I went to this center for a tax visit and the doctor, before starting the visit, wanted me to sign a privacy document that was not filled in correctly (THE RELATIVE CLAIM NUMBER AND TWO OTHER PAGES WAS MISSING), when I refused, he refused to pay me a visit!
