Coral trace has had my mom for about 8 years now and has been like family. I had been there visiting several times a week for those years until Covid came and never did I see anything I didn t like. Always pleasant always clean and my mom calls it her home. The care has been right on top of it with any concerns and always keep me updated. Wouldn t have her anywhere else.

All reviews are based on personal experiences and mine and my mothers who is living in assisted living in Fort Myers has been here at least 4 times each for lengthy stays to rehab , yes it is a older building , it is clean you can bring your own sheets and have option to do your loved ones laundry which to me was good to give my mother comfort , the nurses are very nice along with the nurses aids who treated my mother with kindness , each time she came back they remembered her and came to her room to see her and talk to her, they know my routine and to me and my husband . There Rehab team is great and they push her to bring her independence back, to where every time she was there she did well when transitioning...and they are very kind and truthful .. so my review is based off of personal experience , she is there now and I requested she go there from hospital , she's comfortable and 5 minutes from me ... so thank you to staff for taking care of my mother!

I want to start off by saying I am a Medicaid provider, I am a social worker for the developmentally disabled. I placed my father at this location because out of Consulate (sister facility) or Coral Trace in Cape Coral payor is Medicare Well Med/United Healthcare. I had previously researched this facility in 2016 when my dad had a stroke for rehab. I opted to do PT at home. Since then the reviews have changed due to Coral Trace employee s have rated this facility high! In the hospital the rep from Coral Trace said yes we are working on that! Literally it s the employees bumping the rating up! My dad was there less than 24 hours and I literally removed him from the facility by telling staff I was jail breaking him out! They laughed and thought that was a funny phrase.
The nurse that my dad had on admission was like RN Ratchett, she was old crabby and had horrible demeanor! They are writing on scrap paper and on anything put official forms! Based on the screaming patient with dementia across the hall and the poignant stench of urine from the top of the hallway I was certain I had made a mistake! Do yourself a favor and run far away from this facility! There are staff that care I am sure but if you truly care about your family member don t leave them here! Do your research! There is no way that the rehab patients should be in with the ones with a long term admission! It s traumatizing and it s uncomfortable for the patients. The equipment, beds and bedding and the lighting is from the Stone Age! I did not see any abuse, neglect or exploitation however if you do please call 1-800-96-ABUSE to file a report! You can remain anonymous!

Do not put a loved one I Coral Trace. They can take a healthy woman of 84, no meds, very independent. Reduce her to a skeleton. Verbally says she wants out and to go home. They threaten families with Dr Nassar of not having to write for meds. Only on eyedrops. So not a big deal. Won't write for HH. We can get HH on our own for Ma. It's like a 'house of glass". Going In is easy....getting out is tough. DO NOT USE CAPE TRACE.

DO NOT Put your family member here!!!!! My mother got placed here and she is very aware of what goes on. Her husband came in during the weekend to see her. She said that she has been calling with the call light but nobody comes. Her boyfriend stood behind the door and told her to push the call light. They did this for 30 to 40 minutes and finally somebody came. She yelled at my mother and told her that she could not press the button like that. My moms boyfriend jumped out from behind the door and gave her a piece of his mind. The place is nasty!!! He walked in the bathroom that is in my mothers room and there was a wash clothe and towel with feces on it and urine on the bathroom floor. This place needs to be shut down!!!!

My mother was there for a couple of weeks. I don't know why there are all these wonderful reviews I thought the staff was horrible. The nurse would never answer any questions I had. The place smells like one big toilet which makes me think that they are understaffed and not caring for other residents. I could not visit because the odor would make me gag and sting my eyes. The receptionist was to busy talking about nothing with someone while we waited for service and then she was rude. The PT department ordered equipment that is not necessary for my mother. Never again. The last place my mom was further away but had none of these problems. If she needs rehab again she won't be going there! Discharging her was unprofessional and paperwork was missing.

My mother was placed here after being in the hospital for 7 weeks. She was fine at discharge but once she was brought here she started declining. I went in one morning to find her with no sheets, her nasal cannula for her oxygen was on the side of the bed, she was unresponsive and her eyes were rolled back. When I spoke with the nurse, she told me that she has been unresponsive "for at least an hour". She stated she took her vitals and she was fine, but when I asked what her temp was she couldn't tell me. She clearly was hot at touch and was having a hard time breathing. I told her to call 911, which she should have done in the first place. I also never recieved any phone call about her status and was lucky that I went in as early as I did. Once at the hospital, we found out that she had a fever of 101.5, had sepsis due to an infection from the Foley catheter that this place put in, and also had multiple ulcers on her backside and front pubis area from not being cleaned properly. She had to be intubated and was admitted into the ICU. THEY DO NOT GIVE PROPER CARE TO THEIR PATIENTS AND THIS INCLUDES THE DOCTORS AS WELL AS THE CNAs and LPNs. DO NOT TAKE ANYONE YOU LOVE HERE!!! If I could give 0 stars, I would.

Well, I guess I was one of the lucky ones. They say the squeaky wheel gets the grease; I'm a squeaky wheel! I came to Coral Trace 6 months ago, recovering from surgery I had for a pressure ulcer. My nurse, Trish, gave me my meds on time, and always had a smile and a friendly joking manner. I also probably had the best CNA on the floor, Jenny. I practically begged her to wash my hair, and she did so every other day. Otherwise, she would just put it up in barrettes. She had a positive attitude, and was in such popular demand, as everyone kept calling her name to help! I give the food a big thumbs down, though. It was absolutely inedible. Most times, unrecognizable!

I visited Coral Trace and was given a tour and was very impressed with the facility and how friendly and professional the staff was. Everyone I talked to went out of their way to make me feel welcome. This is the kind of place I would send my loved one to.

I was so impressed with the office staff, rherapists, nurses, and CNA's. The facility is very clean. Upon my mother's admission I was greeted by guardian angel, April. She handled all my concerns immediately, and really listened and answered all my questions. The therapy center is phenomenal. Mom got strong and came home...unfortunately, life happens and she fell. I immediately told the Dr's at the hospital that I wanted her to go back to Coral Trace. They told us upon her 1st discharge that we would be more than welcome to come back if we needed help again. I felt so comfortable knowing she received the best attention and care that's why I chose to send her back there. I definitely recommend this facility to anyone. The office staff are cheerful and welcoming. The nurses and CNA's continually check in on the patient. I cannot say enough good things of the therapy. So here we go again...mom is in a great place for recovery. I hope this review is shared with the staff. They deserve to hear the positives of our experience. Thank you Coral Trace!!!

Recently, my family members and I have been searching for a health care facility that is actually good! Luckily, we came to Coral Trace.
Especially Sharon M. She answered the questions we had, needs we needed filled and just overall a great person and experience. If Coral Trace is half as good as their staff, we are going to be in good hands.

I have worked at this building for over 3 years and its had some challenges, but over the last year its been a pleasure to work there. I work with some of the most passionate, knowledgeable, respectful, and genuinely caring souls that you could ever ask for in teammates.

My brother is here and pushed the call button for over an hour with no response the light does nothing but flash no sound at all to alert the staff of the need for help after being there for several hours he was given only one pill of his meds as opposed to the 2 every three hours at the hospital. My wife and I called a total of 9 times until someone answered the phone and the best they could do was say we will go check on him at which time he was told they would say something to the other nurses. This is not the way a healthcare facility should be run. Even the administration seems uncaring.

I have been an employee since September 2013 in this facility and with Consulate for 11 years this month. This facility from the moment the lobby doors open you will be graciously greeted by our very own Fran Provenzano Director of first impressions as her so deserving plaque states. This is definitely one of the most welcoming warm environments you will ever step into. From the holiday decorations, to the smiling faces, to the knowledgeable staff, to the understanding supervisors, it is just an excellent place to work. There is no other place I'd rather spend my days then here with my 2nd family.

I would like to express my great appreciation for CNA Ste-Heleine Joseph. She was kind & helping with anything and everything I wanted. Not only did she go way beyond expectations- she was so friendly, always smiling and never made me feel like I was bothering her. She is truly an "Asset" to this facility. I had her for 2 separate stays here and was a pleasure even just to see her smiles.

I took a tour of the building and was impressed. Yes, it is older, but very clean, the staff was warm and friendly and professional. The head of nursing was very knowledgeable and answered all my questions. There were no smells and the residents all looked well taken care of. Several were in the courtyard planting flowers in the garden. They were laughing and having a good time. I would send my loved one here.

I highly recommend Coral Trace because of the wonderful staff. They are all very professional, friendly, and willing to take care of any questions or concerns. Everyone works hard to make it a clean and comfortable environment for residents and their loved ones. Go take a tour and you ll understand that it really is a great facility.

I worked there for over 6 months before I finally quit. They treat the employees like they are worth nothing. Half the CNAs speak little english, and most show no compassion for their patients...those that do get taken advantage of. They barely clean anything, many of the CNAs do not practice basic health safety precautions. I would NEVER put one of my loved ones in this place.

This place is horrible. It stinks of urine and the care is way below average. Discharged my friend to home and he could even make it to his front door, started falling. Called EMS and went to hospital with pneumonia.

I ve been interviewing facilities on behalf of a loved one who needs additional care, and after touring Coral Trace, I was extremely impressed. Namely, a professional, welcoming staff and inviting atmosphere. Great first impression. I would recommend to anyone researching care facilities for their family or friends.

This place is awful and the Staff don t care about the Residence. My brother in law is there now n he was ringing the help bottom. It took my hubby n I to call about 9 times to get a hold of anyone to go help him. So if I could give a zero I would.

I will start with the rehabilation portion of this facility was good and they cared and did their jobs well, now here is the bad part. If you can not be there to check on the care for your loved one WATCH OUT. Their is a Dr Nassiff that prescribes meds as he pleases against the health surrogates wishes. They drug patients and put them in the hallway if they have memory health issues, I gues this is how they treat dementia. PATHECTIC
Ring the bell for help and it could be 5 minutes, 15 minutes or no one comes at all. Seating when you come to visit is non-existent. Wound care is nonexistent along with just plain being rough to the Elderly.

This is a POOR POOR FACILITY
