They are fraudsters. They say they cover preventative care, but when I submitted a claim for reimbursement after getting a flu shot, they said it was out of network (how is buying a flu shot at a store out of network?), and applied the amount they were willing to cover to my deductible, even though their plan summary says preventative care applies before reaching the deductible. Appealed with proof of this and they still deny it, and don't even respond to half of the issues in the appeal.
