At the end of the 1950s, several East Java Muslim community leaders came up with the idea of establishing Islamic religious colleges under the Ministry of Religion. To realize this idea, they held a meeting in Jombang in 1961. In the meeting, Professor Soenarjo, Chancellor of the State Islamic University of Sunan Kalijaga, was present as a resource person to convey the points of thought needed as the foundation for the establishment of the intended Islamic Religious College. In the final session of the historic meeting, the forum ratified several important decisions, namely: (1) Establishing an IAIN Establishment Committee, (2) Establishing a Sharia Faculty in Surabaya, and (3) Establishing a Tarbiyah Faculty in Malang. Furthermore, on October 9, 1961, the Foundation for Welfare Waqf Board was established by the Faculty of Sharia and the Tarbiyah Faculty which prepared the following work plans:

Conducted preparation for the establishment of Sunan Ampel IAIN which consisted of the Sharia Faculty in Surabaya and the Tarbiyah Faculty in Malang.

Providing land for the construction of an 8 (eight) hectare IAIN Campus located on Jalan A. Yani No. 117 Surabaya.

Providing official housing for professors.

On October 28, 1961, the Minister of Religion issued Decree No. 17/1961, to ratify the establishment of the Sharia Faculty in Surabaya and the Tarbiyah Faculty in Malang. Then on October 1, 1964, the Ushuluddin Faculty in Kediri was inaugurated based on Decree of the Minister of Religion No. 66/1964.

Starting from the 3 (three) faculties, the Minister of Religion deemed it necessary to issue Decree Number 20/1965 concerning the Establishment of Sunan Ampel IAIN which is domiciled in Surabaya, as described above. History records that without taking a long time, Sunan Ampel's IAIN was able to develop rapidly. In the period between 1966-1970, IAIN Sunan Ampel had 18 (eighteen) faculties spread over 3 (three) provinces: East Java, East Kalimantan and West Nusa Tenggara.

However, when faculty accreditation in the IAIN environment is applied, 5 (five) of the 18 (eighteen) faculties are closed to be combined with other accredited faculties and adjacent to the location. Furthermore, with the government regulation number 33 of 1985, the Samarinda Tarbiyah Faculty was released and handed over to the Antasari Banjarmasin IAIN. Besides that, the Tarbiyah Bojonegoro faculty was moved to Surabaya and its status changed to Tarbiyah IAIN Surabaya faculty. In its subsequent growth, Sunan Ampel's IAIN has 12 (twelve) faculties spread throughout East Java and 1 (one) faculty in Mataram, Lombok, West Nusa Tenggara.

Since mid-1997, through Presidential Decree No. 11 of 1997, all faculties under the auspices of Sunan Ampel's IAIN outside Surabaya were separated from IAIN Sunan Ampel to become an autonomous State Islamic College (STAIN). Sunan Ampel's IAIN has since been concentrated only on 5 (five) faculties all of which are located on the campus of Jl. A. Yani 117 Surabaya.

On December 28, 2009, Sunan Ampel Surabaya IAIN through Minister of Finance Decree No. 511 / KMK.05 / 2009 officially as a Public Service Agency (BLU). In the document that was signed on December 28, 2009, IAINSA Surabaya was given the authority to carry out financial management flexibility in accordance with Government Regulation Number 23 of 2005 concerning Financial Management of Public Service Agencies (PK-BLU).

Starting from October 1, 2013, Sunan Ampel's IAIN changed to UIN Sunan Ampel (UINSA) Surabaya based on RI Decree No. 65 of 2013.
