Don't accept 45 minute appointments. You get charged ($106) per 30 minutes, so insist on either 30 or 60 minutes to get the appropriate value.

Mediocre medical care. For example, between two visits that generated 8 ICD codes (4x ICD-9, 4x ICD-10), 3 were medically/anatomically incorrect (citing tissue injury outside of the area of injury) and 1 was a logical fallacy (billing for an "initial visit" on an nth followup). Obviously I can't be sure, but this seems either sloppy or like they are trying to generate revenue by billing for non-existent conditions.
