Community Health Connections, Inc. is a non-profit organization that has been providing healthcare services to low-income, underinsured, uninsured, publicly housed and homeless populations in North Central Massachusetts for the past 19 years. Founded on February 25th, 2002 as the only Federally Qualified Health Center (FQHC) in the region, Community Health Connections has been committed to providing high-quality healthcare services to those who need it most.
