This company is either intentionally taking your money with no intention of paying for services on your behalf, or it is incompetent. Either way, if you have any other option, DO NOT GET Blue Care Network to cover your health needs.

I needed physical therapy two years ago, and BCN paid some other company to send me a letter telling me I didn't need any more physical therapy. Which is odd, because the company was in California, and my foot, which was not better was in Michigan. And this company had never seen me, and both my foot, my doctor, and my physical therapist all said I still had a problem.

Last year, surprise, still my foot isn't better, and again this time, they say no more visits are authorized. Which is strange, since I have a high deductible plan, and they haven't had to pay A SINGLE dollar this year for ANYTHING. So they're telling me, and the physical therapist, I don't need any more care, and I'm not allowed to pay for it!?

I call them to find out why, and to see if I can get this decision changed, I was just on the phone with them for 40 minutes. At the end of which, I still don't have satisfaction, they're saying that physical therapist must not have submitted an authorization. This seems pretty doubtful, since the therapist says they did, and I believe them, one they seem honest, and since they're still in business, they know how to bill insurance companies.

Now I'm on year three, after the doctor I saw wanted to do surgery, I got a second opinion, this doctor says physical therapy. I'm expecting of course for them to say I don't need it even though standing, and walking are painful, and running is not possible, and I'm only 41.

Final update: They "paid" aka let me pay for a round of physical therapy and several more visits, again they weren't even paying anything, because high deductible plan, they just didn't deny me and tell me I couldn't pay for more visits to treat my foot.

I pushed for folks at my workplace to vote for a different health plan this year, and we've got one, thankfully. I hope they take better care of me than BCN did, but that won't be hard.

For whatever it is worth, maybe they're working on changing things, as they reached out to my comment (several years after I left it originally), and they did approve me to pay for foot treatment this time around, but I was long over them, life is too short to have your health endangered by a crummy health care company.
