My grandmother was at Achieve for several weeks and it was an absolute nightmare. She has dementia and was doing physical therapy for a fractured pelvis. She was often left unattended and fell down 3 separate times. Nurses lied about her food consumption and how often she used the bathroom. Very unattentive and rude staff. We had her moved to another facility.
