I completed one of my clinical affiliations at this location during PT school and truly enjoyed working with the entire rehab department (PT, OT, SLP) and nursing staff while I was there. The staff truly goes above and beyond when prescribing a plan of care to make sure it is individualized for each patient and is a fun group to work with also!
I was at Apple rehab for a whole year due to an amputation of my right leg below the knee. The facility met all if my needs to having my own rooms to having a shower in my own room. The staff was very professional and also very nice. I have to give the therapy team a five stars by itself Jake and Vanessa were the best that I have ever encountered during therapy they pushed for things to be done so that I can heal faster, they taught me how to maneuver without my prosthetic to be able to be a bit more independent and be able to move around. I have to thank them because with their positive attitudes everyday and their encouragement it made my not so good days into good days! They are an excellent team and really care about their patients needs and how to met those needs!
I am a former PT student of this facility. I had an incredibly enriching experience that truly benefited my career in a multitude of ways. The staff was very supportive as a whole, and each employee demonstrated a lot of compassion towards their patients. I have to say that Jake and Vanessa from the rehab team were AWESOME!! They worked very hard at their job each and every day.
My Grandmother in-law was a resident at Apple rehab Farmington Valley for 5 years. I cannot begin to give enough praise to all of those that cared for her. She passed away in March of this year with loving staff at her side. The nurses aides and nurses became her second family. She loved them all dearly. When she needed a splint on her hand due to contractures rehab came right away to evaluate her and get her a splint to ease her pain. In the last 9 months of her life she refused to leave her room. The recreation staff visited with her on a regular basis to provide her with stimulation. Sometimes with music , sometimes with gentle touch and sometimes just chit chat. She loved the food and all of the staff that cared for her. She had a great sense of humor and when visiting I would see the great interaction and the laughter that was shared with staff. I cannot recommend this facility enough. I have such deep gratitude toward everyone that made her quality of life the best it could be. Thank you to all of them!
I can't say enough good things about Apple Rehab Farmington Valley. My mom received excellent care and the residents rooms are private and very spacious! No other building we toured had such large, private rooms. We especially loved the cafe/bistro, which made it feel more like a hotel than a nursing home! I highly recommend Apple Rehab if you are looking for a caring and attentive rehab facility!
I went to visit a relative here and the staff was not very warm at all. Cold, in fact. Not what I was expecting, very disappointed.
The new administrator Laura is doing a great job. I have had very good experiences with the therapists at Apple rehab.
My love one was at this facility 3 to 4 weeks ago for rehab. The nursing staff was wonderful and provided great care therapy was phenomenal. My family all agreed we would return if we would need services again
My father arrived today from the hospital. Tonight he was in lots of pain and no nurse in sight. I went to front desk asking to see his nurse, they indifferently paged her. she never showed up. The level of indifference was freighting.
The last thing your family needs is a weed smelling nurse sitting on the front desk doing nothing when ppl needs them. We are so out of here. Save your money and look for a facility that would really care about your love ones.
The nurses are great people as well as the aids but here at this location that I know of aren't that great at taking care of the long term patience it's pretty ridiculous.
I believe Apple to be one of the best companies I have ever worked for since becoming a nurse in 2010. I worked here my first year as a nurse and then returned 7 years later. They truly care for their employees. I am very happy being employed here.
Please do not send your loved ones here, at all costs. Staff was incredibly rude - we went elsewhere after only a few atrocious days.
They are not equipped to take care of dimensia patients. Short staffed and all staff not trained adequately. No common sense. Very poor hygiene care and communication. Never saw the room get cleaned.
Too many issues to list! Thank God for a nice roommate that looked out for my family member when we weren't there! I would not recomend this place!
My dad has been here for short term care for a week now, has quite a few weeks to go. He has a private room that is well appointed and very clean. The aides, nurse, and staff have been nothing but pleasant, helpful and attentive. The food is varied and he loves it. I feel at ease knowing that he is a few minutes from my home and he is being well cared for. He has therapy every day and loves all the therapist, as well as, the Doctor. His favorite nurse is Kerry who checks in on him daily. We are grateful for Apple Rehab short term rehab. Highly Recommend!!!
My mother has been there for 2 1/2 years. She is very happy. She enjoys the food, thinks the place is well kept and appreciates the friendliness of all of the staff. I think so, too. They take very good care of her.
Where do I begin
1st of all the pictures look nothing like the facility. Its out-dated ,the halls reeked of bleach ,the repair work in the rooms is horrible.
The patients are given a styrofoam cup of ice water in the morning due to the fact that are no pitchers, that they can fill and leave in the patient's rooms .Also you cannot even get a simple box of tissues for your nose or whatever. They simply do not provide them .
There are a lot of costs being cut at our love cost .They also run out of food items and if your family member doesn't happen to eat the alternative or if they should not have one they will be hungry till the morning. Unless someone bring them something. If they get what they order for breakfast...
As for their physical therapy department I say kudos to them, they do provide very very good physical therapy. As far as the rest of the facility well that remains to be seen. Also forget about asking your aid or someone to get something for you because it simply will not get done they do not come back, with whatever you may have requested.
So I would look into this facility long and hard before you place a loved one that may be there for long-term or who may have a voice in their stay as to their care. If you have a loved one that is silent and we'll be quiet and just simply can't express the situation it may not be the place for them .However I would want someone to have a voice for my loved one should they not be able to speak up.

The therapy has been very helpful to me..It has taken from a wheelchair to a walker, and from not speaking at all to clearly speaking complete sentences. The atmosphere in positive for the most part, however I don't like people talking different languages to each other.. The recreation dept includes everyone in every activity, and puts forth a wide range of activities.Moving from a homebody to a more social environment has helped me to thrive and Apple rehab is a great part of my success.
Such a warm and welcoming facility. Admissions office focused on making the transition to STR or LTC easy and comfortable for residents and their families!!
I truly enjoyed my stay at this facility after my surgery. The staff were caring and compassionate, and helped me to recover in a short period of time. My room was very clean and the housekeepers did a wonderful job. The nurses and CNA's were very attentive to my needs. The therapists were top notch. I also enjoyed eating in their cafe, the food was very good. I would highly recommend Apple Rehab Farmington Valley
I just had a family member go to Farmington Valley for some rehab and nursing care. He was there for a few weeks and was able to return home to his family. The rehab therapy helped him to become strong enough to go home and the nurses took great care of him.
One of my acquaintances has resided at Apple Rehab in Plainville for over a month now. The care this individual has received has been the Upmost Professional on Every level of Job Titles. Upon getting a release date, the Physical Therapists, Themselves, came to my acquaintance's home to assess the ability for this individual to live Independently. I, Personally, have been a c.n.a for 30+ years. I have worked in facilities from 5 star to 1. All of the skilled caregivers have provided my acquaintance with the Highest levels of Respect. I have visited Apple Rehab now for this residents entire stay. Each and Every staff member: From Administration, D.O.N, to Kitchen, Should be Acknowledged for their Upheld Compassion. I Believe Apple Rehab is, most Definitely a 5 Star facility.
Great company and a fantastic place to work. I have worked here for 8 years and have enjoyed every minute. Our therapy team is top notch!!!
Please, please keep your family members out of this place by all means necessary! Very understaffed and no infection control whatsoever. The dietary department does not give any attention to special diets that people are on, which makes things very difficult for people who have swallowing issues, diabetes, etc. The nurses and aides are overworked and constantly exhausted and clearly don't even get along with each other. The whole company just wants to get as many people in as possible but once you are there, you are treated like dirt! If you walk down the hallway all the call lights are lit up and everything smells like urine. Everything is absolutely disgusting. You would think this facility would be cheaper with all its problems, but its actually quite pricey!
I feel bad for all of the patients and all of the nurses/aides. Clearly this is a VERY poorly managed facility and we will never be coming back.
MY dad had great care at Apple. Attentive nurses, knowledgable staff. They cared about their patients. Would highly recommend this facility.
