Poor customer service at front office, manager alw...

Poor customer service at front office, manager always with an attitude, don't have the patience to explain.
Today I went in and with the news that appointment was canceled "we called you to both numbers," I put my phone in front and I showed no missed call and no voice-mail. I even checked my Verizon call log and no incoming call from them.
How can I ask for a manager if she has a bad attitude?
I know for a fact that they have a note already saying that I give them attitude, but how am I not going to because they can't simply can do their job, ts either something with my insurance, I always have to call and apparently it's the dentist office not the insurance or they either cancel and say they called when they don't.
I've worked in customer service more for quite some time and they are not even close to a 2 star here.

Orthodontist is different, could be because they deal with less people and at the dental group they can't deal with pressure plus they are always busy and phones are always ringing, maybe they need more staff and stop giving attitude!
