Plan Forward provides a dental membership platform designed to offer in-house membership plans to patients that do not have traditional insurance which ultimately gives more patients access to quality care and loyalty to quality providers. The company partners with dentists, group practices, and dental practice office managers to create and power their in-office dental membership plans. Plan Forward was established in 2018 and is headquartered in Indianapolis, Indiana.
