Fraudulent billing practices and they are charging...

Fraudulent billing practices and they are charging advance for future service and saying that amount are due at 27th week(in my wife case they take $610 by saying your insurance have $500 deductible but i had already part deductible part another provider service) never return any of my left over balance amount. no transference in bill process.. I have all paid receipts and Not sure who i need report about this provide.Can someone pls suggest
