FRAUD!

FRAUD!
I contacted this facility by phone to inquire after their outpatient programs. I provided my insurance for them to verify my coverage. They notified me that they were out-of-network, so I declined to book an appointment. I then received an EOB from my insurance company. Dr. Poliquin sent TWO claims for $1200 for service 2 months prior to my ever having even called them. I was never seen or treated in any way by Dr. Poliquin or anyone at this facility. Beware of insurance fraud!
