We took in a stray cat that was so sick and congested it was unable to breathe through its nose. The vet thought there might also be fluid build-up around the lungs and said she could do an X-ray to verify, but also advised us that if she was right, there was not much that could be done for it anyway. We opted to just treat the secondary infections with a simple antibiotic, and wait and see. The staff was very helpful in explaining the options, and didn't try to talk us into doing more or make us feel guilty for our decision. By the way, the cat pulled through and is doing great!
