Astiva Health is in violation of several laws, and constantly deceives customers.

Astiva Health displayed their EOB details on the internet for 2026 already in 2025, attracting customers who believed what they would display on the internet would guarantee truthfulness. WRONG. Nothing they stated on their EOB for 2026 was truthful. Astiva Health's DECEPTION PRACTICES turned out to be well-established:

I had no idea about this all when I signed up. January 2026, the truth revealed itself. First, Astiva Health denied me pen needles for my insulin injections. Astiva said to my pharmacist: "Try other steps instead of needles. If she insists on needles, she must get a Pre-Authorization". My pharmacist was perplexed. Astiva KNEW that I am diabetic and injecting insulin. There is NO other way to administer insulin other than with pen needles. Astiva is so unprofessional, so greedy, so deceptive, that they refused to allow me even this simple health care service.
After several complaints, calls, and actions, they finally approved the pen needles.
Next: I am on Humalog. They said there is a cheaper insulin: Novalog. Now they are switching me to Novalog, so they can increase their profit

Here comes the worst:
I had an eye doctor appointment on Jan. 21, 2026. Astiva's EOB states that we can have 36 one way trips. As soon as you want to use this service, they sabotage it, by not only giving the driver intentionally the wrong address, but also delaying the driver's arrival, so your appointment would need rescheduling due to intentional transportation failure. Because of my persistence, I was able to force Astiva to transport me to the appointment. On Jan. 26, 2026, they staged another sabotage by telling me "there is no driver" -- again for a trip to the eye doctor. I had to drive myself with 1 seeing eye, as my R eye is dysfunctional.
Quyen Huynh from Astiva's Quality Management Dept. stated in writing to me: TRANSPORTATION IS NOT A GUARANTEED SERVICE. SHE LIED. IT IS, BY LAW. This is non-emergency medical transportation, and these kinds of intentional failures, staged by a health insurance company, are pure sabotages, to save Astiva money. My complaint to the LICENSING DEPT. OF TRANSPORTATION IS UNDERWAY, in addition to several complaints to other State Agencies.
Astiva does not stop with intentional transportation sabotages. As the CMS exposes publicly, they have been abusive and negligent to thousands of their members by blocking access to medication, due to severe greed. Many insulin dependent patients faced the threat of death as Astiva did not allow them to get their diabetes medication. CMS / Medicare intervened and punished Astiva, threatening with revocation of their license.
Astiva evidently continues their deceptive practices, in an effort to maximize their profit at the expense of patients' lives. They do NOT deserve a health insurance license.

Lastly, after sabotaging 2 of my transportation entitlements, they did the unthinkable:
On my return trip today, I found my L front tire slashed to the degree that it didn't make it beyond a few miles, needing emergency tire replacement. Nobody knew about my appointment other than Astiva, and they knew where my car was parked. The slash was meant to cause an accident, possibly a fatal one. Would Astiva go this far ? YES, absolutely YES. Go to the URL I gave above and read how deceitful they are, and how they violate the law, for profit. They are very well capable of slashing tires, to further their sabotages. This matter is under investigation now. DO NOT ENROLL IN ASTIVA.

Lies, Deception, Denials, Substandard "care"

They hired professional liers during Enrollment, to tell us all sorts of lies. On Jan. 1, 2025, all of their lies surfaced. Transportation, Dental Care, etc....multiple lies. In addition, they are contracted with criminals posing as doctors, dentists. Together with these criminals, they exploit patients to the max, to funnel money from CMS into their own pockets. Fraudsters, thieves, liers. One of their tricks: They approve a specialty consult, then sabotage it by not providing you with transportation. I am supposed to see a CFIDS specialist in Loma Linda, which Alignment approved, but is sabotaging by blocking transportation to the appointment. Dentistry is a total DISASTER. One of their dentists manipulates xrays to defraud patients, and Alignment is fully aware of it, and does NOTHING about it. With Liberty Dental, you will lose ALL of your teeth. That is what Alignment wants. Then they give you ill-fitting "plates", meaning you are completely teethless. Their dentists are so unqualified that every dental "work" damages your mouth futher and further.
Grievances: Don't bother to file. All they do is do NOTHING. Grievances land in the trash bin. With Alignment, one is completely exploited, victimized, and left with no healt care. I am in the process of filing a major FRAUD complaint with the Department Of Insurance, and the US Dept. Of Justice.
