It turns out I was filling in the wrong form after speaking with someone else at the division of taxation. The number provided to me by a previous representative was correct, and I must have taken it down wrong.

Still a crumby agency to deal with. It took four separate calls with four separate representatives to sort out what form I needed to fill out and ultimately submit after being given information that was inaccurate.
