Facility constantly smells of urine. Floors are stained. It's difficult to get help when needed. Not observant to changes in patients conditions. When there is an accident, I can never get anyone to come in with a mop. Floors are sticky. Food is subpar. No communication between staff and family. It's been a very frustrating experience.
