I am a developmentally disabled adult with a communication disorder. I ve repeatedly requested accommodations to ensure my examinations are a proper account of the symptoms and complications I m experiencing, but continue to be ignored by Evolve.
The location of my psychiatric assessment was changed at the last minute and I was sent into the middle of major road construction, rough terrain and heavy equipment.
I require a communication intermediary but am provided chaperones that have no idea what my needs are or why.
I m experiencing extreme sensitivity to light, seeing floaters, major after image requiring me to cup my eyes. The pain at times is unbearable and always present. I become easily disoriented, confused, poor balance, extreme pain that requires me to lay down for a reset.
Middlesex Mutual Insurance Company requires these assessments with providers that keep changing in order to determine whether I am eligible for further treatment.
