Drove over 100 miles to this place trying to get a document. Upon arrival, I was declined because I was the spouse of the person needing the document and my wife did not go with me. I didn't know she needed to go because the application did not state the document could only be given to the requesting party. I asked if I could leave the application my wife signed there to begin the process and was told no it would have to be mailed back. I also tried to request the document on a separate application I filled out and signed just to be on the safe side. That was not acceptable either although the application had under requesting party a block for other and you fill in your relationship to the party where I wrote son-in-law. To say I was frustrated is an understatement. The application forms need to clearly state the procedure instead of finding out the way I did.
Got back home and mailed the information and will have to wait about 2 weeks for a response.
This is pathetic!
