DISGUSTING! Report to HHS

I had to report this facility to the Department of Health and Human Services due to unsanitary conditions and ongoing staff negligence. After more than a month of back-and-forth, my paperwork remains unresolved—largely because the assigned social workers failed to complete it accurately and showed no due diligence in reviewing or verifying critical information. I even have the CEO on all emails and there has been no resolution. No patient advocacy at all.
