Continually "forget" to send a report of my mammog...

Continually "forget" to send a report of my mammogram to my Ob-Gyn even though I have tried to make sure they have her practice name and address. Now they are giving me the run around on obtaining my previous images in order to have them sent to a different Breast Imaging Center. After 4 different phone hand-offs I get an answering machine that just states to leave a name and phone number. It's bad enough that the med records dept took almost 3 weeks to let me know I had to contact the Radiology dept for the images, but now it looks like getting them in time for my appointment is going to be impossible. POOR customer service.
