CoachMe Health is a nonprofit organization that offers free health coaching to low-income patients with diabetes and heart disease. It aims to fill a critical gap in the healthcare system by helping turn a diagnosis of diabetes or high blood pressure into step-by-step action plans patients can follow. CoachMe Health works with patients on Medicaid and without insurance. Its health coaches remotely support patients over text and video, to build healthy lifestyle habits that can change and lengthen their lives.
