Be very careful to monitor the medication orders issued by the attending physicians upon discharge. Ever time that my mother has been in this hospital, upon discharge her regular medications have been changed without my knowledge. The last incident was just last week. They discontinued her lasix (water pill) and potassium and discharged her to a nursing home. Less that 24 hours later 911 had to be called to take her to another emergency room. He potassium level was critically low, and she had fluid building up in her chest cavity.
