Hello, Embassy staff won't see the question you wrote and probably won't answer. But I'll try to briefly document what you need, even though you may already be in America)))
1) 2x5x5 photos
2) Passport (no ID card)
3) For men also a military brochure
4) If you work, then reference from the workplace (this will be considered an advantage).
5) If you are leaving for treatment, you will need to provide the details of the physician to whom you should undergo treatment.
