Failure to provide adequate care, did not check on my mother often considering she was an aspiration risk, next to nurses station, they left her unattended during meals unless I was there, and failed to frequently check on her. I felt my mother was left unattended for too long, resulting in her being down after aspiration or heart attack requiring extreme measures to revive her. Ambulance called, had to be hospitalized and sent home for hospice. I would not recommend sending a family there unless another family member can be there 24 hours a day.
