I don't have time to even write the review that th...

I don't have time to even write the review that this place deserves..... First off... Most importantly. During covid 19 it would be nice to even be able to get anybody on the phone to find ANYTHING out about your loved ones. When your not able to go visit....not able to get updates...most of the time the nurses you speak to don't have any info or answers for you. The couple times I was actually able to get a nurse on the phone I couldn't get any information out of them. One lady finally answered laughing loudly all in the phone. So frustrating.... After I argued with one lady who told me she would make sure I got updates every few days....I DIDN'T GET 1 phone call. Then my father was sent to the emergency room and was there for almost 3 days before the emergency room finally dug around to find out that he actually did have a family. Completely unacceptable!!!! Do NOT let your loved ones go to this place.

Don't put your love ones in this place. Just left there a resident was hollering please give me a blanket I'm cold. We went in her room she had nothing on no sheet or blanket the air was in on her room it was cold. Don't put your love ones in this place. We are in the process of moving ours.

Management there is very unprofessional short staf...

Management there is very unprofessional short staffed under paid work house I would not recommend anyone put their loved ones in that facility bad experience this place has bed bugs the residents suffer the most management collects the funds that's what's important to them. They only make right when the state is in the building and other days the place is a joke this is a place in the building with bed bugs

Awful care; the dementia area care is poor. They let their patients lay sick for days, a week until family complains enough to get nurse involved and then takes 2 days for doctor to send to hospital and all suffering from dehydration!! No UTI NO Flu, pure lack of NEGLECT!!
Aides, any staff could care less. State needs to come in a reevaluate the whole place. Very sad... Don't deserve even 1 star

I wish they would shut this place down, the staff is rude when you can find them. No one never answer the phone after hours they just allow you to walk the halls, no questions ask, the staff don't even speak and the patients look helpless, the place isn't that clean and the food sucks my mom loss 20 lbs in 7 days hurried up and moved her out of there.

I am an employee at Hyde Park Health Center and this is by far the best place that I have worked in 30 years. The residents are awesome and the staff is very friendly and helpful. I love coming to work everyday and look forward to helping our residents anyway I possibly can. Please stop in for a visit and see for yourself how awesome Hyde Park Health Center has become.

This review is coming from someone who has worked ...

This review is coming from someone who has worked in two different nursing homes and medical settings...please do not leave your loved one in this facility. It's seems poorly ran and lacking communication. We encountered 2 nurses and one nurse aide. The aide was friendly and helpful. The nurse on the floor acted as if we were bothering her by asking for information reguarding our loved one. The nurse supervisor was condescending when speaking to us about the care plan for our loved one. When she realized she couldn't pull a fast one on us, she became agitated and aggressive with her words. Seems to me scare tactics are used in this facility by staff members to manipulate the situation. I don't think the nurse expected the response she got from us. The facility is deplorable and needs attention. The rooms are small, dirty(dust on window sill in rooms and well as on curtain panels) halls smell of urine, rust around sink in room, panel coming off near sink, light above sink did not work, stains on bed cover(this cover is used as the main cover, not just as bedspread), there was a recliner in the room and who knows where it came from or if it had been cleaned and if your loved one is admitted on the weekend there doesn't seem to be sufficient supervision of staff to set up incoming resident. A room phone was not given to resident until the next morning after being there from 5pm the day prior. They were of sound mind and alertness so a phone was not given because of a medical condition. One was asked for but always forgotten. There was also a food menu of options that our loved one couldn't have but was in the room on the bedside table. The nurse said she didn't know much about our loved one and would print us off what she had...this is where it all went south. Seriously save yourselves the aggravation and nightmare and get pre-cert recommendations for other facilities.

I have a family member in this facility and I can't express enough how great her experience and my families experience has been. The nurses, her caregivers and even the management has been extremely friendly, helpful and attentive to her every need. The facility is always very clean and the food is as good as it gets. Please keep up the great work and doing what you all do for families.

I am grateful for the experience my mom had in this facility. When I went to check it out, the front staff was very welcoming and kind. Not to mention comforting during one of the worst times of my life. My mother was in the ICU for a month and all of sudden I had to find a place to transfer her to for rehab. This is a scary decision for anyone. I am happy I made the choice to send her here. My dad felt very welcomed. My mom ate well everyday. Her therapists were awesome! And she made huge progress while she was there. In fact, she is walking better now than she has in years due to her therapy and after care they ordered. The staff work hard and are not shown the appreciation they deserve. Thank you Hyde Park Health Center from the Parkers!!!!!

Dishonest business practices. This company will fl...

Dishonest business practices. This company will flat out lie to make you liable for a bill that should be paid through Medicaid. Still waiting for confirmation that they did what they said, but they lied and cannot show proof of submission. Also patient care is questionable, as the aides are incompetent and the nurses cover for them. They will retaliate if you speak up about their practices, so instead remove your loved ones from this place. My only hope is that when these employees need to be placed in a nursing home their karma will come back to them tenfold. Shameful.

My mom lives here and I love their food. Meatball subs, chef salads, different soups and taco salads that mom talks about all of the time. If I have a question or need something, the nurse always helps us. The soda shop lady is super nice too :)

My grandmother has been there for about 3 months and we've always felt like a member of the family by the staff, the food has been way better then you'd expect for a nursing home and the location is great and in a nice area of the city.

This place killed my 56 year old father. You can ...

This place killed my 56 year old father. You can search the Department of Health records for complaints. They've had everything from Fire Safety violations, silent fire alarm drills at 6:30am, to patients not getting their medicines to abuse not being reported by the Admin staff, to unwarranted random deaths in this facility. My father was not a healthy man but missing dialysis appointments because of not scheduling his transportation to missing meds and not given at the correct times, all contributed to his death. My father died an excruciating death from not being able to have a bowel movement. They dont even know a time of death. They can only tell us it happened between midnight and 4am. They were pumping him with LOTS of laxatives knowing her had renal disease. He called the Health Department more than enough times to prove the errors. He told them he was in terrible pain and something wasnt right, yet not a single doctor came in to talk to him and he wasnt sent to the emergency room after 7 days of not having a BM. Additionally, the morning he died, 12/3/2019, the staff ripped off the sheet off my dads body exposing his face, a sight my sister did not want to see. They never told the transport team that he had passed away so they went into the room to wake him up for dialysis!!! Not to mention, there was fecal matter on the floor at the head of his bed!! My dad rarely had meals that were correct, or available for him to eat. Every dialysis appointment he was left without dinner as the dinner staff would put the food in his room and then take it back before he could come back. My dad was ind and was fed things he was allergic to, when he found out, he would ask for something else. The staff would tell him the kitchen was closed. My dad kept records! NEVER EVER SEND YOUR LOVED ONE HERE!!!

The memory care unit is awful. The staff know when visitors are on the floor because it is a lockdown unit. They act all nice and pleasant when visitors are around, but as soon as they leave its a different story. Scheduled activities are not done and meals consist of cold fried bologna multiple times a week. Check on your loved ones if they are here!!

Tremendous instability, remarkably high staff turnover, staff generally unhappy (and understandably so). The memory care unit staffing is highly erratic and unpredictable - quality of care is steadily declining (the most recent Medicare ratings list the facility as MUCH BELOW AVERAGE in Overall Rating, Health Inspections and Staffing, and BELOW AVERAGE in Quality Measures (see for yourself, go to Medicare.gov Nursing Home Compare). There is really no excuse for a facility to have such deplorable ratings - and there is no indication that the administration has any intention of correcting the deficiencies.

If you intend to place a loved one/family member in this facility, be prepared to do battle to ensure that the care you are paying for is the care that is delivered. Quite a shame, as this was once a very nice facility, staffed with competent, caring professionals...that is no longer the case.
