Family Bridges is a non-profit organization that provides empowerment programs to those in need. With a mission to improve the quality of life for individuals and families, Family Bridges offers a range of services that cater to different age groups and needs.
One of the flagship programs offered by Family Bridges is the Hong Fook CBAS Center. This program provides culturally sensitive adult day health care services to seniors with chronic medical conditions, disabilities, or cognitive impairments. The center offers a safe and supportive environment where seniors can receive medical care, socialize with peers, participate in recreational activities, and enjoy nutritious meals.
In addition to the Hong Fook CBAS Center, Family Bridges also offers Social Services Programs that provide assistance with housing, transportation, legal issues, immigration matters, and other social needs. These programs are designed to help individuals and families overcome barriers that prevent them from achieving their full potential.
For seniors who prefer more independent living arrangements but still require support services, Family Bridges operates two Hong Lok Senior Centers in Oakland Chinatown and San Francisco Chinatown. These centers offer a variety of activities such as exercise classes, arts & crafts workshops, computer training sessions as well as hot meals served daily.
For working parents who need affordable child care options for their children while they work or attend school during the day time hours; Lake Merritt Child Care Center is an excellent choice. The center provides high-quality early childhood education for children aged 2-5 years old in Oakland's diverse community.
Another program offered by Family Bridges is Friendly Visitors which matches volunteers with isolated seniors who may be lonely or have limited mobility due to physical limitations or language barriers. Volunteers visit these seniors regularly providing companionship while engaging them in meaningful conversations about their interests or hobbies.
Finally Health Home Program helps people manage chronic conditions like diabetes or heart disease by providing comprehensive care management services including medication management support; coordination between primary care providers specialists hospitals emergency departments pharmacies labs radiology facilities etc.; health education coaching on self-management skills such as healthy eating physical activity stress reduction smoking cessation etc.; referrals for community resources like food banks housing assistance transportation etc.; advocacy on behalf of patients when needed; monitoring progress towards goals set together between patient family caregiver provider team members involved in patient's care plan development implementation evaluation revision if necessary based upon changing circumstances over time.
