My mom has had disabilities for most of her life. When she moved to this valley appx 2010, she was introduced to 5 Counties. I don't know all the details from back then, but I do know they were able to help her connect with different programs that made it so she could live independently. She absolutely loved and appreciated her case worker and if it weren't for them, I don't know what she would have done.
Years later as she declined, we switched from one 5 Counties Program to another, that is when I was involved.
Lisa was her second case worker and that women was such a blessing. She guided us thru all the changes and programs that my mom was qualified for and she was so friendly and helpful. It was nice to have someone with a lot of knowledge hold my hand so I did not feel all alone. I don't know what I would have done without her. Eventually I had to put my mom in a nursing facility, so we had to leave 5 counties. That was a scary time, since I was used to being under their umbrella.
My mom just recently passed and I wanted to take a moment to thank 5 Counties for EVERYTHING. What a great resource for our community.
