So beware that you need an ID for a NEWBORN. And the birth certificate does not count. I sent an application through the mail and they sent it back with a denial and didn't say why it was denied. I go to this office and that's when the guy asked for my infant's ID. He tells me I need a medical record. So I ask what it needs, like does it need his DOB, a doctor signature, etc. And this guy just says "I don't see why it's so hard just to get a medical record". I am beyond fed up with this place and the process. PLEASE PRINT EVERY SINGLE LITTLE THING I NEED TO BRING IN WITH ME BECAUSE I AM SICK OF DRIVING ALL OVER TOWN FOR THIS. I have read the list of acceptable documents, there needs to be something added saying you can use a medical record and what it requires. Also, your employees need to work on their attitude.
