will NEVER go back, Treaded my mom like nobody cares. i even have pictures when my mom got released, nobody would help her out the door, she is 82 and uses oxgen, and NOBODY helped her outside or to the car, WORSE PLACE EVER.

My mother was a resident in the nursing center for over three years. The staff is wonderful and we were very happy with the care and services BrookHaven provided. The activity staff are always planning fun things to entertain everyone.
This place was a great place at one time. My mom passed away in October 2020. They took it upon themselves to pack up my moms personal items with in 6 hrs of her passing. They took personal items and throw items away with out my consent. I have been fighting with them to return my moms personal items or pay me for them. Everything in her room I bought for her. So I no every item in her room. My brother was in the rehabilitation center at this time. They did not go down to get him to watch them pack stuff up. They have the nerve to tell me to go buy ever that is missing and give them the receipt. I have done that and sent them copies. Now they tell me that I need to buy the items again and bring in the receipt. I bought everything on Amazon and when it was delivered to my house I sent it back without opening it. I wonder how many family s they have done this too. I had already made arrangements to come pack up her room, but they did not want to wait 2 days for me to do that. My heart is broken over this .
Staff are friendly and the place is kept clean, but they don't seem to give the patients the attention that they should. They are too restrictive when it comes to meals and availability of food and drink. They do call to give medical updates but they don't seem as knowledgeable about the patient as they should. They are too focused on profits and not enough on quality care.
never again given wrong meds. Worst place in ohio .Years ago was great not the same now NO HELP $$$$$$$$$$ wasnt there 2 days got bill from them that fast
The facility and grounds are really nice. It seems the quality of service was also in the past. However, it seems that it has recently sold and is under new ownership. It seems they are cutting everything back as far as possible to maximize profits. The food quality and quantity is worse than anything I have seen and I have been in quite a few facilities. They brought in an untrained large pup to live with the patients. It seems to be damaging their skin. Our parent's health deteriorated significantly after moving to this facility. Promises they made before we moved were ignored after we moved our parent here. I really hope the new ownership decides to turn things around and not ruin this once great community.
My grandfather was cared for several years ago at this facility. We opted to have him stay in the memory support unit for more supervision. Within the first week he'd gotten outside past the coded doors, walked across campus, and was spotted on the ground outside by a resident in independent living. (He was in his 80's w/ walker. He was not fast.) Not a few days later we discovered scratches on his arms. At the time, the facility had a puppy in the memory unit to act as a support animal. Concerned for his skin and resident safety (undertrained, jumping puppy), my mother discussed our concerns with the nurse on the floor. She denied the possibility of the puppy scratching him, and told my mother "he had those when he came in here." After clarifying that he did not (apparently no skin assessment done?) have them upon admission, the nurse told my mother "well that's what I was told." Whoever was in charge of my grandfathers case made a major medication change without our consent. He began to fall after starting the medication. We once found two sopping wet briefs in his room, his mattress wet with urine (made up neatly my staff) and soiled clothes. Several escapes and falls later, he ended up in the hospital with a facial wound requiring stitches. Within three months of living here, my grandfather that fed himself, walked, could speak, complete ADL's w/lmtd assistance was dead. Near the end he was nervous, anxious, needed assistance with most all ADL's and said he "wanted out of this pen" when we asked how he was feeling about his stay. His rapid decline does not line up for us. I know this was years ago, but as I advance my career in the healthcare field I am made sick as I realize just how poor the quality of care was here. Perhaps our story is a particularly bad one, perhaps it is not. But as I see more units and more facilities, I am made aware of poor of an experience my grandfather was shown. I currently work in a memory support unit, and see many different manifestations of Alzheimer's/Dementia and many different levels of care. I understand the stressful demands placed upon nursing/support staff, but the care here was truly unacceptable.My family and myself regret sending our loved one here. It still upsets us to this day to think of how he was treated.
