Management listens to 1 or 2 senior (s) (not experts in the respective domain) and ignores the expert's advice. In many cases, they end up with problems and technical debt. They treat new designs like school projects.
Zero documentation, no lesson learned, no design description, no test results, etc IMAGINE! This is not serious.
