A collection notice showed up on my Credit Report ...

A collection notice showed up on my Credit Report last week, for $40 from 2015!! I never got billed for this and further more my insurance fully covers my scans! Saw on the report that the billing is handled by a third party. Not happy!!!!! There is clearly an error.
I mean 3 years ago!!
