I haven't worked since the end of February. My employer is ready to bring me back today. All I need is an MCSA-5870 form from this office documenting my a1c lab results from May 18 of this year to provide to the DOT medical examiner. I tried all day to get them to provide it for me. Dr. Choi was my doctor but he's not there anymore. I loved this doctor and miss him. I'm so frustrated. I need this form to return back to work and start earning a paycheck again. I will try again tomorrow. I'm not feeling they're any too concerned about it though.
