HealthFinders Collaborative: Providing Quality Healthcare and Wellness Education to the Community
HealthFinders Collaborative is a non-profit organization that provides quality healthcare, advocacy, and wellness education to people in our community who have limited alternatives. The organization was founded in 2005 by a group of healthcare professionals who recognized the need for accessible and affordable healthcare services for underserved populations.
The mission of HealthFinders Collaborative is to improve the health and well-being of individuals and families in our community by providing high-quality, culturally sensitive healthcare services. The organization operates two clinics located in Faribault and Northfield, Minnesota. These clinics offer a range of medical services including primary care, dental care, behavioral health services, medication management, chronic disease management, women's health services, pediatric care as well as wellness education programs.
At HealthFinders Collaborative clinics patients are treated with respect regardless of their ability to pay or insurance status. The organization offers sliding fee scales based on income levels so that everyone can access quality healthcare without financial barriers. In addition to medical care services offered at the clinic locations Healthfinders also provides outreach programs such as mobile dental clinics which travel throughout Rice County providing free dental screenings for children.
The team at Healthfinders collaborative consists of highly skilled medical professionals including physicians (MDs), nurse practitioners (NPs), dentists (DDS), hygienists (RDH) , licensed clinical social workers(LCSW) , registered nurses(RNs) , certified nursing assistants(CNAs) among others . They are dedicated to providing compassionate care while ensuring that patients receive comprehensive treatment plans tailored specifically for their individual needs.
In addition to its clinical operations Healthfinders collaborative also offers various wellness education programs aimed at promoting healthy lifestyles within the community. These programs include nutrition classes led by registered dietitians(RDs), exercise classes led by certified personal trainers(CPTs), smoking cessation support groups led by licensed counselors(LPCs).
Healthfinders collaborative has been recognized nationally for its innovative approach towards improving access to quality healthcare among underserved populations. In 2019 it was awarded "National Quality Leader" designation from HRSA(Health Resources & Services Administration). This recognition highlights organizations that have achieved exceptional performance on clinical measures related to chronic disease management such as diabetes control rates or hypertension control rates .
