have seen nothing but caring for the needs of others. When staff make mistakes which affect residents, those problems are dealt with swiftly, and there are no favorites. Over the years I have worked at close to 50 different facilities and Avalon buildings are my favorites. Avalon West is a great building, and I would live there if I had to choose a place.

I wouldn't even give them 1 star! Sad that they allowed my Mother to spiral downhill until she had to be taken back to emergency and then into ICU, in which she didn't survive. Do you think one person has called to check on her and to ask if she is alright- little on dead? Be careful they are not on the side of getting them back to health, if a incoherent person says I don't want food, I don't want my medicine they don't do a thing to help. Very sad that they did this, sad they thought it was alright to let someone NOT eat for 2 weeks before my sister demanded them to do something.

Worst place for loved ones. They leave them there to fend for themselves. They ignore there guest and lie to your face about items they swear they have taken care of. My father (god bless his soul) was placed here under a very concerning serious medical issue. They left him in his room without checking on him. He was unable to get up on his own and soiled himself. He was left there in his own urine for hours! How embarrising and terrible to treat someone who is there for your help so poorly. I will always regret placing him here for rehabilitation.

My heart hurts with the disappointment my family experienced at this facility. My father was admitted here after back surgery. He was bed ridden, and they would take their sweet time coming into answer a call. One morning we received a phone call from him being in distress I rushed down. After the PT person had come in they had not made sure that his call button or the device that would help him move the bed up and down where with him so he could use them. In fact when I arrived I had found they where on the floor. He was half way off the bed trying to reach for the call button on the floor. He had been calling out for help with the door open, and no tv on and no one had gone to help him. I heard him as I rushed in. He was hanging off the bed. I helped him in got him settled and called for the help. No one came in for 10 minutes. I headed to the desk to ask for some assistance I was rudely told they would leave a note for nurse. When the nurse came in and I told him the problem he seemed confused at what his job even was. He said he was just there to make sure he was ok. How was that not making sure he was ok? On top of that twice they put a fitted sheet over the top of him. Do they not have a laundry service or are they not trained on how to make a bed? The head of nursing walked right by my mom pretending she didn t see her as she stood outside her office to speak to her. When addressing situations they asked for the name of the person doing it but staff does not where name tags or they turn them around so you can t read who they are. There was more than one situation where an aid was called to use the bathroom and was needed to be told he could not get up. There was a time when they did not flush his feces down the toilet and had to be addressed twice before they did their job. An aid was doing flips in a room by my Dad and a nurse had to tell him to stop. One night when looking for an aid we found them styling each other s hair in the hall way. Also when my father left there by ambulance due to the fact his arm was swelling and they thought he may have sepsis it was NOT the nurse that had brought attention and called the doctor, but the speech therapist that had brought attention to the nurse about it. While I understand these facilities are under staffed some of the things that went on are inexcusable. Luckily we have a family that lives close and where able to visit twice a day. I feel so bad for these people who have no family and the heart breaking things I had observed here. If your loved one is bed ridden or cannot speak up for themselves I strongly urge you to look else where.

Wasn t served a meal for two days after admission on a Friday. No bedding changes, or checks for water. No ice in cooler post joint surgery. No shower when asked after 2 days. When asking for help the staff seem upset and information is not passed on and there is quite a neglect of care. Would not want to recommend this place. The staff seem very uncaring for the most part.

My grand father was recently admitted during the pandemic for a short stay and we could not have been happier! The entire staff was incredibly professional and made us all feel at ease. Services provided such as physical therapy were exceptional!
Pandemic restrictions aside, they did everything possible to make his stay comfortable including making accommodations for our family to stay in touch. Would highly recommend to anyone looking for peace of mind and a welcoming environment.

Lazy staff. I'm an RN and had to fight every visit to get my Dad the care he needed. He had 4 teeth and I could never get them to provide oral care. One good thing he never got a bed sore but I think that is because he could turn himself. Horrible food. I would think of trying another place before this one. Unfortunately most places for our elderly are understaffed and their aids are under trained. The RNs are all basically lazy.

I have to put one star to get it to post but it should have ZERO stars -- this was my husband's experience this past weekend:



They obviously didn't bother to ask/read about my health care needs or goals, or the purpose of my visit. Prior to the realization I was not going to receive proper care and informing them I was leaving, the nurse admitted she hadn t even read my discharge orders yet. At this point I had been there over 9 hours!

The facility looked run-down and reeked of urine and feces, forcing me to keep my room door closed.

They didn't weigh me when I arrived. My wife had to ask when they were going to do this. We eventually learned that they didn't know if they had the ability to weigh me as they had no idea as to the weight limit of their scale. This was crucial in my care to ascertain whether there had been a reduction in water retention on a daily basis.

Nobody checked the urinary catheter; in fact, they didn't even know that I had one for over 7 hours. It took me having to struggle to make my way out into the hall and down to the nurse s station for my medications for them to notice my catheter. So, of course no one came to clean it, nor did they empty it until then. Nobody logged the input and output of fluids.

Their blood pressure device was erratic and non-functional. They were reluctant to use a manual method, so they tinkered with it until it finally yielded a result that looked "normal". And, no sign of an oximeter to test my oxygen levels on 24/7 oxygen.

Dinner was extremely carb heavy, and there was obviously no attempt to provide adequate diabetic/cardiac compliant meals.

I only received about half of my hospital discharge ordered meds. As for my insulin needs, I only received the Lantus and 4 units of Humalog - whereas the simplified discharge orders clearly stated to administer 30 units per meal and 6 additional units per my 247 mg/dL blood sugar level (subsequently, my at home blood sugar level tested at 366 mg/dL). When I attempted to discuss the discrepancy with the "nurse", I was met with complete obstinance. She claimed the hospital had called and changed the amount. My wife called and spoke directly to the charge nurse at the hospital who said absolutely no changes had been made to my discharge orders once I left the hospital. I was given only half of the prescribed Metformin. The nurse only brought in half of my dose and I questioned the amount -- she came back about half an hour later with the other half of the medication.. The nurse acted put-out when asked to identify pills I didn't recognize.

It became painfully apparent that nobody there had read my discharge orders, or they had completely ignored them. They were ill-equipped, incompetent, and incapable of providing the medical care I required.

Now someone has called to check on me and to see if I would like to receive home health care. Of course this came with a threat that it would be difficult to get a nursing service to accept me as a patient, as they said I had left against medical advice ! The administrator hadn t even had me sign the admitting paperwork, as she was in a hurry to leave and said we would do the paperwork the following day. How could I have left AMA as they had no clue as to what my medical needs even were? I left due to IMC, "Incompetent Medical Care".

Okay, overall I gave them a 5 star rating. The good; they were prompt, helpful and eager to help my dad. The staff is mostly friendly, (I had to remind the receptionist to smile, it couldn't be that bad of a day). The rooms are a tad small, but the entire facility has been updated in the past 3-4 years. The fair; I think some of the staff is a bit apathetic to the residents there. I also think the facility may be a little understaffed, specifically the non nursing staff. It seemed the office staff was a little light. That being said it's a pretty nice place. My dad said the food was great.
